Genetic testing in cardiovascular disease
Med J Aust. 2024 Apr 4. doi: 10.5694/mja2.52278. Online ahead of print.ABSTRACTCardiovascular disease (CVD) is the leading cause of morbidity and mortality globally and is responsible for an estimated one-third of deaths as well as significant morbidity and health care utilisation. Technological and bioinformatic advances have facilitated the discovery of pathogenic germline variants for some specific CVDs, including familial hypercholesterolaemia, cardiomyopathies and arrhythmic syndromes. Use of these genetic tests for earlier disease identification is increasing due, in part, to decreasing costs, Medicare rebates, and c...

Roles of Hospital Type and Community Setting in Rate of Screening for Metabolic Disorders Among Psychiatric Patients
CONCLUSIONS: Despite common resource limitations, screening for metabolic disorders may be driven more by logistics and less by time, finances, or a community's primary care network. Identifying the specific logistical challenges of freestanding psychiatric facilities could aid in the development of targeted interventions to improve the rates of screening for and treatment of not only metabolic disorders but also other common comorbid conditions.PMID:38566560 | DOI:10.1176/appi.ps.20230472 (Source: Psychiatric Services)

Lessons from the 'legitimate' misuse of Medicare Benefits Schedule Item 45503
Aust Health Rev. 2024 Apr 4. doi: 10.1071/AH24073. Online ahead of print.ABSTRACTThis Perspective begins with a case study that raises two important questions: who is responsible for the existence of non-compliant Medicare billing, and who is responsible for eliminating it? In the discussion that follows, I argue, first, that the problem has been created by individual clinicians and by Medicare itself (i.e. the organisational structure that administers Medicare). Second, and more importantly, I argue that the ethical obligation to eliminate the problem extends more broadly to include both the government and the medical pro...

Atrial Fibrillation Management During Surgical vs Transcatheter Aortic Valve Replacement
CONCLUSIONS: In Medicare beneficiaries with AF requiring aortic valve replacement, SAVR+AF was associated with improved longitudinal survival and freedom from stroke compared to TAVR. SAVR with AF treatment should be considered first-line therapy for patients with AF requiring aortic valve replacement.PMID:38570109 | DOI:10.1016/j.athoracsur.2024.03.020 (Source: The Annals of Thoracic Surgery)

Volume and distribution of radiotherapy performed by dermatologists from 2016 to 2021: A national Medicare trend analysis
To the Editor: Growing availability of superficial radiotherapy (SRT) devices and introduction of electronic brachytherapy (eBT) have prompted utilization of these modalities by dermatologists for nonmelanoma skin cancer. Each modality involves application of photon radiation to the skin in the outpatient setting over 5 to 12 (SRT) or 8 to 10 (eBT) fractionated sessions.1 We aimed to characterize eBT/SRT utilization and cost trends by dermatologists since 2016 given recent transition of several nonsuperficial radiotherapy services to the Outpatient Prospective Payment System, restrictions in billable services alongside eBT...
