An update on shellfish allergy
Purpose of review Shellfish is an important cause of food allergy worldwide, and a major cause of food-triggered anaphylaxis. Despite the wide variety of shellfish, there is considerable serological and clinical cross-reactivity of major shellfish allergens, and accurate diagnosis remains a challenge in the management of shellfish allergy. Recent findings Novel minor allergens have been discovered and characterized, and advances in component resolved diagnostics have provided insights into the prevalence of sensitization and their clinical importance in shellfish allergy. The extensive cross-reactivity between tropomy...

Occupational allergy to dog among police dog trainers.
This study was aimed to reveal the prevalence of dog allergy and other common allergy and allergic symptoms in police dog trainers. Fifty-six police dog trainers and 150 workers as control group were included in this study. Medical records of dog trainers including respiratory, skin, eye symptoms and physical examinations and skin prick test results are compared with the medical records of control group. Positive SPT to dog was present in 21.4% of dog trainers, whereas the frequency of sensitization to dog in the control group was 1.3% (p minor 0.001). Dog allergy development risk is found 20 times greater in dog trainers ...

Clinical Features and Culprit Food Allergens of Korean Adult Food Allergy Patients: A Cross-Sectional Single-Institute Study.
CONCLUSIONS: Wheat, fruits with or without PFAS, and crustaceans are important and frequent causative allergens in Korean adult FA; these allergens differ from those found in childhood FA. It is notable that non-classic allergies, such as PFAS, FDEIA, and α-Gal allergy, are the important causes of anaphylaxis in Korean adult FA. PMID: 31332982 [PubMed]

Drug allergy is associated with the development of extraintestinal manifestations in patients with ulcerative colitis.
Authors: Toledo-Maurino JJ Abstract Summary: Drug allergies are developed by antibody or cell-mediated reactions as immunologic mechanisms. It has been demonstrated that hypersensitivity reaction to certain allergens may play a role in the pathogenesis of inflammatory bowel disease (IBD) focused on food allergies. A total of 256 patients with UC were divided in two groups: 203 patients with active UC and 53 in remission UC were included in the present study. In the present study we found that 11.7% had allergy to at least one drug distributed. The most frequent drug-allergies were sulfonamides in 2.8% and penicilli...

The Negative Predictive Value of 5-Day Drug Provocation Test in Nonimmediate Beta-Lactam Allergy in Children.
CONCLUSION: The 5-day prolonged DPT has high NPV and seems appropriate in duration for children with suspected nonimmediate-BLA allergy. PMID: 31923549 [PubMed - as supplied by publisher]

Achieving The Quadruple Aim To Deliver Value-Based Allergy Care In An Ever-Evolving Healthcare System.
CONCLUSION: As healthcare systems continue to evolve, allergists and immunologists will play a key role in optimizing value by translating emerging evidence into practice and communicating novel approaches to prevent and treat allergic diseases. PMID: 32289524 [PubMed - as supplied by publisher]

The economics of drug allergy
Purpose of review As drug allergy research aims to inform clinical practice, implementation of best practices may be influenced by financial resources required to incorporate new interventions and the resulting clinical and economic returns on those resource investments. The present review summarizes new insights into the economics of drug allergy over the past year. Recent findings While considering economic implications of recent drug allergy research, many studies have addressed different contextual factors related to the setting, provider, or outcomes. Advances in technology have enabled specialized allergists to ...

Skin testing and oral amoxicillin challenge in the outpatient allergy and clinical immunology clinic in penicillin-allergic pregnant women.
CONCLUSION: Penicillin skin testing and oral challenge in pregnant women can safely be performed in the outpatient setting. There was no difference in intrapartum antibiotic use between women who were or were not challenged. Further research is needed to determine the utility of oral challenge in pregnant patients. High anxiety and health-related quality-of-life in families with children with food allergy during COVID-19.
CONCLUSION: Mothers with food allergic children reported high anxiety and poor HRQL. Yet, qualitatively, day-to-day food allergy management was better during the pandemic. PMID: 32950685 [PubMed - as supplied by publisher]

Cow's Milk Allergy Prevention.
CONCLUSION: Further research regarding the role early cow's milk ingestion plays in the prevention of cow's milk allergy is required, and questions remain. The absolute amount required for tolerance is unclear. Studies in higher risk populations are required. However, there is an intriguing and increasingly more pervasive association between early ongoing cow's milk ingestion and cow's milk allergy prevention. Cannabis attitudes and patterns of use among followers of the Allergy & Asthma Network.
CONCLUSION: Cannabis was used by <20% of respondents with positive effects more frequent than negative effects. Half of cannabis users with uncontrolled asthma smoke cannabis, but a minority of physicians inquire about its use. PMID: 33465453 [PubMed - as supplied by publisher]

Coconut Allergy: Characteristics of Reactions and Diagnostic Predictors in a Pediatric Tertiary Care Center.
CONCLUSION: Although the rate of reactivity to coconut in sensitized individuals is low, half of reactions from consumption met criteria for anaphylaxis. Clinicians should be aware of the spectrum of reactions and diagnostic utility of sIgE and SPT. PMID: 33548470 [PubMed - as supplied by publisher]

Emerging approaches in the diagnosis and therapy in shellfish allergy
Purpose of review Despite the high prevalence of shellfish allergy, the clinical management of seafood allergy has remained unchanged over decades. Here, we examined the current status in the diagnosis and clinical management of shellfish allergy and highlighted the imminent need for more specific diagnostic methods, as well as effective and safe therapeutic approaches for shellfish allergy. Recent findings With the advancement in the molecular identifications and definition of reactive epitopes of shellfish allergens, new diagnostic designs such as component-resolved diagnosis, basophil activation test (BAT) an...

Anaphylaxis across Europe: are pollen food syndrome and lipid transfer protein allergy so far apart?
Purpose of review Traditionally pollen-food syndrome (PFS) is considered to be a mild cross-reacting food allergy affecting only Northern Europe, with lipid transfer protein (LTP) allergy being more severe and mainly occurring in Southern Europe. This review seeks to update the reader on both types of plant food allergy and to determine whether the stereotypical presentations of these plant food allergies remain the same, with a particular focus on reaction severity. Recent findings Recent findings suggest that both these types of plant food allergy occur in children and adults. Although it is true that PFS alle...
