Primary pulmonary diffuse large B ‐cell lymphoma presenting multiple nodules mimicking metastasis: A case report
We describe a case of pulmonary lymphoma with multiple nodules mimicking metastases in a  treated patient with rheumatoid arthritis. A 73-year-old man was diagnosed with rheumatoid arthritis at the age of 30. He was treated with leflunomide. He was followed up for a nontuberculous mycobacterial infection. He underwent percutaneous coronary intervention for acute myocardial infarction a t the age of 70. In April 2022, routine follow-up revealed new-onset multiple nodules on chest computed tomography (CT). A position emission tomography/CT scan with 18F-fluorodeoxyglucose showed a low-high maximum standardized uptake value ...

Sarcoidosis presenting as optic neuritis with vision loss
We present the case of a 51-year-old man who complained of right vision loss. Brain magnetic resonance imaging showed asymmetrical enlargement of the right optic nerve. Chest computed tomography detected mediastinal and hilar lymphadenopathy. There were cutaneous nodules on the back. Biopsy of the mediastinal lymph node by endobronchial ultrasound-guided transbronchial needle aspiration and the skin showed noncaseating granulomas consistent with sarcoidosis. Serum angiotensin-converting enzyme level was elevated (34.2  IU/L) (normal: 8.3–21.4 IU/L). Based on these findings, he was diagnosed as neurosarcoidosis with opt...

Improved ventilatory response during exercise over time after concussion: A case report
Our case report details the ventilatory response during exercise of a 17-year-old female who suffered a sports related concussion and subsequently post-concussion syndrome. On exercise test 1 (two-years following concussion), the striking abnormality was hypoventilation, however on repeat exercise test two-years later the ventilatory response was normal. Improved ventilatory response following concussion may be related to improved CO2 sensitivity or neuroplasticity remodelling the ventilatory response during exercise. AbstractThis case study reports the exercise ventilatory responses of a 17-year-old female who sustained a...

The effect of lung recruitment maneuverer in COVID ‐19 induced acute respiratory distress syndrome
This case report discusses the effect of recruitment manoeuvre in an ARDS patient who was infected by COVID-19. AbstractCoronavirus disease-2019 (COVID-19) is a highly spread infectious disease around the world. This infectious disease impacts whole body systems, specifically on respiratory system. This 57-year-old women had diagnosed COVID-19 positive and progress to acute respiratory distress syndrome (ARDS) within 1 week. Mechanical ventilation with protective lung strategy, prone position could not reverse the worsen hypoxemia and bilateral lung infiltration. Recruitment manoeuvre was proceeded with 40 –40 strategy a...

Bochdalek hernia masquerading as pleural effusion in a young adult
We report on a 25-year-old man admitted with 3-year recurrent and self-limiting abdominal pain and vomiting. Chest radiograph showed left pleural opacity which shifted position in the decubitus film suggesting pleural effusion. Before attempting drainage, point-of-care (POC) thoracic ultrasound (TUS) demonstrated features of intrathoracic gastrointestinal content above the left hemi-diaphragm. Computed tomography of the thorax confirmed a large left BH. Acute recurrent symptoms 3 months post-discharge was associated with BH enlargement and early bowel ischemia, which was successfully managed by emergency surgery. This case...

Upper gastrointestinal haemorrhage caused by duodenal metastasis from primary lung cancer: A case report
An extremely rare case of upper gastrointestinal haemorrhage caused by duodenal metastasis from primary lung cancer. AbstractDuodenal metastases from primary lung carcinoma are uncommon. They usually occur in terminal-stage disease. Bleeding, as the first presentation of duodenal metastases, is rare. This case reports a rare mechanism of upper gastrointestinal bleeding due to a metastatic involvement of the duodenum and gastroduodenal artery. A 58-year-old man with a past medical history of pulmonary carcinoma presented an episode of hematemesis of great abundance with melena. On physical examination, he was afebrile and p...

Selective IgG2 ‐deficiency in yellow nail syndrome
We present a case of isolated IgG2-deficiency in a patient with Yellow Nail Syndrome (YNS), manifested by yellow-green nails, bilateral pleural effusions, pericardial effusion, bronchiectasis, and lower extremity edema. Given the rarity of the condition (estimated prevalence<1/1,000,000) and lack of thorough population-based studies, association of new or rare manifestations with YNS relies heavily on individual case reports. Key messageIgG2-deficiency increases susceptibility to recurrent pulmonary infections and the risk for bronchiectasis. Isolated IgG2-deficiency has not been previously described in Yellow Nail Synd...

A case of prolonged COVID ‐19 treated with tixagevimab/cilgavimab
A 54-year-old patient with COVID-19 had a medical history of follicular lymphoma treated with obinutuzumab, an anti-CD20 monoclonal antibody. This prevented the production of antibodies against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), thus resulting in persistent pneumonia. Administration of 300 mg of tixagevimab and cilgavimab markedly improved the patient's general condition, and led to remittance of pneumonia. AbstractA 54-year-old woman presented to our hospital with a fever and cough. The patient had a medical history of follicular lymphoma treated with obinutuzumab. She was infected with an omicr...

Treatment of refractory localized pulmonary nocardiosis caused by Nocardia mexicana with a combination of medication and surgery
This report shows that the treatment with combination of medication and surgery was useful for treating refractory localized pulmonary nocardiosis caused byNocardia mexicana. AbstractPulmonary nocardiosis is a rare disease that is often difficult to cure because of its tendency to recur. Here, we report a case of refractory localized pulmonary nocardiosis caused byNocardia mexicana. A 60-year-old Japanese woman had recurring pulmonary nocardiosis four times previously and each time she was treated with antibiotics for a sufficient duration; nevertheless, the disease continued to recur, probably because of resistance to ant...

Novel technique of performing transbronchial lung cryobiopsy (TBLC) for diagnosing diffuse parenchymal lung diseases (DPLD) in infants
We present the first case of infant lung cryobiopsy done using rigid tracheoscope in a suspected case of childhood interstitial lung disease. AbstractChildhood interstitial lung disease (ChILD) diagnosis often requires surgical lung biopsy after the common causes are ruled out. However, surgical lung biopsy has its own set of complications. Hence transbronchial lung cryobiopsy has been used in various studies of adult interstitial lung disease (ILD) with good yield and minimal complications. But this newer mode is rarely used in diagnosing children with suspected ILD. Here, we present the first case of the use of this tech...
