Pulmonary extra ‐nodal mucosa‐associated lymphoid tissue (MALT) lymphoma: A rare cause of persistent lung consolidation
We present a 46-year-old lady with persistent right lower lung consolidation despite earlier treatment efforts with intravenous antibiotics for community acquired pneumonia. Apart from initial presentation with a short 3-day history of fever, cough and shortness of breath, she had remained largely asymptomatic throughout the follow-up period. Flexible bronchoscopy done ruled out infectious aetiologies but transbronchial lung biopsies showed atypical lymphocytes. A computed tomography guided core biopsy of her right lung consolidation was subsequently performed, confirming a diagnosis of pulmonary MALT lymphoma. She was pro...

Successful treatment with atezolizumab in a haemodialysis patient with large cell neuroendocrine carcinoma
In this report, a modified regimen based on IMpower 133 (carboplatin  + etoposide + atezolizumab) was administered to a patient diagnosed with large cell neuroendocrine carcinoma (LCNEC) who was concurrently undergoing haemodialysis. Adverse events led to the discontinuation of carboplatin and etoposide after the first course. Nevertheless, the patient exhibite d reduction in pulmonary nodule and adrenal metastasis while receiving atezolizumab, indicating its sustained efficacy for a duration of 7 months. To the best of our knowledge, this is the first documented case demonstrating successful treatment with atezoli...

Eosinophilic pneumonia developed after dupilumab administration in a patient with atopic dermatitis
A 63-year-old woman with refractory atopic dermatitis developed a cough 4  days later, sputum, and a slight fever 2 weeks later. Chest x-ray and computed tomography scan showed infiltrative shadows with surrounding consolidation of both upper lobes. Bronchoalveolar lavage fluid eosinophil count was increased (50.0%), and histopathological findings were consistent with numerous eosinophilic infiltrations. AbstractA 63-year-old woman with refractory atopic dermatitis started treatment with dupilumab. She developed a cough 4  days later, sputum, and a slight fever 2 weeks later. Laboratory test results showed a blood ...

An effective case of bronchoscopic balloon dilatation for tuberculous bronchial stenosis
This study presents the case of an older patient with tuberculous bronchial stenosis treated with repeated balloon dilatation to aid clinicians in persistent atelectasis and respiratory failure. AbstractEndobronchial tuberculosis often causes bronchial stenosis. Balloon dilation is a minimally invasive and effective bronchoscopic intervention for bronchial stenosis; however, reports on balloon dilation in older individuals are limited. We present a case of a 77-year-old woman with endobronchial tuberculosis and clarify the efficacy and safety of balloon dilation. She presented with dyspnea, right lung atelectasis, and resp...

A case of bilateral pneumothorax following computer ‐tomography guided transthoracic biopsy in a woman with suspected pulmonary cancer
This report describes a patient-case where a woman had undergone a computer-tomography guided biopsy. Approximately 4 hours following discharge the patient was admitted to the emergency ward with severe chest pain and dyspnea. Chest x-ray revealed bilateral pneumothorax and subcutaneous emphysema at the biopsy site. Pleural drainage was administered on the patient's right side. Another chest x-ray following drainage showed regression of pneumothorax on both sides thus indicating communicating pleural cavities. Medical history revealed that the patient had been thymectomized 2  years earlier and a computer tomography visu...

A case of tracheobronchopathia ostochondorplastica
We present an interesting case of a 55  year old man who presented with history of weight loss, cough and right sided chest pain. CT showed right upper lobe cavity. He was evaluated with possibility of TB, malignancy or vasculitis. Fibreoptic bronchoscopy was done that showed a ‘stony cave’ trachea. Histopathology confirmed the dia gnosis to be ‘Tracheobronchopathia osteochondroplastica (TO)’. He was treated with antibiotics for his infection with total resolution of chest sahdows. He is managed conservatively for his TO. Key messageTracheobronchopathia osteochondroplastica (TO) is a disorder caused by the accumu...

Clinical outcome of supervised pulmonary telerehabilitation program among adult patients with post ‐acute COVID‐19 symptoms (PACS): A case series
Here we report the outcome of nine subjects with post-acute COVID-19 symptoms (PACS) who underwent 24 sessions of supervised pulmonary telerehabilitation. An improvised telerehabilitation PR was formulated to accommodate home confinement during the pandemic. Exercise capacity and pulmonary function were assessed using a cardiopulmonary exercise test, pulmonary function test, and St. George Respiratory Questionnaire (SGRQ). The clinical outcome shows improved exercise capacity on the 6-minute walk test for all patients, and most had improvement in VO2 peak and SGRQ. Seven patients improved in forced vital capacity and six i...

Lane ‐Hamilton syndrome
We report the uncommon case of the development of idiopathic pulmonary hemosiderosis almost a decade after the diagnosis of celiac disease. Delayed diagnosis led to recurrent episodes of large volume hemoptysis despite immunosuppressive therapy due to ongoing ingestion of gluten. High doses of glucocorticoids accompanied by a cell cycle inhibitor mycophenolate mofetil were required for treatment. A strict gluten free diet is vital to control the disease. We highlight the importance of identifying this syndrome and definitive treatment, including avoidance of dietary triggers in addition to conventional immunosuppressive th...

Hypercalcemia in metastatic parathyroid carcinoma followed by hypocalcemia after resection: A case report
Parathyroid carcinoma is rare. This is a valuable case in which the serum calcium level was controlled by surgery. The patient also developed post-operative hypocalcaemia, which should be reported. AbstractA 72-year-old man was diagnosed with hypercalcaemia 13  years ago. He was diagnosed with hyperparathyroidism associated with a parathyroid tumour and surgery was performed. Postoperatively, his serum calcium levels, which had once normalized, rose again. Medical treatment for hypercalcaemia failed to control the condition. A chest computed tomography showed multiple pulmonary nodules which were diagnosed as pulmonary m...

Telomere biology disorder presenting acutely with pulmonary fibrosis and hepatopulmonary syndrome in a young adult male
A 33-year-old man presented with acute dyspnoea and orthodeoxia. CT chest showed established pulmonary fibrosis in a usual interstitial pneumonia pattern and additional investigations revealed oesophageal varices and portal hypertensive gastropathy from liver cirrhosis. Telomere length testing demonstrated short telomeres (<1st percentile) with a gene panel identifying a pathogenic variant in TERT and a variant of uncertain significance in PARN. AbstractA 33-year-old man presented with acute dyspnoea and profound hypoxaemia, and had clubbing, greying of hair, orthodeoxia and fine inspiratory crackles. CT chest showed es...
