A case of disseminated intravascular coagulation following tumour lysis syndrome due to small cell carcinoma of the lung
Disseminated intravascular coagulation (DIC) following tumour lysis syndrome (TLS) is a rare but serious life-threatening complication that clinicians need to be aware of when treating patients with advanced small cell lung cancer (SCLC). To the best of our knowledge, this is the first report of DIC following TLS in a case of SCLC. AbstractA 64-year-old man was diagnosed with small cell lung cancer (SCLC) with multiple bone and liver metastases and bone marrow metastases. Spontaneous tumour lysis syndrome (TLS) was observed before starting chemotherapy with carboplatin, etoposide, and atezolizumab. The tumour further colla...

Real ‐time visualization of lung malignancy with needle‐based confocal laser endomicroscopy during shape‐sensing robotic‐assisted bronchoscopy
The combined use of robotic-assisted bronchoscopy with needle-based confocal laser endomicroscopy may potentially enhance the differentiation of malignant cells in real-time and increase sample adequacy, accuracy, and diagnostic yield when doing a biopsy of a suspicious pulmonary lesion. AbstractRobotic-assisted bronchoscopy (RAB) improves endoscopic diagnostic yield of solitary pulmonary nodules (SPN). Needle-based confocal laser endomicroscopy (nCLE) is an emerging technology that allows high-resolution, in-vivo, real-time assessment of living tissues at a cellular and subcellular level. Their combined use has been scarc...

Pneumomediastinum and subcutaneous emphysema complicating a patient with a lung abscess
We report the clinical and radiological findings and following treatment in such a patient. AbstractPneumomediastinum and subcutaneous emphysema are conditions that carry significant morbidity. They are uncommonly seen as complications of lung abscess formation and prompt recognition and treatment is necessary. We present a 59-year-old male patient who complained of shortness of breath and chest pain for 2  weeks. Computed tomography (CT) of the thorax showed a left lower lobe lung abscess. This was associated with leucocytosis and raised C-reactive protein. Ultrasound-guided drainage revealed viscous pus requiring manua...

The panorama of pulmonary leiomyoma: A tale of two tumours
Pulmonary leiomyoma has a diverse clinical spectrum of disease. Here we describe two patients with vastly different presentations, which highlights the clinical heterogeneity in this disease, and thus, complexity in devising standardized diagnostic and therapeutic protocols. AbstractPulmonary leiomyoma has a diverse clinical spectrum of disease. Here we describe two patients with vastly different presentations. The first case is a 23-year-old female with a chronic cough and no visible tracheal lesion on computer tomography (CT) chest imaging. Flexible bronchoscopy revealed a small tracheal nodule, with histopathology confi...

Pneumatosis intestinalis with free air in the abdominal cavity caused by nintedanib
This is the first case confirming the association between pneumatosis intestinalis (PI) development and nintedanib by the reproducibility of PI development. In patients taking a combination treatment with corticosteroid and nintedanib, clinicians should be careful regarding the development of PI although the patient improved only after discontinuation of nintedanib treatment. Key messageThis is the first case confirmed the association between PI development and nintedanib by the reproducibility of PI development. In patients taking a combination treatment with corticosteroid and nintedanib, clinicians should be careful reg...

Recurrent pleural effusion from ovarian hemangioma: A rare pseudo ‐Meigs syndrome presentation
We present a case of recurrent right-sided pleural effusion caused by an ovarian hemangioma and diagnosed as pseudo-Meigs syndrome. AbstractPleural effusion is a common condition related to various diseases such as heart failure, malignancies, and pneumonia. Ovarian hemangioma is a rare type of female genital tumour and can rarely cause pleural effusion. In this case, we present a 48-year-old female with repeated episodes of recurrent right-sided pleural effusion over 1 year with no clear aetiology. Abdominal computed tomography revealed a large left ovarian mass. After surgical removal of the mass, the repeated pleural ef...

Unusual presentation of alveolar capillary dysplasia with misalignment of the pulmonary veins in a child with respiratory syncytial virus pneumonia: A case report
We describe an 18-month-old boy who presented with respiratory syncytial virus pneumonia and pulmonary hypertensive crisis requiring extracorporeal membrane oxygenation. Exome sequencing revealed aFOXF1 frameshift variant, NM_001451.2:c.995_998delACTC, inherited from his asymptomatic mother. Genetic findings were compatible with histopathology findings from a lung biopsy. Based on the disease course, histopathology, and outcomes of this case, we believe ACDMPV should be considered a possibility in an infant presenting with hypoxemic respiratory failure, resistant pulmonary hypertension, and vasodilator-induced pulmonary ed...

Pleuroparenchymal fibroelastosis in mycobacterium avium complex lung disease
We report a rare case of pleuroparenchymal fibroelastosis (PPFE) with interstitial lung disease that progressed after an onset of mycobacterium avium complex (MAC) lung disease in an 80-year-old Japanese woman. To our knowledge, little has been written and reported on the development of PPFE with ILD in MAC lung disease. The pathogenesis of PPFE with ILD in MAC lung disease remains obscure but it seems likely that triggers such as chronic/recurrent respiratory inflammation due to MAC infection may be involved. AbstractWe report a rare case of pleuroparenchymal fibroelastosis (PPFE) with interstitial lung disease progressed...

A case of a pulmonary ‐renal syndrome caused by streptococcal infection
This report highlights the facts that streptococcal infection is a relatively rare cause of pulmonary-renal syndrome, and short-term high-dose steroid monotherapy may be sufficient for its treatment. AbstractPulmonary-renal syndrome (PRS) is defined as a combination of diffuse alveolar haemorrhage and glomerulonephritis. An 18-year-old woman visited our hospital with a 2-day history of fever, dyspnoea, and leg edema. Laboratory investigations revealed an elevated inflammatory reaction, increased serum creatinine levels, and normocytic anaemia. Additionally, the anti-streptolysin-O titre was positive, and complement compone...

An unusual case of interstitial lung disease: Revisiting peribronchiolar metaplasia interstitial lung disease (PBM ‐ILD)
We present a rare case of welding-associated PBM-ILD with clinical, radiographic, and histologic evidence consistent with the disease. We outline an approach to the diagnosis of PBM-ILD based on our experience through multidisciplinary discussion. AbstractPeribronchiolar metaplasia (PBM) is a histological finding of uncertain significance commonly seen in interstitial lung disease (ILD). PBM is thought to be secondary to small airway injury from insults such as tobacco smoke and other environmental exposures. The term PBM-ILD has been proposed for patients with ILD where PBM is the major histologic finding, however a lack ...

Apoplexy in pituitary metastasis revealing a lung carcinoma
Pituitary metastasis (PM) is an uncommon manifestation of systemic malignant tumours. Herein, we present a rare case of a PM unveiling lung cancer. AbstractPituitary metastasis (PM) is an uncommon manifestation of systemic malignant tumours. It is the least common site of intracranial metastases. As PM has no clinical or radiological pathognomonic features, their diagnosis is challenging. Herein, we present a rare case of a PM unveiling lung cancer. A 60-year-old male with no medical history of malignancy was admitted with a sudden headache, retro-orbital pain, and a severe loss of both eyes' visual acuity. After proper in...

Obstructive shock due to a spontaneous haemothorax caused by primary lung cancer
We describe the case of a 67-year-old man with shock and hypoxemia. Chest X-ray showed bilateral lung mass shadows and left pleural effusion with a mediastinal shift, suggesting malignancy. Physical examination and point-of-care ultrasound findings did not suggest obstructive or cardiac shock, but the patient had prolonged shock refractory to fluid and blood transfusion therapy. We inserted a drain into the left thoracic cavity, which enabled the patient to recover from shock. We diagnosed the patient with obstructive and hypovolemic shock due to spontaneous haemothorax caused by primary lung cancer. Tension haemothorax du...
