Study finds obese people may struggle to reach a healthy weight
ConclusionThis research makes use of a general practice database providing just under 10 years of BMI observations for a large, nationally representative UK sample. It demonstrates that low proportions of people in the obese categories were able to achieve a normal BMI over a year of follow-up, and the common problem of weight cycling. However, there are points to consider when interpreting these results: The probability of obtaining a normal BMI over a year was very low: only 1 in 210 for men and 1 in 124 for women in the "simple obese" category of 30 to 35kg/m2, and much lower than that for the higher categor...

Is there really a middle-class drinking 'epidemic' in over-50s?
Conclusion This study showed that higher-risk drinking was linked to a number of factors the researchers described as "middle-class", like higher educational attainment, being socially active and good ratings of health. Professor Jose Iparraguirre, author of the research, said in the Guardian: "Because this group is typically healthier than other parts of the older population, they might not realise that what they are doing is putting their health in danger". There are a few reasons to be cautious with these findings. The study produced a lot of results, so there is a risk some were chance findings. Thi...

Relationship of sitting time and physical activity with non-alcoholic fatty liver disease
Physical activity is well-recognized to reduce the incidence and mortality of various chronic diseases, including cardiovascular disease, diabetes, stroke and several types of cancer [1–3]. More than one half of the average person’s waking day involves sedentary activities associated with prolonged sitting such as watching TV and using the computer [4]. Recently, the deleterious effects of sedentary behavior regardless of additional physical activity have received a great deal of attention [5–8].

Low alanine aminotransferase levels and higher number of cardiovascular events in people with Type 2 diabetes: analysis of the Fenofibrate Intervention and Event Lowering in Diabetes (FIELD) study
ConclusionsThe data may indicate that in people with Type 2 diabetes, which is associated with higher alanine aminotransferase levels because of prevalent non‐alcoholic fatty liver disease, a low alanine aminotransferase level is a marker of hepatic or systemic frailty rather than health. This article is protected by copyright. Association of whole blood viscosity with non-alcoholic fatty liver disease.
CONCLUSIONS: The findings showed that WBV at low shear stress is increased in NAFLD. Moreover, WBV at low shear stress is independently associated with NAFLD even after adjusting other cardiovascular risk factors. PMID: 26444605 [PubMed - as supplied by publisher]

Minocycline-induced pigmentation of the skin and nails
Introduction Minocycline is an effective treatment for inflammatory cutaneous conditions, such as acne vulgaris and rosacea, infectious conditions and has recently been investigated for its neuroprotective properties. However, treatment with minocycline can induce iatrogenic pigmentation, affecting both the skin and other organs. Recognition of these changes may avoid unnecessary investigations. This image case report aims to remind clinicians of this often-persistent side effect of minocycline. Case A 73-year-old man attended the dermatology outpatient clinic with a 20-year history of progressive swelling of the nose, ass...

Hemodialysis with end-stage renal disease did not raise the risk of intracranial hemorrhage after a head injury
Conclusions: HD +ESRD did not increase the post-HI risk of ICH. Therefore, it may not be necessary to lower the threshold of head CT in HD +ESRD patients.

Increased activity of factor VIII causing acute intestinal ischemic disorder with recurrent and multiple arterial thromboses
We present the case of a 52-year-old man with hypercoagulability-induced acute arterial occlusive mesenteric ischemia. The patient suffered from arterial occlusive mesenteric ischemia, recent ischemic stroke, and previous renal infarction. He had no history of atrial fibrillation, atherosclerosis, vascular diseases, autoimmune diseases, malignancy, liver diseases, copper deficiency, or other risk factors for arterial thromboembolism. Segmental resection of the gangrenous bowel with primary end-to-end anastomosis was performed. A pathological examination revealed multiple thrombi within the mesenteric arteries of the infarc...

Donor risk factors in orthotopic liver transplant: analysis of the OPTN/UNOS registry.
This study of the United Network for Organ Sharing registry data analyzed reputed donor factors using the post-MELD data and evaluated them further in HCV positive and HCV negative recipients with hepatic cirrhosis to develop a modified DRI (mDRI). We found that HCV negative cirrhotic recipients have generally higher tolerance against inferior qualities of donors than HCV positive cirrhotic patients. In addition, the results revealed the post-MELD prognostic factors that should be considered in the donor procurement processes. The group of "all recipients" and the subset of HCV positive cirrhotic recipients showed a simila...

Sedating the apprehensive debilitated patients for dental procedures by combining parenteral sedation and hypnosis with supplemental acupuncture therapy.
Authors: Lu DP, Wu PS, Lu WI Abstract Treating apprehensive debilitated patients (i.e. geriatric patients, patients with cardiac, pulmonary, kidney, or liver diseases, and those with other severe systemic conditions) for dental procedures can cause unexpected medical complications such as cardiac arrest, stroke, asthma or shock, etc. Due to diminishing functional capacities of their organs, sedating those patients with sedative drugs in normal regular dosage could increase the risk of adverse events for this group of patients and can also increase the risk of liability for the clinician. The authors treated 34 appr...

Relation Between Hepatitis C Virus Exposure and Risk of Osteoporosis: A Nationwide Population-Based Study
Abstract: The effect of hepatitis C virus (HCV) exposure on bone mineral density without advanced liver disease remains debated. Thus, we assessed the relation between HCV exposure and the risk of osteoporosis. From 2000 to 2011, patients aged>20 years with HCV exposure were identified from the Longitudinal Health Insurance Database 2000. Of the 51,535 sampled patients, 41,228 and 10,307 patients were categorized as the comparison and the HCV exposure cohorts, respectively. The overall incidence of osteoporosis in the HCV exposure cohort was higher than in the comparison cohort (8.27 vs 6.19 per 1000 person-years; crude ha...

Nonalcoholic Fatty Liver Disease and Mean Platelet Volume: A Systemic Review and Meta-analysis
Background: Nonalcoholic fatty liver disease (NAFLD) is linked to obesity, metabolic syndrome, and cardiovascular disease. Increased mean platelet volume (MPV), a marker of platelet activity, is associated with acute myocardial infarction, stroke, thrombosis, and increased mortality after myocardial infarction. The purpose of this study was to perform a meta-analysis to investigate the relationship between NAFLD and MPV. Methodology: A systematic search of MEDLINE (Ovid), PubMed, and CINAHL databases from 1950 to May 2014, complemented with manual review of references of published articles for studies comparing MPV in pati...

Cardiac remodelling identified by cardiovascular magnetic resonance in patients with hepatitis C infection and liver disease
Abstract Chronic cardiac dysfunction in patients with chronic liver disease (CLD) in the absence of alcohol consumption or other cardiac disease is well described. Whilst functional and morphological features of this condition remain unclear, diastolic dysfunction has been implicated by echocardiography. We aimed to evaluate myocardial structure, function and tissue composition with cardiac magnetic resonance (CMR) imaging in patients with hepatitis C and histological evidence of liver disease on biopsy. Contrast-enhanced CMR imaging for morphological, functional and tissue characterization was performed on 16 pat...
