What Are the Types of Tracheoesophageal Fistulas?
Discussion Respiratory problems in infants can caused by many things including any type of obstruction from the nose to lung. Neonates and infants are particularly susceptible to changes in the size of the airway because of the physics of airflow resistance. Important reminders about infant airways: Resistance = 1/radius4 (Poiseuille’s equation) therefore even a 1 mm decrease in the airway circumferences increases the airflow resistance x16. Increased airflow causes less pressure along the walls, which can lead to collapse of the walls (Bernoulli’s and Venturi effects). Neonates have smaller lungs relative to ...

The incidence of skin lesions in contrast media-induced chemical hypersensitivity.
Authors: Iordache AM, Docea AO, Buga AM, Mitrut R, Albulescu D, Zlatian O, Ianosi S, Ianosi G, Neagoe D, Sifaki M, Rogoveanu OC, Branisteanu DE, Calina D Abstract Contrast agents are used in radiology to increase the sensibility and specificity of radiological techniques. Some of these compounds have side effects that include organ toxicity (with kidney being the most affected organ) and hypersensitivity reactions. We performed multiple PubMed searches from January, 2008 to January, 2018 for studies regarding adverse reactions to compounds used as contrast agents in imagistic techniques. The initial research identi...

Intralymphatic Immunotherapy: Update and Unmet Needs
Allergen-specific immunotherapy (AIT) is the only allergy treatment that confers long-term symptom amelioration for patients suffering from allergy. The most frequently used allergen application route is subcutaneous injection (SCIT), commonly taken as the gold standard, followed by sublingual (SLIT) or oral (OIT) application of allergen preparations. This is an up-to-date review of the clinical evidence for a novel route of allergen application, i.e., directly into lymph nodes – intralymphatic immunotherapy (ILIT). The major advantages of ILIT over the current AIT approaches are its short duration and the low allergen d...

Postpartum stretchmarks: repairing activity of an oral Centella asiatica supplementation (Centellicum ®).
CONCLUSIONS: Oral supplementation with Centellicum® appears to improve SMs in a relatively short period of time. Larger studies are needed to evaluate the protective and repairing potentials of CA. PMID: 30293400 [PubMed - in process]

What Do We Really Know About Intralymphatic Immunotherapy?
AbstractPurpose of reviewAllergen-specific immunotherapy (AIT) is the only causal treatment method of IgE-mediated allergies that may lead to long-term symptom amelioration even after the end of treatment, positively interfere with the course of disease, and improve the immunological situation of the patient. While AIT in general requires treatment periods over 3 up to 5  years, intralymphatic immunotherapy (ILIT) needs only three ultrasound-guided injections of low allergen doses into inguinal lymph nodes with 4-week time interval making the entire treatment possible within 2 months.Recent findingsThe number of publishe...

Superb Micro-vascular Imaging (SMI): a Doppler ultrasound technique with potential to identify, classify, and follow up endoleaks in patients after Endovascular Aneurysm Repair (EVAR)
ConclusionsThe analysis showed that SMI is effective, repeatable, and comparable with the CEUS modality in identification endoleaks after EVAR; it may be considered as a potential tool to monitor patients after EVAR implantation, especially those with renal insufficiency or with an allergy to any contrast media.

Oncocytic thyroid carcinoma and multiple sclerosis: Fortuitous or drug-related association?
Authors: Handra-Luca A, Bacci F Abstract Sir, Thyroid tumors occur rarely as associated to nervous system diseases. We had recently studied a case of thyroid carcinoma, peculiar by the predominant eosinophilic/oncocytic/oxyphil/Hurthle cell-component associated to multiple sclerosis (MS). The patient (man, 31-years old) presented with hair loss, diarrhea, thermophobia, tremor and tachycardia. The medical history revealed allergy (pollen, prawn), recurrent uveitis and optic nevritis (at the age of 12-years). A Behcet disease was suspected. Corticoid treatment was given for more than 6 years. Actually, a right iso- a...

Epinephrine, auto-injectors and anaphylaxis: challenges of dose, depth and device.
CONCLUSION: EAIs should be available in the community with doses and needle depths that meet the needs of all patients. More research on epinephrine pharmacodynamics are needed in children and adults in anaphylaxis, in order to better delineate what optimal doses should be. Optimizing epinephrine dose and delivery has the potential to improve anaphylaxis outcomes and prevent adverse events. PMID: 29746901 [PubMed - as supplied by publisher]

Endovascular Aortic Repair Follow up Protocol Based on Contrast Enhanced Ultrasound Is Safe and Effective.
CONCLUSIONS: The introduction of a CEUS based protocol for EVAR follow up was safe and effective and it was similar to the previous CTA based follow up protocol with regard to identification of endoleaks in a mid-term period. Moreover, CEUS allowed for 90% reduction of CTA, thereby decreasing radiation exposure for patients. PMID: 29673811 [PubMed - as supplied by publisher]

Vascular mapping for the decision of hemodialysis access method: Case report.
Authors: Uchino T, Nakai H, Ichihashi H, Sasaki T, Azuma N Abstract Preoperative ultrasound vascular mapping was performed to determine the surgical method for two cases where standard arteriovenous fistula was not possible. In case 1, the previous arteriovenous fistula of the left arm was occluded, and arteriovenous graft of the right arm was thrombosed after frequent percutaneous transluminal angioplasty. Contrast medium was contraindicated because of allergy. Using preoperative ultrasound vascular mapping, we were able to establish the appropriate surgical method. In case 2, hemodialysis was started by catheter ...

A Diagnostic Dilemma: Sometimes You ’ ll Never Know
Discussion It can be difficult to consider what is the problem when a health care provider is presented with a group of seemingly disparate signs and symptoms with a history and time course that do not match classic (or commonly atypical) disease presentations. Common things happen commonly, so when symptoms occur without the usual co-occurring symptoms, unusual symptoms or time courses, it can be challenging. The provider must develop a reasonable differential diagnosis and balance how far to go with the evaluation against how much to monitor the patient for new signs or symptoms which can provide new information. Patient...

Extracranial vertebral artery dissection: Findings and advantages of ultrasonography
Vertebral artery dissection (VAD) is not uncommon in the young adult population. Owing to the various clinical manifestations; the diagnosis of VAD mainly depends on imaging examinations. Ultrasound has found an increasingly wide utilization in the diagnosis of VAD because of the continuous improvement in the resolution of ultrasonic instruments and accessibility. We retrospectively collected the data of patients with a US-proven extracranial vertebral artery dissection. In accordance with the sonographic findings, all patients were classified as having intramural hematoma, double-lumen dissection, and occlusion dissectio...
