Hypercalcemia in metastatic parathyroid carcinoma followed by hypocalcemia after resection: A case report
Parathyroid carcinoma is rare. This is a valuable case in which the serum calcium level was controlled by surgery. The patient also developed post-operative hypocalcaemia, which should be reported. AbstractA 72-year-old man was diagnosed with hypercalcaemia 13  years ago. He was diagnosed with hyperparathyroidism associated with a parathyroid tumour and surgery was performed. Postoperatively, his serum calcium levels, which had once normalized, rose again. Medical treatment for hypercalcaemia failed to control the condition. A chest computed tomography showed multiple pulmonary nodules which were diagnosed as pulmonary m...

Telomere biology disorder presenting acutely with pulmonary fibrosis and hepatopulmonary syndrome in a young adult male
A 33-year-old man presented with acute dyspnoea and orthodeoxia. CT chest showed established pulmonary fibrosis in a usual interstitial pneumonia pattern and additional investigations revealed oesophageal varices and portal hypertensive gastropathy from liver cirrhosis. Telomere length testing demonstrated short telomeres (<1st percentile) with a gene panel identifying a pathogenic variant in TERT and a variant of uncertain significance in PARN. AbstractA 33-year-old man presented with acute dyspnoea and profound hypoxaemia, and had clubbing, greying of hair, orthodeoxia and fine inspiratory crackles. CT chest showed es...

A customized multimodality approach for prolonged air leaks (PAL) in mechanically ventilated patients
Persistent air leaks in mechanically ventilated patients can be challenging. Multimodality approach using various bronchoscopic techniques followed by pleurodesis can be game changer. AbstractARDS in general and severe COVID ARDS (CARDS) is particularly associated with high rates of barotrauma. Two cases with severe CARDS developed bilateral pneumothorax with persistent air leak (PAL). Conservative management with prolonged chest tube drainage did not help in PAL resolution and both patients continued to be on high-end ventilatory support. The course was further complicated by the presence of septic shock. The 1st patient ...

Diffuse pulmonary meningotheliomatosis
(DPM) is a rare pulmonary disease of unknown aetiology. These imaging findings are characteristic of DPM, adding to the limited number of literature reports on the rare disease. Key messageDiffuse pulmonary meningotheliomatosis (DPM) is an ultra-rare pulmonary disease characterized by innumerable bilateral minute meningothelial-like nodules, sometimes presenting a characteristic ‘cheerio-sign’ on imaging. Most patients with DPM are asymptomatic and experience no disease progression. Although little is known about its nature, DPM may be associated with pulmonary malignancies, mostly lung adenocarcinoma. (Source: Respirology Case Reports)

Pathological complete response in an advance stage IIIB non ‐small cell lung cancer secondary to neoadjuvant osimertinib targeted therapy: A case report and review of literature
The use of osimertinib as a safe and effective neoadjuvant targeted therapy for potentially resectable advance non-small cell lung carcinoma. AbstractNeoadjuvant chemotherapy is a therapeutic option for potentially resectable non-small cell lung cancer (NSCLC). The role of neoadjuvant targeted therapy (NTT) remains less explored. This case highlights the use of neoadjuvant osimertinib in a case of advanced NSCLC. A 67-year-old woman had a left lower lobe lung mass measuring 5.0  × 5.1 × 7.0 cm with an enlarged subcarinal lymph node (LN) on her positron emission tomography scan. Following biopsy, a diagnosis of ...

Paediatric foreign body aspiration and endobronchial granuloma
Paediatric foreign body aspiration may result in pulmonary complications. Both surgical and bronchoscopic intervention have been described in managing larger endobronchial lesions. In this case, complete resolution of the granulomatous lesions was achieved with conservative management. Key messagePaediatric foreign body aspiration can result in pulmonary complications including the formation of endobronchial granuloma, which can be managed conservatively or via surgical or bronchoscopic intervention. This case highlights the importance of having a high index of suspicion for foreign body aspiration in patients with persist...

Endobronchial indocyanine green instillation to identify the intersegmental plane for successful segmentectomy
We describe three cases of anatomical segmentectomy (AS) using the combination of endobronchial infiltration of indocyanine green (ICG) to identify the intersegmental plane (critical for the performance of AS), and Computed Tomography (CT) guided methylene blue injection for lesion localisation. The operations were completed successfully demonstrating satisfactory post-operative outcomes including lesion resection with clear surgical margins and acceptable length of stay. We believe that endobronchial instillation of ICG and CT-guided methylene blue injection for lesion localisation show promise as a technique to complemen...

Cystic fibrosis modulator therapy can reverse cystic bronchiectasis
This report describes the reversal of cystic bronchiectasis following a year of CF modulator therapy, providing exciting new insights into the pathogenesis of bronchiectasis in CF. AbstractBronchiectasis is often considered progressive and irreversible, so cases of regression or reversal are an important step in understanding the underlying pathophysiological mechanisms. Cystic fibrosis, (CF) caused by pathogenic variants in thecystic fibrosis transmembrane conductance regulator (CFTR) gene has been a success story in personalized medicine. The recent development ofCFTR modulator therapies has revolutionized care. Dramatic...

Pulmonary hypertension secondary to interstitial fibrosis with pulmonary venous lesions masquerading pulmonary veno ‐occlusive disease
We present a 41-year-old man with idiopathic interstitial pneumonia and pulmonary hypertension (PH) in the setting of a non-autoimmune background whose clinical presentation masqueraded pulmonary veno-occlusive disease (PVOD). Because of no histological evidence of venous occlusion in his previous lung biopsy, phosphodiesterase type-5 inhibitor was given, resulting in sudden onset of pulmonary edema. At autopsy, there were histological features of interstitial fibrosis with occlusion of the lobular septal veins and venules. Clinical presentations of PH due to interstitial fibrosis with pulmonary venous lesions may simulate...
