Usefulness of the Left Anterior Descending Artery Wrapping Around the Left Ventricular Apex to Predict Adverse Clinical Outcomes in Patients with Anterior Wall ST-segment Elevation Myocardial Infarction (An INFUSE-AMI Substudy)
The relationship between left anterior descending artery (LAD) anatomical features and clinical outcomes in patients with anterior ST-segment elevation myocardial infarction (STEMI) has not been fully investigated. The INFUSE-AMI trial randomized 452 patients with anterior STEMI undergoing mechanical revascularization to intralesional abciximab versus no abciximab and to manual thrombus aspiration versus no aspiration. The primary endpoint was infarct size (% left ventricle [LV] mass) by contrast magnetic resonance imaging (cMRI) at 30 days. (Source: The American Journal of Cardiology)

Pharmaceutical aspects of anti-inflammatory TNF-blocking drugs
Abstract Tumor necrosis factor (TNF) is a key regulator of inflammatory processes in several immune-mediated inflammatory diseases such as rheumatoid arthritis, ankylosing spondylitis, Crohn’s disease, ulcerative colitis, psoriasis and psoriatic arthritis. Inactivating TNF has been found to be a plausible approach in treating these conditions. Two major strategies have been adopted by scientists to inactivate TNF: one is to use monoclonal antibodies (mAbs) that bind to TNF, and the other is to use fusion proteins that bind to TNF, both inactivate TNF and help to prevent TNF-mediated inflammatory process...

Carotid stenting and intracranial thrombectomy for treatment of acute stroke due to tandem occlusions with aggressive antiplatelet therapy may be associated with a high incidence of intracranial hemorrhage
Conclusions Primary stenting of the extracranial carotid artery combined with intracranial mechanical thrombectomy can be an effective treatment for tandem occlusions, can be performed with a high rate of technical success, and can achieve good clinical outcomes in selected patients. However, the incidence of SICH may be higher than in other patient populations and may be associated with the use of abciximab and advanced patient age. Similar anti-inflammatory effects of intracoronary and intravenous Abciximab during primary percutaneous coronary intervention: a randomized study
Conclusions: Our study suggests that Abciximab has similar anti-inflammatory effects irrespective of the administration route. It is unlikely that the potential clinical benefits of intracoronary Abciximab can be related to modulation of integrin receptors. (Source: Journal of Cardiovascular Medicine)

Utility of Peak Creatine Kinase-MB Measurements in Predicting Myocardial Infarct Size, Left Ventricular Dysfunction and Outcome After First Anterior Wall Acute Myocardial Infarction (From the INFUSE-AMI Trial)
Infarct size after ST-segment elevation myocardial infarction (STEMI) is associated with long-term clinical outcomes. However, there is insufficient information correlating CK-MB or troponin levels to infarct size and infarct location in first–time occurrence STEMI. We, therefore, assessed the utility of CK-MB measurements after primary percutaneous coronary intervention (PCI) of a first anterior STEMI utilizing bivalirudin anticoagulation in patients who were randomized to intralesion abciximab vs. (Source: The American Journal of Cardiology)

Prophylaxis of Experimental Endocarditis With Antiplatelet and Antithrombin Agents: A Role for Long-term Prevention of Infective Endocarditis in Humans?
Conclusions. Antiplatelet and direct antithrombin agents may be useful in the prophylaxis of IE in humans. In particular, the potential dual benefit of dabigatran etexilate might be reconsidered for patients with prosthetic valves, who require life-long anticoagulation and in whom S. aureus IE is associated with high mortality. Sex, adverse cardiac events, and infarct size in anterior myocardial infarction: An analysis of Intracoronary Abciximab and Aspiration Thrombectomy in Patients With Large Anterior Myocardial Infarction (INFUSE-AMI)
Conclusions In the INFUSE-AMI randomized trial, women with anterior STEMI experienced a higher rate of MACE, attributable to older age. Despite longer delay from symptom onset to reperfusion therapy, there was no difference between women and men in infarct size or reperfusion success. (Source: American Heart Journal)

The regional STEMI network registry: benefit of the introduction of the 24/7 primary PCI service.
CONCLUSIONS: The introduction of the 24/7 primary PCI regional service (the STEMI network) increased the accessibility of the invasive diagnosis and treatment, the frequency of the reperfusion therapy and improved the results of the treatment of patients suffering from acute ST-segment elevation myocardial infarction (STEMI). The patients with STEMI treated invasively had better outcome. PMID: 25428812 [PubMed - as supplied by publisher] (Source: Polish Heart Journal)

Safety and efficacy of tirofiban as adjunctive therapy for patients with ST-elevation myocardial infarction: a comparison versus placebo and abciximab.
Authors: Diaz JF, Cardenal R, Gomez-Manchero A, Sanchez-Gonzalez C Abstract Tirofiban is a nonpeptide tyrosine derivative that together with eptifibatide (both small molecules) and abciximam belongs to the group of glycoprotein IIb/IIIa inhibitors. Though similar to abciximab in that it has a high affinity for the GP IIbIIIa inhibitor receptor, tirofiban dissociates from it much faster tan abciximab, what makes its action reversible in a few hours. Initially used upstream for treatment of patients with non ST-elevation acute coronary síndromes, recent evidence has shown its role as adjuntive therapy in pa...

Intracoronary injection of glycoprotein IIb/IIIa, abciximab, as adjuvant therapy in primary coronary intervention.
Authors: Rognoni A, Lupi A, Cavallino C, Rosso R, Veia A, Bacchini S, Bongo AS Abstract Acute coronary syndromes and, in paticular, ST - segment elevation myocardial infarction are the principle causes or mortality and morbidity in the industrialized countries. The manadgement of acute myocadial infarction is much debated in the literature; primary percutaneous coronary intervention is the treatment of choice. In the recent years there has been an increasing interest in the concept of adjunctive pharmacological therapy to improve outcomes in primary percutaneous coronary intervention. In the literature ran...

Abciximab‐induced alveolar hemorrhage treated with rescue extracorporeal membranous oxygenation
We describe a case of a 75 year‐old woman presenting emergently with an anterior S‐T elevation myocardial infarction that deteriorated into ventricular fibrillation requiring prompt resuscitation, resulting in cardiogenic shock. Emergency primary percutaneous coronary intervention of the left anterior descending coronary artery with adjunctive abciximab and heparin resulted in adequate coronary flow, and intraaortic balloon pump was used to support hemodynamics. Within 12 hours of intervention, she developed acute respiratory distress with four‐quadrant opacification of lung fields, difficulty with oxygenation, and h...
