Serious Mental Illness in Assisted Living Communities: Association With Nursing Home Placement
Assess prevalence of serious mental illness (SMI) alone, and co-occurring with Alzheimer disease and related dementias (ADRD), among Medicare beneficiaries in assisted living (AL). Examine the association between permanent nursing home (NH) placement and SMI, among residents with and without ADRD. (Source: Journal of the American Medical Directors Association)

The Potential Need for Multiple IPR Stays for Cancer Patients due to Interruptions from Cancer Treatment: Two Case Reports
The objective of this paper is to present two patients with severe impairment that required cancer related treatment between the 2 stays of IPR. We aim to raise awareness of these patients who required two courses of IPR due to interruptions from chemotherapy or other specialized treatment for their cancer or cancer related condition. Increased Renal Complications After the 2018 UNOS Policy Change: A Linked Analysis of the Scientific Registry of Transplant Recipients and Medicare Claims
Purpose: In 2018, UNOS amended the heart allocation policy to prioritize patients bridged with temporary mechanical circulatory support (tMCS) which led to greater use of IABP, ECMO, and percutaneous LVAD as a strategy to bridge to heart transplantation (HT). Subsequent studies of the Scientific Registry of Transplant Recipients (SRTR) demonstrated a reduced waitlist mortality with no effect on post-HT survival following the policy change. However, the impact of the policy change on morbidity is unknown. (Source: The Journal of Heart and Lung Transplantation)

Characterization of long-term survival in Medicare patients undergoing arteriovenous hemodialysis access
Background Patients undergoing arteriovenous (AV) access creation for hemodialysis often have significant comorbidities. Our goal was to quantify the long-term survival and associated risks factors for long-term mortality in these patients to aid in optimization of goals and expectations. Clinical and Adverse Outcomes Associated with Concomitant Use of CYP2D6-Metabolized Opioids with Antidepressants in Older Nursing Home Residents
This study aimed to investigate the associations of concomitant use of CYP2D6-metabolized opioids and antidepressants with clinical outcomes and opioid-related adverse events (ORAEs). Using a 100% Medicare NH sample, we conducted a retrospective cohort study among 80,319 NH older ( ≥65 years) long-term (> 100 days) residents who initiated CYP2D6-metabolized opioids and antidepressants concurrently or initiated one drug class while already receiving the other. Comparing survival of older ovarian cancer patients treated with neoadjuvant chemotherapy versus primary cytoreductive surgery: Reducing bias through machine learning
CONCLUSIONS: The MCI outperformed the conventional CCI in predicting 1-year mortality, and reducing confounding due to differences in baseline health status in comparative effectiveness analysis of NACT versus primary surgery.PMID:38554626 | DOI:10.1016/j.ygyno.2024.03.016 (Source: Gynecologic Oncology)

Patient Resiliency and Caregiver Burden After Traumatic Spinal Cord Injury Resulting in Quadriplegia
Am Surg. 2024 Mar 29:31348241241737. doi: 10.1177/00031348241241737. Online ahead of print.ABSTRACTTraumatic spinal cord injury (tSCI) resulting in quadriplegia is a life-altering injury for patients and caregivers. We conducted a retrospective review of patients treated for tSCI and quadriplegia at a level 1 trauma center to assess quality of life (QOL), socioeconomic factors, and mortality. Patients and caregivers were surveyed. Of the 65 patients included, 33 contacts were made. Seventeen surveys were completed (12 caregivers and 5 patients). Six unreachable patients were confirmed alive via medical record. Mortality ra...

Comparative Effectiveness and Cost-Effectiveness of Colorectal Cancer Screening With Blood-Based Biomarkers (Liquid Biopsy) vs Fecal Tests or Colonoscopy
CONCLUSIONS: CMSmin could contribute to CRC control by achieving screening in those who will not use established methods. Substituting blood-based testing for established effective CRC screening methods will require higher CRC and APL sensitivities that deliver programmatic benefits matching those of FIT. High APL sensitivity, which can result in CRC prevention, should be a top priority for screening test developers. APL detection should not be penalized by a definition of test specificity that focuses on CRC only.PMID:38552670 | DOI:10.1053/j.gastro.2024.03.011 (Source: Cancer Control)
