Mirtazapine and Vitamin B6 May Be Best Options for Antipsychotic-Induced Akathisia
APA ’s Good Faith Estimates Survey: Please Respond TodayThe No Surprises Actrequires clinicians to provide patients who are uninsured or are insured but choose not to submit claims through their health plan with a Good Faith Estimate of the cost of care. The Centers for Medicare and Medicaid Services has asked APA for your feedback on efforts to comply with these requirements and any educational support you may need from CMS. Teratogenicity of zonisamide and other little-used antiseizure medications
Over more than two decades, a very considerable majority of the pregnancies of Australian women with epilepsy that have been enrolled in the Raoul Wallenberg Australian Pregnancy Register of Antiepileptic Drugs (APR) have been managed with a relatively small number of the antiseizure medications (ASMs) that are available in the country. The most frequently used ASMs have been carbamazepine (CBZ), lamotrigine (LTG), levetiracetam (LEV) and valproate (VPA) ([8]b). Topiramate (TPM) and clonazepam (CZP) have had less extensive use ([8]b). (Source: Seizure: European Journal of Epilepsy)

Polysomnographic features associated with clonazepam and melatonin treatment in isolated REM sleep behavior disorder: Time for new therapeutic approaches?
CONCLUSION: These results suggest that the administration of MLT or CLO impacts (positively) on sleep parameters of iRBD patients. However, there is a need to better stratify patients, in order to treat them in a targeted manner, depending on the patient's individual sleep architecture and expected differential effects of these agents.PMID:38421131 | DOI:10.1111/cns.14569 (Source: CNS Neuroscience and Therapeutics)

Seizure control and complications in patients switching from clobazam to clonazepam due to drug shortage
CONCLUSION: The abrupt switch from CLB to CLZ was observed to increase seizure frequency without leading to status epilepticus in PWE. CLZ-associated side effects were found to be tolerable despite the abrupt switch. Future studies may explore the effect of alternative switching ratios.PMID:38382407 | DOI:10.1016/j.yebeh.2024.109690 (Source: Epilepsy and Behaviour)

A case of Stiff-person syndrome with muscle tonicity of the extremities and neck after use of Dulvalumab for lung adenocarcinoma
Rinsho Shinkeigaku. 2024 Feb 17. doi: 10.5692/clinicalneurol.cn-001923. Online ahead of print.ABSTRACTA 74-year-old woman taking dulvalumab for lung adenocarcinoma developed muscle tonicity in the extremities and trunk. Painful paroxysmal muscle spasms with profuse sweating were frequently observed, and surface electromyography showed simultaneous contraction of the active and antagonist muscles. Blood tests were strongly positive for anti-amphiphysin antibodies, and stiff-person syndrome (SPS) was diagnosed. Intravenous immunoglobulin therapy and clonazepam were initiated, and the paroxysmal painful muscle spasms disappea...

Differential inflammation responses determine the variable phenotypes of epilepsy induced by GABRG2 mutations
CONCLUSION: We successfully established transgenic zebrafish epileptic models expressing human mutant GABRG2(P282S), in which CBZ and VPA showed antiepileptic effects. Differential inflammatory responses, especially the SOCS/JAK/STAT signaling pathway, might be related to the phenotypes of genetic epilepsy induced by GABRG2 mutations. Further study will expand the pathological mechanisms of genetic epilepsies and provide a theoretical basis for searching for effective drug treatment.PMID:38357846 | PMC:PMC10867793 | DOI:10.1111/cns.14583 (Source: CNS Neuroscience and Therapeutics)

Benzodiazepines for the Treatment of Seizure Clusters
AbstractPatients with epilepsy may experience seizure clusters, which are described by the US Food and Drug Administration (FDA) as intermittent, stereotypic episodes of frequent seizure activity that are distinct from a patient ’s usual seizure pattern. Untreated seizure clusters may increase the risk for status epilepticus, as well as decrease quality of life and increase burden on patients and care partners. Benzodiazepine therapies are the mainstay for acute treatment of seizure clusters and are often administered by nonmedical care partners outside a healthcare facility. Three rescue therapies are currently FDA-appr...

Physician Approaches to the Pharmacologic Treatment of Dystonia in Cerebral Palsy
CONCLUSIONS: This study summarizes current indications and dosing for the six most commonly used medications to treat dystonia in CP as per treating physicians in the US and Canada and also demonstrates physician support for a randomized trial comparing the effectiveness of these treatments.ARTICLE SUMMARY: Dystonia is common and debilitating in people with CP, with little data on pharmacologic treatments. We describe physicians' current approaches to using these treatments.WHAT’S KNOWN ON THIS SUBJECT: Comparing the effectiveness of existing pharmacologic treatments for dystonia in CP is a research priority shared by cl...

US Generic Antiseizure Medication Supply Chain: Observations from Analysis of US Government Databases
In 2015, the Center for Disease Control (CDC) estimated that in the U.S., 1.2% of the population have active epilepsy (including 3 million adults and approximately 500,000 children).[1] Over 25 antiseizure medications (ASM) are currently approved by the United States Food and Drug Administration (FDA).[2] During a critical shortage of an ASM, the feasibility of a rapid transition to an alternate ASM is often difficult.[3] The FDA drug shortage database in July 2023 indicated a national shortage of ASMs often used in status epilepticus, including rectal diazepam, oral clonazepam, and intravenous preparations of lorazepam, m...

Sleep Terrors
Sleep terrors, categorized under disorders of arousal, more prevalent in pediatric population, generally are self-limited but sometimes can persist or occur in adulthood. These are primed by factors enhancing homeostatic drive on backdrop of developmental predisposition and are precipitated by factors increasing sleep fragmentation resulting in dissociated state of sleep with some cerebral regions showing abnormal slow wave activity and others fast activity. This phenotypically evolves into abrupt partial arousal with individual arousing from N3 or N2 sleep with behaviors representing intense fear such as crying with auton...

Combining surface ‐enhanced Raman spectroscopy and paper spray mass spectrometry for the identification and confirmation of psychotropic substances in alcoholic beverages
AbstractCriminal practices in which an individual becomes vulnerable and prone to sexual assault after ingesting drinks spiked with doping substances have become a social concern globally. As forensic protocols require a multi-tiered strategy for chemical evidentiary analysis, the backlog of evidence has become a significant problem in the community. Herein, a fast, sensible, and complementary dual analytical methodology was developed using a single commercial paper substrate for surface-enhanced Raman spectroscopy (SERS) and paper spray mass spectrometry (PS-MS) analysis to identify psychotropic substances added to alcoho...
