The interaction between intestinal microenvironment and stroke
CONCLUSION: The structure and function of the intestinal environment can influence neurological function and cerebral ischemic outcome. Improving the intestinal microenvironment by targeting the gut microbiota may be a new direction in treating stroke.PMID:37309254 | DOI:10.1111/cns.14275

A Cable-Driven Exoskeleton With Personalized Assistance Improves the Gait Metrics of People in Subacute Stroke
This study aimed to determine the effects of SEAExo with personalized assistance on immediate changes in gait performance of people after stroke. Gait metrics (i.e., the foot contact angle, knee flexion peak, temporal gait symmetry indices) and muscle activities were the primary outcomes to evaluate the assistive performance. Seven subacute stroke survivors participated and completed the experiment with three comparison sessions, i.e., walking without SEAExo (served as baseline) and without/with personalized assistance, at their preferred walking speeds. Compared to the baseline, we observed increases in the foot contact a...

Astragaloside IV alleviates neuronal ferroptosis in ischemic stroke by regulating fat mass and obesity ‐associated—N6‐methyladenosine—acyl‐CoA synthetase long‐chain family member 4 axis
Ischemic stroke (IS) is a detrimental neurological disease with limited treatments options. Astragaloside IV (As-IV) was a promising bioactive constituent in the treatment of IS. In our study, As-IV promoted the transcription of fat mass and obesity-associated (Fto) by upregulating activating transcription factor 3(Atf3), resulting in a decrease of long-chain acyl-CoA synthetase-4 (Acsl4) N6-methyladenosine (m6A) levels, thus improving neuronal injury in IS by inhibiting ferroptosis. These findings provided a better understanding of the molecular mechanisms underlying the neuroprotective effects of As-IV and led to novel t...

Real world evidence of improved attention and cognition during physical therapy paired with neuromodulation: a brain vital signs study
DiscussionThe current study highlighted real-world neuromodulation improvements in neurophysiological correlates of attention. Overall, the real-world findings support the concept of neuromodulation-related improvements extending beyond physical therapy to include potential attention benefits for cognitive rehabilitation.

Mediation effect of stroke recurrence in the association between post-stroke interleukin-6 and functional disability
CONCLUSIONS: Stroke recurrence mediates less than 20% of the association between IL-6 and functional outcome at 90 days among patients with acute ischemic stroke. In addition to typical secondary prevention strategies for preventing stroke recurrence, more attention should be paid to novel anti-inflammatory therapy to improve functional outcomes directly.PMID:37287421 | DOI:10.1111/cns.14289

Effect of left ventricular ejection fraction Spectrum on 1-Year mortality in patients with acute ischemic stroke or transient ischemic attack
CONCLUSIONS: Patients with AIS or TIA with decreased LVEF (≤60%) had a lower 1-year survival rate after onset. LVEF 50%-60% even within the normal range, may still contribute to poor outcomes in AIS or TIA. Comprehensive evaluation of cardiac function after acute ischemic cerebrovascular disease should be strengthened.PMID:37287419 | DOI:10.1111/cns.14285

Supporting Post-Stroke Language and Cognition with Pharmacotherapy: Tools for Each Phase of Care
AbstractPurpose of ReviewThere is enormous enthusiasm for the possibility of pharmacotherapies to treat language deficits that can arise after stroke. Speech language therapy remains the most frequently utilized and most strongly evidenced treatment, but the numerous barriers to patients receiving the therapy necessary to recover have motivated the creation of a relatively modest, yet highly cited, body of evidence to support the use of pharmacotherapy to treat post-stroke aphasia directly or to augment traditional post-stroke aphasia treatment. In this review, we survey the use of pharmacotherapy to preserve and support l...
