Mesalazine-Induced Obstructive Intrahepatic and Extrahepatic Bile Duct Inflammation Mimicking Cholangiocarcinoma
A 22-year-old man with ulcerative colitis was started on mesalazine slow-release tablets 2 grams twice daily and was hospitalized 3 months later for a 9-day history of jaundice, epigastric pain, nausea, vomiting, low-grade fever, and coughing. He had discontinued mesalazine 4 days before and received no other medications. (Source: Clinical Gastroenterology and Hepatology)

Lack of Effectiveness of Computer Aided Detection for Colorectal Neoplasia: A Systematic Review and Meta-analysis of Non-Randomized Studies
Benefits of computer-aided detection (CADe) in detecting colorectal neoplasia were shown in many randomized trials where endoscopists ’ behavior was strictly controlled. However, the effect of CADe on endoscopists’ performance in less-controlled setting is unclear. This systematic review and meta-analyses were aimed at clarifying benefits and harms of using CADe in real-world colonoscopy. (Source: Clinical Gastroenterology and Hepatology)

Reliability and Responsiveness of Histologic Indices for the Assessment of Crohn ’s Disease Activity
The operating properties of histologic indices for evaluating Crohn ’s disease (CD) activity are poorly characterized. We assessed the reliability and responsiveness of existing histologic indices/items used in CD and ulcerative colitis (UC), in addition to 3 novel items, and developed exploratory ileal, colonic, and colonic-ileal CD instruments. Unusual Villiform Re-epithelialization in Barrett's Esophagus Following Endoscopic Treatment
A 57-year-old male patient with a history of high-grade dysplasia and intramucosal carcinoma in the context of Barrett's esophagus, treated with several endoscopic mucosal resections and radiofrequency ablation (RFA) procedures, presented for a follow-up gastroscopy. The endoscopic assessment revealed an unusual finding in the esophagus. The Barrett's segment, which had been previously treated, now exhibited a very fine villiform appearance (Figure  A, Video) with absence of the typical mucosal pattern when viewed under virtual chromoendoscopy (Figure B). Accuracy of the enhanced liver fibrosis test in patients with type 2 diabetes mellitus and its clinical implications
The diagnostic performance of the Fibrosis-4 (FIB-4) index and nonalcoholic fatty liver disease (NAFLD) fibrosis score (NFS) is poor in patients with type 2 diabetes mellitus (T2DM). We determined the usefulness of the enhanced liver fibrosis (ELF) test in patients with T2DM. (Source: Clinical Gastroenterology and Hepatology)

EXPLORing the Therapeutic Ceiling in Crohn ’s Disease: Will Combination Pave the Way?
There are numerous synonyms for explorer including: adventurer, pathfinder, pioneer, seeker, experimenter, pilgrim, searcher, and traveler. The investigators in the EXPLORER trial sought to improve the “efficacy ceiling” for treating Crohn’s disease (CD) by venturing into combination therapy by combining 3 semi-independent mechanisms of action (MOAs) that, individually, have demonstrable efficacy.1 Both vedolizumab and adalimumab are biological drugs approved to induce and maintain remission , whereas (parenteral) methotrexate has not been evaluated as an independent induction agent. What the New Definition of MASLD Left Behind: Dual Etiology With Viral Hepatitis
We read with great interest the published recently article about the prevalence of steatotic liver diseases by Kalligeros et  al.1 Undoubtedly, the nomenclature for fatty liver disease was in need of revision. In 2020, Eslam et al2 introduced the metabolic dysfunction–associated fatty liver disease (MAFLD) nomenclature, which subsequently was widely accepted and implemented by various national and pan-national societi es. In practice and research, MAFLD has proven superior to the previous name, nonalcoholic fatty liver disease (NAFLD), in several respects. (Source: Clinical Gastroenterology and Hepatology)
