A Case of Nonclostridial Gas-Producing Soft Tissue Infection in a Neutropenic Leukemia Patient With Literature Review
Gas-producing soft tissue infections have been historically attributed to clostridial species; however, organisms in both the Enterobacteriaceae family as well as certain Gram-positive organisms have also been identified as causative organisms. Soft tissue infections are known to progress with an aggressive course, one that often necessitates urgent surgical debridement, amputation, and a prolonged course of antibiotics. Chronic HIV infection is increasingly common in clinical practice, and it is necessary for clinicians to have an understanding of specific infections and know how to treat them in this patient cohort. Combined Myopericarditis and Pneumonitis Post–Pfizer Vaccine Against Coronavirus Disease 2019 in a Healthy Caucasian Middle-Aged Man
A generally healthy middle-aged Caucasian man developed significant cough, fever, chest tightness, dyspnea, and orthopnea after second dose of Pfizer-BioNTech coronavirus disease 2019 (COVID-19) vaccine. He was hypoxemic and had bilateral patchy ground glass opacities and mediastinal adenopathy. Those objective signs indicated an acute pneumonitis. His acute heart failure, new onset dilated cardiomyopathy with an left ventricular ejection fraction of 25% are consistent with the diagnosis of suspected myopericarditis despite normal troponin. A Rare Case of Cytomegalovirus and Herpes Simplex Virus Coinfection Gastritis and Colitis in a Person Living With HIV/AIDS
We report the case of a 46-year-old man with a medical history of poorly controlled HIV/AIDS who presented with 2 months of progressively worsening nonbloody diarrhea. Esophagogastroduodenoscopy showed gastritis and sigmoidoscopy revealed severe colitis. Biopsy immunostaining revealed cytomegalovirus and HSV viral inclusions in the stomach, appendix, and rectum. The patient's symptoms largely improved with ganciclovir. To our knowledge, this is the first case demonstrating cytomegalovirus and HSV coinfection gastritis and colitis in a patient with HIV/AIDS.
