Very high-energy electron therapy as light-particle alternative to transmission proton FLASH therapy - An evaluation of dosimetric performances
CONCLUSIONS: VHEE-based FLASH-RT using sufficiently high beam energies may provide a lighter-particle alternative to TP-based FLASH-RT with comparable dosimetric plan quality.PMID:38378075 | DOI:10.1016/j.radonc.2024.110177 (Source: Radiotherapy and Oncology : journal of the European Society for Therapeutic Radiology and Oncology)

Geographical and temporal differences in gastric and oesophageal cancer registration by subsite and morphology in Europe
ConclusionA wide variability in oesophago-gastric cancers topographic subsites and histopathological types patterns was observed, with a corresponding improvement in accuracy of registration in the analysis period. PBCRs are ideally placed to guide the epidemiological evaluations of such a complex group of diseases, in collaboration with clinicians, patients and other public health stakeholders. Unusual cause of recurrent aspiration pneumonia
Clinical introduction An 88-year-old man with intermittent fever, cough, vomiting and progressive dysphagia was transferred to our ED. His medical history was moderate hypertension and hyperlipidaemia. His family disclosed he had frequent repeated aspiration pneumonia within 3 months. A chest radiograph showed an obvious consolidation of the right lower lobe. Lateral radiograph of cervical spine was shown in figure 1. Question What is the most likely cause of recurrent aspiration pneumonia? Diffuse idiopathic skeletal hyperostosis Acute calcific retropharyngeal tendinitis Crowned dens syndrome Posterior longitudinal ligame...

Wake up to gastro-oesophageal reflux disease: The interplay between arousal and night-time reflux
J Sleep Res. 2024 Feb 14:e14158. doi: 10.1111/jsr.14158. Online ahead of print.ABSTRACTThis review examines the temporal association between nocturnal gastro-oesophageal reflux and sleep-arousal cycles. Most nocturnal gastro-oesophageal reflux events occur during the awake cycle, and arousals precede most nocturnal gastro-oesophageal reflux events, indicating that arousal from sleep predisposes to nocturnal gastro-oesophageal reflux. This sheds light on the complex relationship between nocturnal gastro-oesophageal reflux and sleep, and has implications for managing nocturnal gastro-oesophageal reflux symptoms. The appearan...

Gastro-oesophageal reflux disease and hiatus hernia
Gastro-oesophageal reflux disease (GORD) is a common disorder with an increasing prevalence in the UK. The presence of a hiatus hernia is the strongest risk factor for developing GORD, although it is not essential. The most common symptoms experienced by patients with GORD are heartburn and acid regurgitation. The diagnosis of GORD is usually made from the clinical history. Endoscopy and ambulatory pH monitoring are adjunctive tests that can influence management decisions. The main goals of treatment are symptom control and the prevention of complications such as Barrett's oesophagus and peptic stricture. (Source: Medicine)

Eosinophilic oesophagitis: practical management and novel therapeutics
Eosinophilic oesophagitis (EoE) is a chronic type 2 allergic inflammatory disease of the oesophagus, with a rising prevalence in adults and children. Intermittent dysphagia and/or food impaction occurs in the presence of eosinophilic infiltration of the oesophagus. Patients tend to have a history of atopy. The underlying cause of EoE is an antigen-driven effect occurring only in the oesophagus, with food the most likely source of antigens. All adults with dysphagia or food bolus obstruction require endoscopy and at least six biopsies from the lower, mid- and upper oesophagus if they have signs associated with EoE or have a...

Evidence for Minimal Invasive Oesophageal Resection
Zentralbl Chir. 2024 Feb 5. doi: 10.1055/a-2241-0439. Online ahead of print.ABSTRACTIn the course of the last 20 years, minimally invasive therapy has become much more important in all areas. In particular, surgical procedures have been established in oncological surgery, even without generating the necessary evidence to assure that the quality is equal to that achieved with open procedures. For this purpose, it has only been in recent years that appropriate randomised controlled studies followed by meta-analyses have been carried out. In this article, we summarise the evidence for minimally invasive resection of the oesop...
