Glycoprotein IIb-IIIa inhibitors for acute ischaemic stroke.
CONCLUSIONS: The available trial evidence showed that, for individuals with acute ischaemic stroke, GP IIb-IIIa inhibitors are associated with a significant risk of intracranial haemorrhage with no evidence of any reduction in death or disability in survivors. These data do not support their routine use in clinical practice. The conclusion is driven by trials of Abciximab, which contributed 89% of the total number of study participants considered. In vitro effects of the glycoprotein IIb/IIIa receptor antagonists abciximab and eptifibatide on platelet aggregation in healthy cats.
Conclusions and Clinical Relevance-Eptifibatide caused a significant reduction in platelet aggregation in vitro, but there was no identifiable antiplatelet effect for abciximab. Eptifibatide and abciximab have different binding and inhibitory actions; therefore, it can be hypothesized that abciximab would be ineffective in cats because of a lack of receptor binding, reduced binding kinetics, or lack of downstream signaling. Eptifibatide may be useful in identifying hyperreactive platelets in cats in an in vitro platelet inhibitory assay. Stent thrombosis with an aneurysm 7 years after a drug eluting stent implantation.
We report a case of very late stent thrombosis 7 years post sirolimus eluting stent implantation presenting as ST elevation MI while on dual antiplatelet therapy. Angiography revealed an aneurysm at the proximal end of the stent. The patient was managed successfully by primary percutaneous coronary intervention (PCI) with adjunct thrombus aspiration and intracoronary abciximab administration followed by deploying a mesh-covered stent MGuard. This very late complication is a rare presentation after a drug illuting stent (DES). PMID: 24814120 [PubMed - in process] (Source: Indian Heart J)

Off-target effects of glycoprotein IIb/IIIa receptor inhibitors.
Abstract Soon after identification of the platelet membrane glycoprotein (GP) IIb/IIIa receptor and due to its key role in platelet aggregation, it became the target of antithrombotic therapy. There are three intravenous glycoprotein IIb/IIIa receptor inhibitors (GPI) used in clinical practice, particularly in patients with acute coronary syndrome undergoing percutaneous coronary intervention. The aim of the current review is to summarize available knowledge concerning off-target effects of GPI. All three drugs have similar antithrombotic properties, but differ with respect to pharmocodynamics, pharmacokin...

Outcomes in Patients With ST-Segment Elevation Acute Myocardial Infarction Treated With Clopidogrel Versus Prasugrel (from the INFUSE-AMI Trial)
Prasugrel is more potent than clopidogrel, but it is not known whether this translates into clinical benefit in patients undergoing primary percutaneous coronary intervention (PCI) with bivalirudin for ST elevation myocardial infarction. In the Intracoronary Abciximab and Aspiration Thrombectomy in Patients With Large Anterior Myocardial Infarction trial, 452 patients with anterior STEMI undergoing primary PCI with bivalirudin were randomized to intralesional abciximab or placebo and to thrombus aspiration or no aspiration. Clopidogrel or prasugrel were administered at physician discretion. The primary end point was infarc...

Treatment of Radial Artery Occlusions Using Balloon Angioplasty and Localized Intra‐Arterial Abciximab
ConclusionsBalloon angioplasty and intrathrombus administration of abciximab via a catheter appears to be a safe, effective, and durable technique for reestablishing the patency of an occluded radial artery following transradial catheterization. Larger studies are needed to confirm our findings and establish the role for this technique in an algorithm for treatment of RAO. (Source: Journal of Interventional Cardiology)

The effectiveness of different functional fibrinogen polymerization assays in eliminating platelet contribution to clot strength in thromboelastometry.
CONCLUSIONS: FFPAs based solely on glycoprotein-IIb/IIIa inhibition, such as FFTEG or EXTEM-ABC, are less effective than cytochalasin D-based assays, such as FIBTEM or FIBTEM-S, at inhibiting the platelet component of clot strength. The FIBTEM PLUS assay, and the combination of FIBTEM and abciximab, sufficiently inhibits platelet contribution to clot elasticity. The combination of a glycoprotein-IIb/IIIa receptor blocker and cytochalasin D allows evaluation of functional fibrinogen polymerization without platelet "noise." In a clinical setting, the significance of potent platelet inhibition ensures a more accurate assessme...

Aspiration of intact coronary bifurcation thrombus in ST-elevation myocardial infarction
We report a case of a 74-year-old female patient with acute chest pain who was admitted for primary percutaneous coronary angioplasty. The electrocardiogram indicated an acute ST-elevation myocardial infarction of the anterolateral wall. Coronary angiography showed a thrombus-containing lesion in the left anterior descending coronary artery with absence of a major diagonal branch (A; arrowhead). Administration of abciximab was not considered as there was some leakage of blood from the punctured, calcified femoral artery. An aspiration catheter was slowly advanced into the lesion, before vacuum-suction was initiated to manu...

Systematic evaluation of evidence on veterinary viscoelastic testing Part 5: Nonstandard assays
ConclusionsNontraditional assays identified included thrombelastography (TEG)‐PlateletMapping (PM), functional fibrinogen assessment, and rapid‐TEG (r‐TEG). Direct veterinary evidence was found for only the ADP‐activated PM, which appears to generate valid data in dogs but not cats or horses. Arachidonic acid activated PM shows high variability and requires further assessment and validation in veterinary species. Functional fibrinogen assays may be performed in veterinary species but may require modification due to species differences in response to abciximab. While tissue factor (TF)‐activated TEG has been well ...

Construction and characterization of a novel chimeric antibody c3C7 specific for the integrin αIIbβ3 complex.
In conclusion, after the modification, c3C7 retained the properties of its parent mAb with no loss of its biological activity. Therefore, c3C7 has the potential to become a novel agent for the treatment of thrombosis. PMID: 24113827 [PubMed - as supplied by publisher] (Source: Applied Microbiology and Biotechnology)

TEG and ROTEM: Technology and clinical applications
ABSTRACT Initially described in 1948 by Hertert thromboelastography (TEG) provides a real‐time assessment of viscoelastic clot strength in whole blood. Rotational thromboelastometry (ROTEM) evolved from TEG technology and both devices generate output by transducing changes in the viscoelastic strength of a small sample of clotting blood (300 uL) to which a constant rotational force is applied. These point of care devices allow visual assessment of blood coagulation from clot formation, through propagation and stabilization, until clot dissolution. Computer analysis of the output allows sophisticated clot formation/dissol...
