Novel Prostate Biopsy Technique Using Imaging Fusion in a Patient With Absent Rectum
A 70-year-old male with prior total colectomy for ulcerative colitis was referred for elevated prostate specific antigen (PSA) (8.01) with PIRADS 4 lesion on magnetic resonance imaging (MRI). Described is a novel technique using pre-operative multi-parametric prostate MRI and intraoperative computed tomography (CT) 3D/3D fusion for systematic and targeted prostate biopsy in a patient lacking a rectum. Colonic Resection in an 8-Year-Old Girl with Intractable Functional Constipation and Diffuse Colonic Dysmotility and Failed Antegrade Flushes
We present a patient with such a history, who ultimately achieved successful surgical management of constipation through a stepwise approach. An 8-year-old female presents with longstanding constipation and diffuse colonic dysmotility demonstrated with colonic manometry. She initially underwent sigmoid resection and cecostomy which failed and required diverting ileostomy. We initially proceeded with an extended resection, colonic derotation (Deloyers procedure), and neo-appendicostomy (neo-Malone) which resulted in successful spontaneous stooling for 1 year. Her constipation recurred and she subsequently underwent completi...

Implementation of a robotic surgical practice in inflammatory bowel disease
AbstractRobotics adoption has increased in colorectal surgery. While there are well-established advantages and standardised techniques for cancer patients, the use of robotic surgery in inflammatory bowel disease (IBD) has not been studied yet. To evaluate the feasibility and safety of robotic surgery for IBD patients. Prospectively data in IBD patients having robotic resection at Guy ’s and St Thomas’ hospital. All resections performed by a single colorectal surgeon specialised in IBD, utilising DaVinci platform. July 2021 to January 2023, 59 robotic IBD cases performed, 14 ulcerative colitis (UC) and 45 Crohn’s dis...

Medical treatment selection and outcomes for hospitalized patients with severe ulcerative colitis as defined by the Japanese criteria
ConclusionsTacrolimus and infliximab in addition to corticosteroids were used as first-line treatment in severe hospitalized patients. PRO2 on day 3 is a useful marker for switching to second-line therapy or colectomy. (Source: Journal of Gastroenterology)

Breastfeeding status and disease severity in pediatric patients with ulcerative colitis: an exploratory retrospective cohort study
While the protective effect of being breastfed on the development of ulcerative colitis has been well established, it is not known if being breastfed decreases colectomy rate and disease severity for pediatric patients. This work is designed to better understand the connection between breastfeeding status and disease severity of ulcerative colitis among pediatric patients. (Source: Gastroenterology)

Sarcopenia is a Poor Predictor of Outcomes in Elective Colectomy for Diverticulitis
CONCLUSIONS: Incidence of sarcopenia was high in patients undergoing elective colectomy for diverticulitis in our practice, but sarcopenia was not associated with adverse perioperative outcomes. In select patients, elective colectomy for diverticulitis can be safely performed in the presence of sarcopenia.PMID:38263953 | DOI:10.1177/00031348241229630 (Source: The American Surgeon)

Complicated Airway Management Because of Multiple Tracheal Diverticula in a Patient with Mounier –Kuhn Syndrome
A 52-yr-old man undergoing laparoscopic colectomy had a significant peritubal air leak after initiation of mechanical ventilation using a 7.5-size endotracheal tube (ETT). Peritubal air leak continued even after an 8.0-size ETT with excessive cuff inflation had been used. Multiple tracheal diverticula were detected during flexible bronchoscopy, and the tube was inserted beyond the diverticula. After excessive cuff inflation, leakage was limited to a clinically acceptable value. Preoperative computed tomography revealed Mounier –Kuhn syndrome with multiple tracheal diverticula (fig. 1,A to C,red arrows). (Source: Anesthesiology)

Right Colectomy for an Inflammatory Mass in the Cecum Later Found to Be Tuberculosis
We present a case of an 85-year-old male patient with delayed diagnosis of gastrointestinal TB who underwent colonic resection for a cecal mass that was initially suspected to be malignant. Acid-fast staining of the surgical specimen later revealed acid-fast bacilli and multiple lymph nodes with necrotizing granulomas. The purpose of this study is to stress the importance of including gastrointestinal TB as a differential diagnosis for patients with suspected colorectal malignancy, especially when initial biopsy results do not reveal malignant features.PMID:38213128 | DOI:10.1177/00031348241227175 (Source: The American Surgeon)

Comparison of intracorporeal and extracorporeal anastomosis in laparoscopic right colectomy: an updated meta-analysis and trial sequential analysis
In this study, the Cochrane Handbook for Systematic Reviews of Interventions and the PRISMA guidelines were applied. The study protocol received a PROSPERO registration (CRD42020214596). A systematic literature search of the electronic scholar databases (Medline, Web of Science and Scopus) was performed. To reduce type I error, a trial sequential analysis (TSA) algorithm was introduced. The quality of evidence was evaluated based on the GRADE methodology. In total, 46 studies were included in this meta-analysis, Pooled comparisons and TSA confirmed that IC is superior in terms of incisional hernia (0.29; 95%CI: 0.19, 0.44)...
