Hospital Quality Mediates Impact of Care Fragmentation Following Elective Colectomy
CONCLUSIONS: Care fragmentation was associated with greater morbidity and resource utilization at readmission following elective colectomy. Further, rehospitalization at non-index LQH conferred significantly inferior outcomes. Novel efforts are needed to improve continuity of care.PMID:38659168 | DOI:10.1177/00031348241248795 (Source: The American Surgeon)

Ileorectal anastomosis in ulcerative colitis: what do surgeons and patients need to know? A systematic literature review
CONCLUSIONS: For some patients, IRA can offer restorative surgery in the short or long term, with acceptable cancer risk, failure rate and postoperative complications, while avoiding the higher risks associated with IPAA.PMID:38660816 | DOI:10.1308/rcsann.2024.0012 (Source: Annals of the Royal College of Surgeons of England)

Real-world efficacy and safety of tofacitinib treatment in Asian patients with ulcerative colitis
CONCLUSION: TOF was more effective in UC patients with mild activity at baseline and its efficacy was not affected by previous treatment with anti-TNF-α agents. Most relapsed patients responded again after re-increase of TOF and nearly half relapsed after tapering off TOF. Special attention is needed for tapering and HZ.PMID:38659488 | PMC:PMC11036499 | DOI:10.3748/wjg.v30.i13.1871 (Source: World Journal of Gastroenterology : WJG)

Exceptional lymph node recurrence of an unusual ovarian tumor 16 years later: a case report
ConclusionSex cord-stromal tumors with annular tubules is a slow-growing ovarian tumor with a high potential for recurrence and metastasis. Surgery is the mainstay of treatment. Due to the rarity of these tumors, they are often unsuspected and thus incompletely staged before primary surgery; the diagnosis is made by histological examination. The prognosis of these patients is unknown, and they require long-term follow-up. (Source: Journal of Medical Case Reports)

A case of refractory severe pyoderma gangrenosum successfully treated with upadacitinib
A 59-year-old woman affected by colonic Crohn's disease and perianal disease underwent colectomy and end ileostomy. Persistently active luminal disease was associated with severe pyoderma gangrenosum localized to the upper and lower limbs. Previous treatments with intravenous steroids and infliximab were not effective on pyoderma gangrenosum lesions. The image shows the evolution of a large and deep lesion localized on the right hand, after starting upadacitinib 45 mg once a day, with clear improvement and almost complete healing in 6 weeks of treatment (Fig. (Source: Digestive and Liver Disease)

Firearm injury and the Deloyers procedure: case report and literature review
This report aims to discuss the circumstances under which the Deloyers procedure was performed, as well as to evaluate the early and late postoperative outcomes, by reviewing cases conducted between 2010 and 2023. In a 22-year-old female patient who suffered major organ and tissue loss (with injuries to the sigmoid colon, descending colon, transverse colon, and mesentery) due to a firearm injury, the Deloyers procedure was applied during restorative surgery following initial damage control surgery. The procedure involved mobilizing the cecum and right colon, performing a cranio-caudal rotation over the ileocolic artery ped...
