Effects of Transdermal Testosterone Gel or an Aromatase Inhibitor on Prostate Volume in older men.
CONCLUSION: The tropic effects of testosterone on the prostate are mediated via its aromatization to estradiol. Administration of AI for 12 months to older men was not detrimental to the prostate. Gonadal steroid–dependent effects on bone turnover and bone mineral density in men
CONCLUSIONS. Estrogens primarily regulate bone homeostasis in adult men, and testosterone and estradiol levels must decline substantially to impact the skeleton.TRIAL REGISTRATION. Testosterone Replacement Shows Modest Benefits for Depression and Mood
In addition to improving sexual and physical function, testosterone-replacement therapy may decrease depressive symptoms and improve mood in older men with low testosterone levels, according to a study in the New England Journal of Medicine.A total of 790 men 65 years and older with a serum testosterone concentration of less than 275 ng per deciliter were randomly assigned to receive testosterone gel or placebo gel for 1 year. Each man participated in one or more of three trials—the Sexual Function Trial, the Physical Function Trial, and the Vitality Trial; the latter trial included assessments of fatigue, positive and n...

Acute endothelial response to testosterone gel administration in men with severe hypogonadism and its relationship to androgen receptor polymorphism: a pilot study
Conclusion Administration of TG causes an acute vasodilation and improves arterial stiffness probably due to non-genomic actions of T. Endothelial vasodilatory response was more pronounced depending on higher plasma TT and DHT levels attained. Clinical implications in elderly frail populations are discussed. Sex Steroid Replacement Therapy in Female Hypogonadism from Childhood to Young Adulthood.
Abstract The overall goal of pubertal sex hormone replacement therapy (HRT) in girls is not only about development of secondary sexual characteristics, but also to establish an adult endocrine and metabolic milieu, as well as adult cognitive function. Estradiol (E2) is the first choice for HRT compared to ethinyl estradiol (EE2). E2 is the most potent endogenous estrogen in the circulation, with established levels during spontaneous puberty. Transdermal E2, compared to oral administration, is the first choice to start pubertal HRT. Transdermal application avoids liver exposure to supraphysiologic estrogen ...

The efficacy, bioavailability and safety of a novel hydroalcoholic testosterone gel 2% in hypogonadal men: results from phase II open‐label studies
Summary Pharmacokinetics, pharmacodynamics and safety of a novel hydroalcoholic testosterone gel 2% (TG) were evaluated in phase II sequential dose escalation studies using 3 application sites (thigh, abdomen and shoulder/upper arm) and 2 application methods. Hypogonadal men (n = 40), 18–75 years, with serum testosterone <300 ng dl−1 were included in both studies. Study 1 evaluated hand‐applied multiple doses of TG 1.25, 2.50 and 3.75 ml (23, 46 and 70 mg of testosterone, respectively), once daily for 10 days to shoulder/upper arm. Study 2 evaluated applicator‐applied (TG 1.25, 2.50 and 3.75 ml) versus h...

Effects of aromatase inhibition vs. testosterone in older men with low testosterone: randomized‐controlled trial
The objective of this proof‐of‐concept randomized, double‐blind, placebo‐controlled, parallel‐group, single‐center trial was to determine the relative effects of testosterone (T) and estradiol (E2) on bone mineral density, body composition, and physical performance in older men. The primary outcome was lumbar spine bone mineral density (BMD), and secondary outcomes were body composition, muscle strength, gait speed, and sex hormone concentrations. Forty three men (age range, 65–82 years; mean age 71 years) with low total T levels <350 ng/dL were randomized to one of three groups: 5 g transdermal testos...

An open‐label, phase 2, single centre, randomized, crossover design bioequivalence study of AndroForte 5 testosterone cream and Testogel 1% testosterone gel in hypogonadal men: study LP101
Summary We compared a novel 5% testosterone (T) cream (AndroForte 5, Lawley Pharmaceuticals, Australia) with a 1% T gel (Testogel, Besins Healthcare, Australia). Using an open‐label crossover design, subjects were randomized to one of two treatment sequences using either the T gel or T cream first in a 1 : 1 ratio. Each treatment period was 30 days with a 7–14 days washout period between them. On Days 1 and 30 of each treatment period blood was sampled at −15, −5 min, 0, 2, 4, 5, 6, 7, 8, 9, 10, 12 and 16 h post study drug administration. Sixteen men with established androgen deficiency aged between 29 and 73...

Oral enclomiphene citrate raises testosterone and preserves sperm counts in obese hypogonadal men, unlike topical testosterone: restoration instead of replacement
ConclusionsEnclomiphene citrate consistently increased serum TT, LH and FSH, restoring normal levels of serum TT. Enclomiphene citrate treatment maintained sperm concentrations in the normal range. The effects on TT were also seen with testosterone replacement via testosterone gel but sperm counts were not maintained. (Source: BJU International)

Treatment of pain in fibromyalgia patients with testosterone gel: Pharmacokinetics and clinical response.
Abstract To test our hypothesis that testosterone deficiency plays an important role in chronic pain, a Phase I/II pilot study was initiated with 12 fibromyalgia patients to verify that a daily dose for 28days with transdermal testosterone gel would 1) significantly and safely increase mean serum testosterone concentrations from low baseline levels to mid/high-normal levels, and 2) effectively treat the pain and fatigue symptoms of fibromyalgia. Pharmacokinetic data confirmed that serum free testosterone concentrations were raised significantly above baseline levels, by assessment of maximum hormone concen...

Pii-lba7 in two phase iii studies, endroxal™ (enclomiphene citrate) significantly improves total testosterone levels compared to androgel 1.62%, without suppression of spermatogenesis and testicular function in overweight males with secondary hypogonadism.
Epidemiological studies of hypogonadism have unequivocally shown that the majority of men with clinically low testosterone levels have secondary hypogonadism. This disorder is primarily associated with being overweight (BMI > 25) or obese (BMI > 30) and is neither idiopathic nor due to normal aging. The secondary hypogonadal male has functional but under stimulated testes. These men are typically fertile but exhibit low total testosterone that results from sub-optimal stimulation of the Leydig cells due to suppressed LH secretions by the pituitary. Pd37-07 testosterone gel treatment significantly ameliorates urinary urgency and sleep disturbance: double-blind, placebo-controlled clinical trial
Recent reports suggested that Late-onset hypogonadism (LOH) is closely related to lower urinary tract symptoms (LUTS). It is also reported testosterone replacement therapy may worsen sleep apnea. We developed a new formulation of testosterone gel (2.5%) and carried out a randomized controlled clinical trial and assessed its efficacy for LUTS and sleeping status as well as LOH. The Implications of Low Testosterone on Mortality in Men
Abstract Testosterone levels in men slowly decline with aging and also decline more abruptly due to medical illness or medications. Prescriptions for testosterone have increased dramatically over the past decade, since a testosterone gel formulation was approved and since numerous studies reported an association between low serum testosterone and increased mortality. However, recent observational studies of testosterone treatment have reported conflicting results with some studies reporting decreased risks for mortality while others reported increased mortality risks with testosterone treatment. This pape...

Effects of Three Different Testosterone Formulations in Female‐to‐Male Transsexual Persons
ConclusionsOne‐year testosterone administration in FtM transsexuals appears to be very safe with no differences among the testosterone formulations used. Our study is preliminary, and the detection of subtle or long‐term differences in the effects of the three formulations may require further larger and longer term studies in this and other populations.
