iPLA2 inhibition blocks LysoPC-induced TRPC6 externalization and promotes Re-endothelialization of carotid injuries in hypercholesterolemic mice
This study suggests that iPLA2 is a potential therapeutic target to attenuate calcium influx through TRPC6 channels and promote EC healing in cardiovascular patients undergoing angioplasty.PMID:37030190 | DOI:10.1016/j.ceca.2023.102734 (Source: Cell Calcium)

Cerebral hemodynamics in symptomatic anterior circulation intracranial stenosis measured by angiography-based quantitative flow ratio: association with CT perfusion
ConclusionsμQFR was associated with CTP in patients with symptomatic anterior circulation ICAS and may be a potential marker for real-time hemodynamic evaluation during interventional procedures.Key Points• Murray law–based QFR (μQFR) is associated with CT perfusion parameters in intracranial atherosclerotic stenosis and can differentiate hypoperfusion from normal perfusion.• Post-intervention μQFR, collateral score, and current smoking status are independent factors associated with improved perfusion after treatment. Procedural embolic protection strategies for carotid artery stenting: current status and future prospects
Expert Rev Med Devices. 2023 Mar 31. doi: 10.1080/17434440.2023.2198124. Online ahead of print.ABSTRACTINTRODUCTION: Carotid artery angioplasty and stenting (CAS) is an established procedure to treat carotid artery stenosis for either primary or secondary prevention of stroke. Randomized clinical trials have shown an increased risk of periprocedural cerebrovascular events with CAS compared with carotid endarterectomy (CEA). Several strategies have been proposed to mitigate this risk, including alternative vascular access site, proximal/distal embolic protection devices, and dual layer stents among others.AREAS COVERED: Thi...

A 13-Year Experience of Carotid Endarterectomy: To Patch, Evert, Bypass or Primarily Close, That is the Question
Several techniques have been described for carotid endarterectomy (CEA) to treat carotid artery stenosis. Although the European guidelines have recommended patch angioplasty over primary closure when undertaking conventional CEA, the adherence to this appears to vary. In addition, no clear consensus has been reached regarding the superiority of patch angioplasty over eversion endarterectomy. We propose that the modern-day vascular surgeon should ideally be competent in all techniques of CEA and should tailor the use of each technique by the individual patient characteristics. Retropharyngeal Left Carotid to Right Subclavian Bypass for Subclavian Steal Syndrome as Salvage Procedure Due to No Ipsilateral Donor Site
Subclavian steal syndrome, although uncommon, can lead to vertebrobasilar insufficiency, upper extremity ischemia, and, even, myocardial ischemia in select patients. The treatment options for symptomatic patients include transthoracic approaches, extra-anatomic cervical bypass, and endovascular intervention. Extra-anatomic bypass has been preferred owing to the lower perioperative morbidity and mortality risk compared with transthoracic procedures and superior patency rates compared with balloon angioplasty with and without stenting. Dilation of proximal internal carotid artery collapse due to severe distal stenosis after angioplasty for distal stenosis: A case report
CONCLUSION: PTA for severe distal stenosis with proximal ICA collapse may result in dilation of proximal ICA collapse over time.PMID:36895219 | PMC:PMC9990815 | DOI:10.25259/SNI_27_2023 (Source: Surgical Neurology International)
