Best ways to get referrals from y'all?
I'm a psychiatrist and potentially starting a private practice from 0. Would take major insurances, some Medicaid, no Medicare. Open to seeing pretty much anything other than ASPD/long criminal histories, but would like to keep substance use disorders to a minimum. Happy to see eating disorders, functional problems, ADHD, autism evals, in addition to the basic depression and anxiety. Do I just send letters to your offices? Should I stop by and talk to your receptionist and give out... Read more (Source: Student Doctor Network)

maximum time allowed between 2nd MBB and 1st RFA?
After two successful Dx blocks, is there a maximum time allowed within which RFA has to be done? I have a patient in this situation, and we are holding off on RFA for a number of reasons, but wanting to know how long we can delay the RFA, without having to potential start over with MBB again, or do the RFA only to have it denied for payment. He has BCBS Commercial insurance. I do not see anything about this in BCBS medical policy or Medicare LCD. (Source: Student Doctor Network)

“Please call me when they go upstairs”
One of the questions I’m still grappling with how to generate a satisfactory answer to. “Please call me when pop-pop leaves with the transfer ambulance for Our Lady of Medicare regional hospital” Or 1000 different permutations of the question above. Let me know when he gets out of the OR. Let me know when the HD cath is in. Let me know when they get to their new room. I have been trying without avail to find an adequate way to politely tell reasonable-ish people that I have no idea when... Read more (Source: Student Doctor Network)

IM Resident planning to apply to Dermatology after Residency
I'm considering Dermatology now that I'm in Internal Medicine. I have below-average Step 2 CK score (235 to 240) when I got into IM. If I apply for Dermatology Residency upon completion of my IM Residency (and let's say Medicare Funding is not an issue for the programs I will be applying to), how much would my Step 2 CK play a factor considering that I will have Step 3 and IM Board Scores during application? Site Neutral Payments Axed
"Congress will not move forward with a controversial policy to equalize certain Medicare payments to hospitals and physicians’ offices in an upcoming government funding package, five lobbyists and sources following the talks told STAT. The delay is a win for hospitals, which have adamantly lobbied against the policy." (Source: Student Doctor Network)

Any private equity lobby on reimbursement?
Just wondering. Since PE really likes anesthesiology practices, and we have obvious problems in anesthesiology, such as low medicaid/medicare reimbursement rates compared to other specialties, have they or big anesthesia companies ever try to lobby on increased reimbursements for medicaid/medicare? We like to say its the ASA's job on these forums, but the truth is ASA is tiny. Their funding is also tiny. Feels to me PE can benefit a lot if anesthesiology medicaid/medicare reimbursements are... News from AAOS
Orthopedic Surgeons on Track to Earn Less Than Taco Bell Workers If current trends continue, patients are in big trouble www.medpagetoday.com "If those numbers aren't disappointing enough for young surgeons who've already spent a dozen years in training, the same group fit a third-order polynomial model to historical data for Medicare reimbursements for arthroplasties, yielding a projection out to 2030. Actual figures from 2007 to 2023 showed mean reimbursements falling over time, and falling faster with each passing year from 2019 onward. T...

SCS multiple procedure reduction
Hello: I’m in a hospital employed wRVU-based compensation model. I’ve been reviewing my coding and the system is applying a 50% reduction in wRVU to 63685 when doing an SCS implant. My understanding was that 63650x2 and 63685 are submitted without a -51 modifier. Do any know if Medicare/payers are subjecting the 63685 payment to a multiple procedures reduction? Thanks (Source: Student Doctor Network)

Ethical Dilemma in Billing
I have an ethical dilemma that I need help with. I am an Endocrinologist, who lives in a neighboring state to my sister and mother. I am DPoA for medical affairs for my 90 year old mother with moderate Alzheimer’s Disease. My sister, who my mother lives with, is DPoA for finances and pays the bills. My mother has both Medicare part A and B and Federal Blue Cross Insurance. She fell, fracturing her pelvis through the right acetabulum (and ilium), which was repaired surgically. She is... It looks like Medicare requires a min of 2 weeks between MBBs trials. Is this the same for the PPO payors for your locality too? (Source: Student Doctor Network)

ASC vs. HOPD - CMS rules
Question: With the Medicare changes regarding prior auth requirements and frequency for basic procedures such as MBB, RFA, ESI, etc., quick question... Is there any difference in performing these procedures in an ASC vs. HOPD facility? The reason I ask is that I currently practice in HOPD facility, but have and opportunity to move my procedures to an nearby ASC. If that would help to avoid some of the CMS LCD requirement nonsense, it would be worth it! Thoughts? Thanks, (Source: Student Doctor Network)
