Combination of Rivaroxaban and Amiodarone Increases Bleeding in Patients With Atrial Fibrillation
CONCLUSION AND RELEVANCE: The combination of rivaroxaban and amiodarone increased the risk of bleeding in patients with atrial fibrillation, especially clinically relevant nonmajor and minor bleeding. Physicians prescribing rivaroxaban and amiodarone together should be concerned about an increase in the risk of nonmajor bleeding.PMID:37960871 | DOI:10.1177/10600280231211306 (Source: The Annals of Pharmacotherapy)

Structural modeling of hERG channel –drug interactions using Rosetta
The human ether-a-go-go-related gene (hERG) not only encodes a potassium-selective voltage-gated ion channel essential for normal electrical activity in the heart but is also a major drug anti-target. Genetic hERG mutations and blockage of the channel pore by drugs can cause long QT syndrome, which predisposes individuals to potentially deadly arrhythmias. However, not all hERG-blocking drugs are proarrhythmic, and their differential affinities to discrete channel conformational states have been suggested to contribute to arrhythmogenicity. We used Rosetta electron density refinement and homology modeling to build structur...

Key CT and MRI findings of drug-associated hepatobiliary and pancreatic disorders
AbstractObtaining an imaging diagnosis of various hepatobiliary and pancreatic disorders caused by certain drugs can often be challenging. Familiarity with these conditions may improve diagnostic accuracy and patient management. This review aimed to describe the imaging findings of drug-associated hepatobiliary and pancreatic disorders and identify suggestions for obtaining a correct diagnosis. We focused on relatively common disorders or those that can present with characteristic imaging findings, such as drug-induced acute hepatitis, sinusoidal obstruction syndrome, focal nodular hyperplasia-like lesions, hepatocellular ...

Prevention of postoperative atrial fibrillation in cardiac surgery: a quality improvement project
CONCLUSIONS: This single-centre QI intervention increased the use of prophylactic amiodarone by 28% for patients at elevated risk of POAF, with no change in the early postoperative initiation of beta blockers (46.7% of patients by POD1). There was a notable reduction in the incidence of POAF in patients at elevated risk undergoing surgery.PMID:37919634 | DOI:10.1007/s12630-023-02619-8 (Source: Canadian Journal of Anaesthesia)

Use of mexiletine in therapy-refractory recurrent ventricular tachycardia storm
AbstractElectrical storm due to recurrent ventricular tachycardias (VTs) is a  life-threatening arrhythmic emergency. The authors present a case report of a 69-year-old male patient with VT storm of non-ischemic etiology. Despite optimal medical treatment escalated by amiodarone antiarrhythmic drug therapy, the patient experienced multiple implantable cardioverter defibril lator (ICD) shocks. An electrophysiological study revealed an epicardial substrate; however, considering the patient’s extreme obesity and active anticoagulant effect, catheter ablation was deemed to be unfeasible. Subsequently, mexiletine was added...

Drug-induced interstitial lung disease: insights from a referral center
Conclusion: DI-ILD is a diagnosis of exclusion requiring high index of suspicion. Timely identification and removal of the causative drug is critical to reduce the likelihood of permanent lung damage. Larger cohorts and registries to establish management algorithms are sorely needed. (Source: European Respiratory Journal)
