A hole lot of trouble: Severe bilateral anastomotic dehiscence following bilateral single sequential lung transplant
We describe a case of a 22-year-old female who developed significant bilateral anastomotic dehiscence with severe ischaemia following a bilateral single sequential lung transplant (BSSLTx). Following an intensive antimicrobial regimen, judicious bronchoscopic surveillance, and a prolonged inpatient stay, the dehiscence resolved without requiring further surgical intervention. Our case highlights a space in the literature for further research with regard to airway complications post-lung transplant and their management. (Source: Respirology Case Reports)

Management of alveolar ‐pleural fistula secondary to invasive pulmonary aspergillosis with bronchial occlusion using a combination of Endobronchial Watanabe Spigot and N‐butyl‐2‐cyanoacrylate: A case report
We report a case of alveolar-pleural fistula managed by bronchial occlusion using a combination of Endobronchial Watanabe Spigot and N-butyl-2-cyanoacrylate. AbstractAlveolar-pleural fistulas that do not improve with thoracic drainage can be conservatively treated via endobronchial occlusion and pleurodesis, among other options. However, for inoperable cases, the treatment strategy to be followed, in the event that conventional conservative management fails, is unclear. Herein, we report a case of alveolar-pleural fistula managed by bronchial occlusion using a combination of Endobronchial Watanabe Spigot (EWS) and N-butyl-...

A 92 ‐year‐old patient with thoracic empyema successfully treated by CT‐guided insertion of a pigtail catheter
We present a case report of a 92-year-old patient with thoracic empyema, who was successfully treated via CT-guided insertion of a pigtail catheter. We believe that this educational case serves as a testament to the fact that even the most aged patients can be successfully treated with resourcefulness. AbstractWe present a case report of a 92-year-old patient with thoracic empyema, who was successfully treated via CT-guided insertion of a pigtail catheter. The advanced age of the patient often poses challenges in managing pyothorax due to limited physical activity and cognitive decline stemming from decreased activities of...

Ultrasound ‐controlled transthoracic true‐cut needle biopsy in pulmonary nodular amyloidosis
The current case report presents a 59-year-old man with imaging studies of the thorax showing nodular lesions in the lungs bilaterally. Preliminary diagnoses for possible granulomatosis (tuberculosis) or pulmonary metastatic dissemination of a neoplastic process were made. An ultrasound-controlled transthoracic true-cut needle biopsy of a subpleural lesion was performed, confirming the diagnosis of ‘pulmonary nodular amyloidosis’. AbstractThe current case report presents a 59-year-old man with imaging studies of the thorax showing nodular lesions in the lungs bilaterally. Based on radiographic and CT images, preliminar...

Severe asthma remaining well ‐controlled after mepolizumab discontinuation: A case report and literature review
Mepolizumab, a humanized anti-IL-5 monoclonal antibody used for severe asthma, results in reduced asthma exacerbations, improved lung function, reduced oral corticosteroid use, and improved quality of life. But little is known about the effects of withdrawal after long-term treatment. Here, we report a case of a patient with asthma, well-controlled without exacerbation after withdrawal of mepolizumab. AbstractMepolizumab, a humanized anti-IL-5 monoclonal antibody used for severe asthma, results in a reduced rate of asthma exacerbation, improved lung function, reduced oral corticosteroid use, and improved quality of life. A...

Massive pulmonary embolism led to cardiac arrest two days after thoracoscopy in a young male with pleural tuberculosis
We describe a fatal case of a 30-year-old previously healthy male with pleural tuberculosis who developed a massive pulmonary embolism with subsequent cardiac arrest after a diagnostic medical thoracoscopy. Computed tomography pulmonary angiogram showed major right pulmonary embolism. Unfortunately, the patient passed away despite resuscitation and extensive organ support in the intensive care unit. This case highlights the thrombotic risk in this population group in order to avoid such devastating complications. AbstractTB itself is considered an independent risk factor for VTE; however, developing pulmonary embolism afte...

Intrapleural therapy for pleural infection from bronchopleural fistula in an adult with hyper ‐IgE syndrome
We presented the case of an adult patient with hyper-IgE syndrome (HIES) who was admitted acutely with a large hydropneumothorax from lung consolidation, a bronchopleural fistula and pleural infection. He has had recurrent pulmonary and skin infections since childhood and longstanding pneumatoceles. He was treated with systemic antibiotics and chest tube drainage. Administration of two doses of low-dose intrapleural therapy (1  mg tissue plasminogen activator and 5 mg deoxyribonuclease) allowed complete evacuation of his residual loculated pleural fluid, aided resolution of his infection without provoking a significant...

PD ‐L1‐negative non‐small cell lung cancer harbouring a rare BRAF mutation with successful treatment of first‐line pembrolizumab plus chemotherapy: A case report and review the literature
We document the case of a 63-year-old patient with NSCLC harbouring a rareBRAF E501Q mutation, who had a prolonged response to immunotherapy combined with chemotherapy in Vietnam. AbstractBRAF mutations are uncommon in non-small cell lung cancer (NSCLC), accounting for less than 5% of all NSCLC cases. The utilization of targeted therapies in non-V600EBRAF mutant NSCLC is considered controversial, although non-V600E genotype is reported in ~50% of allBRAF mutant patients. We document the case of a 63-year-old patient with NSCLC harbouring a rareBRAF E501Q mutation, who had prolonged response to immunotherapy combined with c...

Combined treatment with beaded EWS, Gore ‐tex patch and N‐butyl‐2‐cyanoacrylate for bronchial fistula: The novel “sandwich method”
In this report, we describe, for the first time, a novel method for closing bronchopleural fistula (BPF) using an endobronchial Watanabe spigot (EWS), polyglycolic acid (PGA) sheet and N-butyl-2-cyanoacrylate (NBCA). We named this method the “sandwich method.” AbstractAn 84-year-old man underwent right basal segmentectomy for primary lung cancer and developed empyema accompanied by a bronchopleural fistula (BPF). Emergency open-window thoracotomy was performed. Although the general and nutritional conditions improved, the fistula did not close naturally, and we planned to close it 6  months after surgery. In this rep...

Azygos vein thrombosis despite rivaroxaban therapy in a patient with breast cancer: A rare entity
Azygos vein thrombosis (AVT) is an extremely rare type of venous thromboembolism. It is commonly associated with predisposing factors such as azygous vein aneurysm and malignancy including cholangiocarcinoma and renal cell carcinoma. Here we report a rare case of AVT associated with metastatic breast cancer. Key messageIn the light of this case, azygos and hemiazygos veins should be evaluated carefully in thorax CT scans. This rare case is also important in terms of the occurrence of AVT in a patient receiving rivaroxaban therapy. The outcomes and recurrence rates of these patients should also be reported for the decision ...
