Determining efficacy of dynamic multimedia bowel preparation instructions versus standard instructions on adenoma detection and patient reported measures (DIGICLEAN trial): a study protocol for a multicentre, colonoscopist-blinded, randomised controlled trial
BMJ Open. 2023 Jul 21;13(7):e073843. doi: 10.1136/bmjopen-2023-073843.ABSTRACTINTRODUCTION: Colonoscopy plays important roles in bowel cancer screening and treatment. Poor bowel preparation occurs in 20-25% of colonoscopies. This negatively impacts adenoma and sessile serrated lesion detection rates, procedural time, requirement for repeat colonoscopies, healthcare costs and likelihood of patient withdrawal from screening programmes. It is unclear whether a combination of multimedia modalities can improve bowel preparation quality, adenoma detection rates and patient-reported measures in those undergoing colonoscopy assess...

Geographical and spatial variations in bowel cancer screening participation, Australia, 2015 –2020
ConclusionsDespite having a well-structured evidence-based government funded national bowel cancer screening program, the substantial spatial variation in participation rates highlights the importance of accounting for the unique characteristics of specific geographical regions and their inhabitants. Identifying the reasons for geographical disparities could inform interventions to achieve more equitable access and a higher overall bowel screening uptake. (Source: PLoS One)

Process, structural, and outcome quality indicators to support perioperative opioid stewardship: a rapid review
AbstractOpioids are effective analgesics but can cause harm. Opioid stewardship is key to ensuring that opioids are used effectively and safely. There is no agreed set of quality indicators relating to the use of opioids perioperatively. This work is part of the Yorkshire Cancer Research Bowel Cancer Quality Improvement programme and aims to develop useful quality indicators for the improvement of care and patient outcomes at all stages of the perioperative journey.A rapid review was performed to identify original research and reviews in which quality indicators for perioperative opioid use are described. A data tool was d...

Cancers, Vol. 15, Pages 2766: Optimising Outcomes and Surveillance Strategies of Rectal Neuroendocrine Neoplasms
K. Ramage Rectal neuroendocrine neoplasms are increasing in incidence, in part due to increased endoscopic procedures being performed for bowel cancer screening. Whilst most of these lesions are low-grade well-differentiated neuroendocrine tumours, they can have a varied clinical behaviour. Frequently, these lesions are incorrectly characterised at endoscopy and, therefore, incompletely excised using standard polypectomy techniques. Furthermore, some cases are not fully staged prior to or post resection. In this article we discuss the endoscopic and surgical options available to improve the likelihood of achieving an R...

Colorectal cancer and advanced adenoma incidence during post-polypectomy surveillance: a national cohort study in the English Bowel Cancer Screening Programme
Conclusion This large, national analysis found low levels of CRC in those undergoing surveillance and low advanced adenoma yield in most subgroups. Less intensive surveillance in some subgroups is warranted, and surveillance may be avoided in those with a single large adenoma. Evaluating changes to home bowel cancer screening kits: an end-user perspective study
ConclusionThese findings highlight multiple ways in which bowel cancer screening kits can be changed to increase usability for invitees of national bowel cancer screening programs. Findings have implications for all screening programs that use immunochemical-based bowel cancer screening kits. (Source: Cancer Causes and Control)

Risk Factors of Cancer in Paediatric-onset Inflammatory Bowel Disease in Denmark and Finland
CONCLUSIONS: In this nation-wide study, covering all pIBD patients from Denmark and Finland, we found that pIBD patients treated with thiopurines had an increased risk of TAC.PMID:36961906 | DOI:10.1097/MPG.0000000000003781 (Source: Cancer Control)

Healthy eating and active lifestyle after bowel cancer: feasibility of a randomised controlled trial
Rationale: Evidence from cohort studies indicates that a healthy lifestyle can improve cancer survival.1,2 However, evidence from randomised controlled trials is lacking.3 The study aimed to investigate the feasibility of HEAL ABC lifestyle intervention in patients after colorectal cancer (CRC) surgery and treatment. Things I need you to know: a qualitative analysis of advice-giving statements in early-onset colorectal cancer patients personal accounts published online
Conclusion Healthcare barriers to, and facilitators of, patient self-advocacy may be indicators of quality EORC care. There is a need for greater awareness of the impact of age bias on the responsiveness of clinicians and healthcare services in EOCRC care. Our findings support the development of EOCRC-specific PREMs that can guide age-appropriate policy and practice for this newly identified patient population. (Source: BMJ Open)

Differential to Terminal Ileitis: Terminal Ileum Neuroendocrine Tumor Identified on Screening Colonoscopy
Conclusion: Terminal ileitis encompasses a host of pathologic processes, including inflammatory states, infectious disease, malignancy, and vasculitis. Importantly, small bowel cancer is an increasing cause of terminal ileitis. Screening colonoscopy with ileal intubation can be a valuable tool for early detection of these lesions.PMID:36936484 | PMC:PMC10016213 | DOI:10.31486/toj.22.0035 (Source: Ochsner Journal)
