Syphilis Presenting as Hepatitis in an HIV-Infected Patient
We report the case of a 24-year-old Filipino, working in Zambia for the past 2 months, presenting with a 2-week history of fever and jaundice. Maculopapular rashes were noted on both palms of the hands and both soles of the feet. Workup revealed an abnormal cholestatic pattern of liver function tests, while negative for malaria, viral hepatitis, and bacteremia. We present a case of Stevens-Johnson syndrome/toxic epidermal necrolysis in a 71-year-old male patient who was treated with daptomycin for infection of a prosthetic shoulder joint. After total shoulder arthroplasty, he had developed symptoms concerning for infection and had a PICC line placed for antibiotic therapy. Breakthrough Penile Mucormycosis in a Patient With Acute Myelogenous Leukemia on Posaconazole Prophylaxis
In patients with acute myelogenous leukemia, posaconazole prophylaxis has been shown to prevent deep invasive fungal infections, including mucormycosis. In the present case, posaconazole prophylaxis was initiated in a 58-year-old man undergoing induction chemotherapy for acute myelogenous leukemia. Posttraumatic Endophthalmitis Caused by Medicopsis romeroi: Case Report and Analysis of a Comprehensive Case Series
Fungal exogenous endophthalmitis is rare. Here, we report a case of posttraumatic endophthalmitis caused by Medicopsis romeroi in a 64-year old man. After a tree branch punctured his right eye while hiking in Laos, the patient's symptoms included pain, blurry vision, limited vision of light and dark, and sensation of a foreign body. The patient was unsuccessfully treated in Thailand with itraconazole and intraocular amphotericin. Do Mechanically Ventilated COVID-19 Patients Present a Higher Case-Fatality Rate Compared With Other Infectious Respiratory Pandemics? A Systematic Review and Meta-Analysis
Conclusions COVID-19 patients requiring IMV show a similar CFR compared with SARS and H1N1 influenza patients but a lower CFR compared with MERS patients. To improve survival in future pandemics, we recommend examining the pros and cons of the liberal use of endotracheal intubation and considering drafting guidelines for the selection of patients to intubate and the timing of intubation.
