In 2002, Nieder et al published a classic review using the metaphorical phrase of “cutting down the tree to get an apple” to describe the radiotherapeutic management of squamous cell carcinoma metastatic to the cervical lymph nodes of unknown primary origin [1]. This analogy, which was meant to illustrate the unnecessary and wasteful destruction of all that was good, in the p rocess of achieving a narrow aim, is worthy of consideration in light of advancements in biology and technology which have improved the level of precision in which this disease has been approached over the last 20 years.
