Relationship Between Cerebral Blood Flow and Blood Pressure in Long-Term Heart Transplant Recipients Cerebral Blood Flow
Heart transplant recipients are at an increased risk for cerebral hemorrhage and ischemic stroke; yet, the exact mechanism for this derangement remains unclear. We hypothesized that alterations in cerebrovascular regulation is principally involved. To test this hypothesis, we studied cerebral pressure-flow dynamics in 8 clinically stable male heart transplant recipients (62±8 years of age and 9±7 years post transplant, mean±SD), 9 male age-matched controls (63±8 years), and 10 male donor controls (27±5 years). To increase blood pressure variability and improve assessment of the pressure-f...

Hypertension care in Aseer region, Saudi Arabia: Barriers and solutions.
This study revealed that HTN patients received insufficient care, which could be attributed to many different barriers. In order to improve the quality of HTN care for HTN, these barriers should be overcome by implementation of the recommendations. PMID: 25394461 [PubMed - in process]

Quantitative computed tomography of pulmonary emphysema and ventricular function in chronic obstructive pulmonary disease patients with pulmonary hypertension.
CONCLUSION: The study revealed that percent emphysema is correlated with RV dysfunction among COPD patients with PH. Based on our findings, percent emphysema can be considered for use as an indicator to predict the severity of right ventricular dysfunction among COPD patients. PMID: 25469102 [PubMed - in process]

RV-pulmonary arterial coupling predicts outcome in patients referred for pulmonary hypertension
Conclusions The adequacy of RV functional adaptation to afterload predicts survival in patients referred for PH. Whether this can simply be evaluated using RV volumetric imaging will require additional confirmation.

Hemodynamic effects of different vasodilators in patients with type II pulmonary hypertension (PH)
Conclusions. Inhaled Iloprost causes favorable changes in preload and afterload of the impaired LV and increases its performance in patients with PH associated with LV systolic dysfunction.

Has Brazil found the way to better health care?
Under Brazil’s family health program, when a woman learns that she is pregnant, she contacts her local community health agent, who often is a neighbor. Typically, the agent visits her home to arrange an appointment with the neighborhood’s family health team, and the woman visits the health center for an assessment by a nurse assistant and a physician. During the pregnancy, if she misses a prenatal care appointment, the agent checks in on her at home and helps her reschedule her visit. Any prenatal medications she needs are provided free of charge. Brazil — home to the world’s fifth-largest population and seventh-l...

Pre-existing diabetes and risks of morbidity and mortality after liver transplantation: A nationwide database study in an Asian population.
CONCLUSION: DM is associated with elevated risk of 90-day post-LTx. Moreover, DM patients with coexisting renal manifestations exhibited an increased postoperative risk of mortality after LTx. PMID: 26048000 [PubMed - as supplied by publisher]

Hemodynamic monitoring during liver transplantation: A state of the art review.
Authors: Rudnick MR, Marchi LD, Plotkin JS Abstract Orthotopic liver transplantation can be marked by significant hemodynamic instability requiring the use of a variety of hemodynamic monitors to aide in intraoperative management. Invasive blood pressure monitoring is essential, but the accuracy of peripheral readings in comparison to central measurements has been questioned. When discrepancies exist, central mean arterial pressure, usually measured at the femoral artery, is considered more indicative of adequate perfusion than those measured peripherally. The traditional pulmonary artery catheter is less frequentl...

Frequency of Cardiovascular Events and Effect on Survival in Liver Transplant Recipients for Cirrhosis Due to Alcoholic or Nonalcoholic Steatohepatitis.
CONCLUSIONS: Cardiovascular events occurred with similar frequency in transplant recipients for nonalcoholic steatohepatitis or alcoholic cirrhosis. Patient survival was affected in both groups, but male patients with concomitant hepatitis C virus infection remained at higher risk for a cardiovascular event after liver transplant. Development of a cardiac evaluation protocol for liver transplant recipients could help monitor these patients. PMID: 26581602 [PubMed - as supplied by publisher]

Novel 24-h ovine model of brain death to study the profile of the endothelin axis during cardiopulmonary injury
Conclusions: This novel, clinically relevant, ovine model of brain death demonstrated that increased pulmonary artery pressures are observed after brain death. This may contribute to right ventricular dysfunction and pulmonary injury. The development of this model will allow for further investigation of therapeutic strategies to minimise the deleterious effects of brain death on potentially transplantable organs.

The Safety of Erythropoiesis-Stimulating Agents for the Treatment of Anemia Resulting from Chronic Kidney Disease
Abstract Chronic kidney disease (CKD) anemia treatment was revolutionized in the late 1980s with the introduction of recombinant human erythropoietin. This and related erythropoiesis-stimulating agents (ESAs) greatly benefited patients by decreasing debilitating symptoms, improving their quality of life, and freeing them from dependence on blood transfusions with their associated complications such as infections, sensitization impeding transplantation, and secondary iron overload. However, even in the initial studies, untoward effects were noted in patients receiving ESAs, including worsening hypertension, seizure...

Metabolic Syndrome After Hematopoietic Cell Transplantation: At the Intersection of Treatment Toxicity and Immune Dysfunction
Hematopoietic cell transplantation (HCT) survivors face a multitude of short- and long-term health complications in the years following treatment. One important health complication that is associated with significant morbidity is metabolic syndrome (MetSyn). This constellation of findings, including obesity, glucose and lipid dysmetabolism, and hypertension, place affected individuals at increased risk for type 2 diabetes mellitus, cardiovascular complications and stroke. Previous studies have linked MetSyn in HCT survivors to prior treatment; however, few have addressed the potential roles of systemic inflammation and imm...

The Canadian experience using the expanded criteria donor classification for allocating deceased donor kidneys for transplantation
Conclusions The ECD classification identifies kidneys at increased risk for graft loss in Canadian patients. The performance of more granular measures of donor risk (e.g., Kidney Donor Risk Index) and its impact on organ allocation/utilization in Canadian patients requires further study.
