Portopulmonary hypertension: Success with combined medical treatment
We report here the case of a 57-year-old man with portopulmonary hypertension caused by chronic hepatosplenic schistosomiasis and also liver cirrhosis due to hepatitis C and alcoholism. As well as treating both diseases, portopulmonary hypertension was successfully managed with tadalafil and macitentan as maintenance therapy. This case reminds clinicians that pulmonary hypertension can be multifactorial, a good diagnosis and a multidisciplinary treatment can lead to improved prognosis. (Source: Respirology Case Reports)

Pulmonary embolism or COVID ‐19 pneumonia? A case report
During the COVID-19 pandemic, a high index of suspicion is required for the timely diagnosis of pulmonary embolism, especially in patients with identifiable risk factors. This is specifically true for patients who cannot express their symptoms due to neurocognitive disorders. AbstractPulmonary embolism (PE) is the most severe clinical presentation of venous thromboembolism (VTE), which can be challenging to diagnose due to its non-specific symptoms. The overlapping clinical symptoms of Coronavirus disease 2019 (COVID-19) and PE may make distinguishing between the two difficult. Thus, the diagnosis of PE may be delayed or m...

Using cryobiopsy with Radial EBUS in high ‐bleeding‐risk, peripheral pulmonary lesions (PPL): description of cases and technique
This case series describes four patients with peripheral pulmonary lesions (PPL), undergoing cryobiopsy with radial EBUS for suspected lung cancer, and later diagnosed to have a PPL deemed to be of a high bleeding risk. The use of cryobiopsy with radial ultrasonic examination for the vasculature of the PPL, bronchial blocker use, and airway protection as well as an expert team preserved the safety of the procedure. AbstractCryobiopsy is an emerging tool in the diagnosis of peripheral pulmonary lesions (PPL) and becoming an important tool in the toolbox. Anecdotally the data on cryobiopsy use in the lung was extrapolated fr...

Pseudoprogression during induction treatment with nivolumab plus ipilimumab combined with chemotherapy for metastatic lung adenocarcinoma: A case report
We present the case of pseudoprogression after the first cycle of dual immunotherapy with chemotherapy in a 55-year-old male with invasive mucinous adenocarcinoma. AbstractThe incidence rate of pseudoprogression during immune checkpoint inhibitor monotherapy for non-small cell lung cancer is reportedly 3.6% –6.9%, while pseudoprogression during chemoimmunotherapy is rare. Reports on pseudoprogression during dual immunotherapy combined with chemotherapy are lacking. Herein, a 55-year-old male with invasive mucinous adenocarcinoma (cT2aN2M1c [OTH, PUL], stage IVB, and programmed death-ligand 1 expressi on<1%), renal dys...

Anti ‐synthetase syndrome associated interstitial lung disease after combination dual immune checkpoint inhibition
In this study, we describe the first reported case of antisynthetase syndrome; characterized by circulating anti-EJ (glycyl) t-RNA synthetase antibodies accompanied by relapsing, fibrosing and treatment-resistant interstitial pneumonitis. This case study highlights the need to also consider rarer phenotypes  of ICI-induced inflammatory lung injury beyond conventional ‘ICI pneumonitis’ in the diagnostic workup of these patients. Differentiating between rare forms of CTD-ILD (in particular IIM-ILD) and conventional ‘ICI pneumonitis’ has important implications for the expected natural history and optimal treatment pa...

Combining three ‐dimensional fluoroscopy guide system with single‐use bronchoscopes for diagnosis of peripheral lesions: First two cases
We report two cases of PPLs sampling procedures combining the use of aScope 5 single-use bronchoscope and CIOS 3D Spin Mobile fluoroscopy system. The use of ever-new disposable bronchoscopes for sampling peripheral lesions is a viable alternative to reusable bronchoscopes for advanced bronchoscopy procedures. AbstractPulmonary Peripheral Lesions (PPLs) diagnosis is usually performed using a guidance system in combination with bronchoscopes and diagnostic tools. We report two cases of PPLs sampling procedures combining the use of the single-use bronchoscope Ambu aScope 5 Broncho and CIOS 3D Spin Mobile (Siemens Healthineers...

Eradication of post ‐lobectomy mucormycosis and bacterial empyema with intrapleural antimicrobial therapy in a patient with surgically resected Mycobacterium xenopi (non‐tuberculous Mycobacteria) pulmonary infection
We present a 57-year-old gentleman who developed chronic mucormycosis (Cunninghamella sp.) and bacterial (Enterococcus sp.) empyema in a high-risk post-lobectomy space in the setting of a non-expandable lung following non-tuberculous mycobacterial infection. This was successfully treated with a combination of systemic and intrapleural therapies. AbstractEmpyema thoracis is a collection of pus in the pleural space associated with pleural fibrin deposition. Treatment involves systemic antimicrobials, pleural drainage, intrapleural enzymes and sometimes decortication. Our case is a 57-year-old gentleman who developed chronic ...

Immune checkpoint inhibitor ‐related haemophagocytic lymphohistiocytosis in a patient with non‐small cell lung carcinoma
We present a case, in whom pancytopenia with fever and hyperferritinaemia led us to suspect HLH. Early diagnosis of ICI-related HLH is essential for a better outcome. Key messageHemophagocytic lymphohistiocytosis (HLH) has been reported as a rare complication of immune checkpoint inhibitors (ICI); however, ICI-related HLH is a life-threatening and comparatively late adverse event. Early diagnosis is critical, and it should be included in the differential diagnosis especially in patients with cytopenia with fever and hyperferritinaemia. (Source: Respirology Case Reports)

Pleural fluid milkshakes: three cases with different aetiologies
Contrary to popular belief, the classical milky appearance of pleural fluid is seen in less than half of patients with chylothorax. In this case series, we present three patients with biochemically proven chylothorax: each with a different presentation, pleural fluid appearance, underlying cause, management strategy, and clinical outcome. Lymphoma, tuberculosis, and presumed amyloidosis are underlying causes in our patients. AbstractIn clinical practice, chylothorax is usually suspected in any patient with milky pleural fluid. However, contrary to popular belief, milky appearance of pleural fluid is seen in less than half ...

A case of Rasmussen's aneurysm caused by pulmonary nontuberculous mycobacterium
We report a Rasmussen's aneurysm due to NTM. Key messageRasmussen's aneurysm was originally described as a rare cause of hemoptysis in tuberculosis. Dilatation of the pulmonary artery wall is caused by tuberculosis inflammation. Recently, the incidence of non-tuberculous mycobacterial (NTM) disease has increased; it now exceeds that of tuberculosis. We report a Rasmussen's aneurysm due to NTM. Resection of a colloid adenocarcinoma of the lung: A case report
In this report, we describe a case of colloid adenocarcinoma of the lung with a 5-year recurrence-free follow-up. The patient is a 66-year-old woman. During postoperative follow-up for ovarian cancer, chest CT showed a 45  × 30 mm sized mass in the left lung with mixed low-absorption areas inside that were suspicious of cystic lesion. We suspected metastatic lung tumour, and performed lower lobectomy. Pathological examination revealed pale tumour cells forming a glandular lumen with internal mucus production. Based on the results of immunostaining we diagnosed colloid adenocarcinoma of the lung. She received postope...
