Anaplasmosis-Induced Atrial Flutter Resolving With Antibiotic Therapy
We describe a case of a 56-year-old man with recent tick exposure who presented in atrial flutter with variable atrioventricular block after 3 days of fever, chills, and arthralgias. The patient was successfully treated and stably returned to sinus rhythm after initiation of antibiotics without cardioversion and was found to be positive for A. phagocytophilum but negative for Lyme disease. To our knowledge, this represents the first reported case of anaplasmosis-associated atrial flutter. As infections increase in incidence, clinicians in endemic areas should be aware of the potential manifestations and treatment considera...

Immunoglobulin A (De Novo) Vasculitis After mRNA Moderna COVID-19 Vaccination: A Case Report
Concurrent with the administration of vaccines for the novel coronavirus (COVID-19), there have been many reported adverse effects, and many of them anticipated as with any vaccine administration. This case report, however, focuses on a patient who, shortly after receiving the first dose of the Moderna COVID-19 vaccine, developed a pruritic maculopapular rash with typical distribution and clinical characteristics, along with high levels of immunoglobulin A (IgA), consistent with IgA vasculitis, formerly known as Henoch-Schonlein purpura. Although there are reported cases of IgA vasculitis after different vaccine administra...

High Morbidity of Mycobacterial Infections Following Cosmetic Surgery Tourism
Conclusion Patients who undergo cosmetic surgery as medical tourists are placed at risk of infection by rapidly growing mycobacterial species. Management is highly morbid, and they are often left with extensive surgical scarring and unanticipated additional out-of-pocket expense. (Source: Infectious Diseases in Clinical Practice)

Detectability of Hepatitis B Virus in Peripheral Blood Mononuclear Cells Among Naive Chronic Hepatitis B Patients With Negative Viremia: A Multicenter Study
Conclusions A considerable number of patients with negative plasma HBV-DNA are still harboring subtle form of virus within remote extrahepatic compartments. Thus, dual testing for both plasma and PBMCs is mandatory especially in epidemiologic studies. (Source: Infectious Diseases in Clinical Practice)

The Outcome of High-Dose Corticosteroid Treatment Among Coronavirus Disease 2019 Patients: A Retrospective Cohort Study
This study aimed to demonstrate the association between high-dose corticosteroid administration and adverse outcomes in coronavirus disease 2019 patients. Data were collected retrospectively from medical records. The primary outcome was invasive mechanical ventilation or death, whichever occurred first. The secondary outcome was all-cause in-hospital mortality. The standard dose was defined as a daily dose of ≤1.5 mg/kg of prednisolone or equivalent, and the high-dose was defined as ≥250 mg of prednisolone or equivalent. Data were analyzed using frequentist and Bayesian logistic models. In addition, a propensity scoreâ...

Aspergillus Tracheobronchitis With Airway Obstruction: A Case Series With Positive Patient Outcomes
We present 3 cases of ATB with obstruction of airways in immunocompromised patients with a history of malignancy and describe their clinical course. Two of the cases we present had a mixed pattern with pseudomembranous/obstructive type and one had ulcerative type of ATB. All showed nonspecific radiographic evidence of pathology and were ultimately diagnosed by a bronchoscopy. After bronchoscopy, voriconazole was prescribed to all the patients and 1 patient showed paradoxical worsening of a nodule with new cavitation. (Source: Infectious Diseases in Clinical Practice)

Decreasing the Rate of Antibiotics Administration to Newborns of Mothers With Prolonged Rupture of Membranes and Unknown Group B Streptococcus Status Using the Plan-Do-Study-Act Quality Improvement Model
Background Prolonged rupture of membranes (PROM) and maternal group B Streptococcus (GBS) colonization are major risk factors for early-onset sepsis. Managing asymptomatic newborns remains burdensome, as exposing them to unnecessary antibiotics or withholding them when needed is potentially harmful. Decreasing the rate of antibiotics administration to newborns of mothers with PROM and unknown GBS status is important. Methods A quality improvement project applying the Plan-Do-Study-Act model was conducted to test the efficacy of a proposed protocol to lower the rate of antibiotics administration. This protocol us...

Dantrolene for Generalized Tetanus in Pediatrics: A Case Report and Review of Treatment Options
We describe an unimmunized child with generalized tetanus who required nearly a month of mechanical ventilation with analgesia, sedation, and neuromuscular blockade. Dantrolene showed benefit in spasm control and patient comfort when used in combination with other drugs. (Source: Infectious Diseases in Clinical Practice)

TandemHeart-Associated Fever: Does the Fever Signify Infection?
Percutaneous mechanical circulatory support devices such as TandemHearts have been increasingly used in the treatment of patients with refractory cardiogenic shock. Fever can develop as a consequence without the development of an infection. Infectious disease physicians are commonly consulted for the onset of fevers after device placement, and it is been a challenge to identify the cause of fevers and delineate infection as cause of fever. Fever and associated infections have not been well characterized after TandemHeart placement. The aim of the current study is to correlate the development of fever with infections and de...

Longer Steroid Treatment Increases Secondary Bloodstream Infection Risk Among Patients With COVID-19 Requiring Intensive Care
Conclusions We found a positive association between the duration of corticosteroids and the development of BSI. Considering immunosuppression is now the cornerstone of guidelines for COVID-19 treatment, further studies are needed to evaluate risks and mitigation strategies for these therapies.
