The autoimmune IgG subclass response defines the IgG deposition pattern in pemphigus patient skin
Pemphigus is an autoimmune blistering disease that is caused by autoantibodies to the extracellular domains of desmoglein (Dsg) 1 and/or 3 (Egami et al., 2020). Desmogleins are cadherin molecules that interconnect two half-desmosomes, ensuring stable binding between neighboring keratinocytes. When bound to desmoglein, these autoantibodies deposit in the stratified epithelial layers of skin and mucosa. Indirect immunofluorescence of healthy skin or mucosa sections shows a smooth and even desmoglein staining around cells, while direct immunofluorescence of the patient lesional skin shows granular IgG deposits to a greater or...

Giant Ulcer with Necrosis on Right Vulva, Groin, and Thigh-- A Case of Necrotizing Fasciitis Associated with Erythematous Pemphigus
We report a case of a 72-year female with erythematous pemphigus who developed erythema, swelling and ulceration on right vulva, groin, and thigh. The early clinical manifestations of the patient were nonspecific and easily misdiagnosed as cellulitis. However, upon the occurrence of ulceration and necrosis, deep fungal infection, pyoderma gangrenosum or lymphoproliferative disorders were considered. The pathology suggested IgG4-related diseases, plasmacytoma et al. But at last, surgical exploration and postoperative pathology confirmed the diagnosis of necrotizing fasciitis. The patient recovered after multiple aggressive ...

Inflammatory conditions of the oral mucosa
Inflammatory mucocutaneous conditions that affect the oral cavity are wide ranging with overlapping clinical and histological features. Clinically these can present with a range of appearances including lace to plaque like patches to erosive and ulcerative conditions. Many of these conditions are associated with systemic affects including gastrointestinal, ocular and dermatological. Early recognition is essential to ensure optimal patient care and appropriate referral to other specialists. This review will concentrate on lichenoid mucositis including lichen planus and erythema multiforme, the vesiculo-bullous conditions pe...

Comparison of gut microbiota dysbiosis between pemphigus vulgaris and bullous pemphigoid
CONCLUSIONS: Our study suggests that the composition of gut microbiota is specific in different bullous diseases and the role of gut microbiota differs. Gut microbiota could help distinguish BP and PV, and might play a role in the pathogenesis of different bullous diseases.PMID:38185033 | DOI:10.1016/j.intimp.2023.111470 (Source: International Immunopharmacology)

Eponyms and Clinical Entities from the Land of Israel
Seven eponyms havd been pioneered by dermatologists in Israel: Brenner's sign, Chanarin-Dorfman syndrome, granulated sweetener packet sign, isopathic the phenomenon of Sagher, lanolin paradox, Nakar-Ingber disease, and Wolf's isotopic response. In addition, three id reactions described by Israeli dermatologists are listed: leishmanid, pediculid and scabid. There is also the acronym PEMPHIGUS, which stands for the causative reasons for pemphigus. We celebrate these eponyms and clinical entities, which reflect the impressive progress made by dermatologists in Israel during the past century, which has helped to build an acade...

C3d Immunohistochemical Staining on Paraffin-Embedded Tissue for Diagnosis of Pemphigus
CONCLUSIONS.—: Our study indicated that C3d-IHC results for paraffin-fixed tissues were not significantly different from DIF results for the diagnosis of pemphigus. The C3d-IHC assay has the potential for routine diagnosis of pemphigus, especially in the absence of fresh-frozen tissue.PMID:38149406 | DOI:10.5858/arpa.2023-0274-OA (Source: Archives of Pathology and Laboratory Medicine)

Potential Indications of Dupilumab in Th-2 Inflammatory Disease
Rev Recent Clin Trials. 2023 Dec 20. doi: 10.2174/0115748871263396231121060901. Online ahead of print.ABSTRACTDupilumab is a fully humanized IgG4 monoclonal antibody, inhibiting IL-4 and IL-13 signaling, which are the main cytokines involved in type 2 inflammatory diseases. Its introduction was a breakthrough in the treatment of moderate-to-severe atopic dermatitis, but it is also used in other inflammatory diseases, including asthma, eosinophilic esophagitis and chronic rhinosinusitis with nasal polyposis. Recent advances in the understanding of inflammatory pathways have revealed that Th2-type inflammation is involved in...
