Endoscopic submucosal dissection for oesophageal cavernous haemangioma accompanied by squamous epithelial dysplasia
A 70-year-old man with dysphagia presented to our hospital. Gastroscopy revealed a submucosal eminence with a bluish surface in the anterior wall of the oesophagus 20 –23 cm from the incisor, which was soft to the touch (Fig. 1A). Endoscopic ultrasound (EUS) showed a 10.5 × 18.5 mm hypoechoic and homogeneous mass in the submucosal layer (Fig. 1B). The patient was initially diagnosed with a submucosal bulge of the oesophagus, and atypical hemangioma was t he first consideration. Barretts oesophagus with indefinite for dysplasia shows high rates of prevalent and incident neoplasia in a UK multicentre cohort

Sems Sensibility: Biliary Stent Migration into the Oesophagus
We report a case of a patient affected by cancer of the pancreas who was treated with a proximal metallic biliary stent. The stent migrated to the oesophagus and was endoscopically removed. Oesophageal injury
is a rare but serious event, which is potentially fatal. Morbidity remains high even with prompt treatment. Injuries can be described as either partial or full-thickness wall disruption. They may not present typically and a high index of suspicion is required for successful diagnosis, taking into account history and examination findings. All patients should be discussed with a centre which treats high volumes of oesophageal cases, and initial management should focus on general resuscitation principles with an early decision for definitive care. Ethical and legal implications of implementing risk algorithms for early detection and screening for oesophageal cancer, now and in the future
ConclusionsNovel pathways incorporating risk prediction algorithms hold considerable promise, especially when combined with low-cost sampling. However immediate priorities should be to develop risk communication strategies that take account of using validated risk algorithms, and to ensure equitable implementation.
