Probable Drug Eruption Eosinophilia and Systemic Symptoms Due to Colistimethate Sodium: A Challenging Case Report
A 28-year-old male patient was admitted to our hospital with an altered mental status. During his hospitalization, he experienced 2 events of ventilator-associated pneumonia. Acinetobacter baumannii and Pseudomonas aeruginosa were separated from sputum culture in the first and second episodes, respectively. Two events of drug reactions with eosinophilia and systemic symptoms also occurred during hospitalization. Generalized morbilliform rash, high fever, eosinophilia, and increases in liver aminotransferase and bilirubin were observed in the first episode. Initially, medications with a higher likelihood of skin eruption in...

A Novel Case of Life-Threatening Thrombocytopenia Secondary to Anaplasmosis
This case report discusses an atypical presentation of anaplasmosis characterized by severe, potentially life-threatening thrombocytopenia. From the case and the ensuing discussion, medical professionals should appreciate the growing incidence of anaplasmosis across the country, as well as its potential to provoke profound hematologic disturbances in humans. Primary Herpes Simplex Virus Type 2 Infection Associated With Monoarthritis of the Knee
Monoarthritis associated with herpes virus infection is rare and previously only described in herpes simplex type 1, varicella zoster, and Epstein-Barr virus infections. Here, we report a case of monoarthritis of the knee associated with primary herpes simplex virus type 2 (HSV-2) infection in an immunocompetent 50-year-old woman. Herpes simplex virus type 2 DNA was detected in plasma and in the joint fluid. The patient had only mild symptoms of herpes simplex virus type 2, suggesting that virus-associated monoarthritis may be underrecognized and underreported. A Case of Rapidly Progressive Neurological Decline
We describe a very intriguing case of progressive neurological decline where the patient initially presented to the primary care physician and was ultimately diagnosed with Creutzfeldt-Jakob disease in a community hospital setting. We have highlighted the difficulties in diagnosing this uniformly fatal condition and its potential transmission from animals. (Source: Infectious Diseases in Clinical Practice)

Influenza A Encephalitis in an Adult Patient
A 45-year-old woman presented with 7 days of worsening dyspnea, nonproductive cough, myalgias, and chills. The following day, the patient had acute respiratory failure, requiring mechanical ventilation, and eventually developed acute respiratory distress syndrome. Respiratory panel tested positive for influenza A. The patient's mental status progressively worsened to coma. Magnetic resonance imaging of the brain demonstrated relatively symmetrical bilateral T2 FLAIR abnormalities in the thalami, corpus callosum, pons, and cerebellum. Cerebrospinal fluid polymerase chain reaction was positive for influenza A. The patient wa...

Meropenem-Induced Resistant Hypokalemia
Meropenem is a β-lactam antibiotic from the carbapenem class that has been one of the most effective drugs in treating some of the most serious infections in hospitalized patients. Because of its unique β-lactam structure, it has a wide spectrum of activity against some of the most resistant bugs especially penicillinase-producing gram-positive bacteria, gram-negative bacteria, and anaerobes. Common adverse effects that have been well documented with meropenem include nausea, vomiting, diarrhea, and in rare cases skin rashes from hypersensitivity reactions. Hypokalemia is not a known adverse effect of meropenem use. In t...

Organizing Pneumonia as a Manifestation of AIDS in a Patient With Partially Treated Pneumocystis Jiroveci Pneumonia: Case Report and Literature Review
Organizing pneumonia (OP), formerly called bronchiolitis obliterans organizing pneumonia, is a rare disease characterized by necrotizing granulomas and fibrosis in the small airways of lungs. We arguably present the fourth reported case of a patient with human immunodeficiency virus who was diagnosed with OP in the setting of partially treated Pneumocystis jiroveci pneumonia (PJP). A Case of Vasculitis-Like Skin Eruption Associated With COVID-19
We described the case of a 72-year-old woman without a history of dermatologic diseases with COVID-19 who developed vasculitis-like skin eruptions. Because she has not received any treatment for COVID-19, we suggest that the dermatologic clinical picture was related to SARS-CoV-2. (Source: Infectious Diseases in Clinical Practice)

Multisystem Inflammatory Syndrome in an Adult After COVID-19
We describe the first case of multisystem inflammatory syndrome in an adult patient after coronavirus disease-19 (COVID-19). A 34-year-old Hispanic man, who tested positive for severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) 1 month before presentation, was admitted for fever, abdominal pain, headache, and myalgia. The patient deteriorated, developing multisystem organ failure. He required intensive care unit admission, mechanical ventilation, vasopressors, and continuous renal replacement therapy. Laboratory workup showed depletion of serum complement levels. After immunosuppressive therapy with intravenous i...
