Long-Acting Lipoglycopeptide Antibiotics (Dalbavancin and Oritavancin) for the Treatment of Endotipsitis in a Patient With Cirrhosis: A Case Study
Endovascular Staphylococcal infections involving prosthetic material typically require long courses of intravenous antibiotics, which are often completed at home or in a rehabilitation facility with outpatient parenteral antibiotic therapy. An alternative to outpatient parenteral antibiotic therapy is the use of long-acting lipoglycopeptides (LaLGPs). Dalbavancin and oritavancin are LaLGPs that have been used for the treatment of catheter-related blood-stream infections and endocarditis. However, their use in patients with liver dysfunction is not well documented in the literature. Herein, we describe a case of LaLGPs used...

Hemophagocytic Lymphohistiocytosis Secondary to Disseminated Histoplasmosis: A Report of 3 Cases and Review of the Literature
Hemophagocytic lymphohistiocytosis is a syndrome of immune dysregulation that can lead to an overwhelming inflammatory state. In this case series, we describe 3 cases in which disseminated Histoplasma capsulatum infection caused hemophagocytic lymphohistiocytosis. (Source: Infectious Diseases in Clinical Practice)

Allergic Bronchopulmonary Aspergillosis Progression to Chronic Pulmonary Aspergillosis
Allergic bronchopulmonary aspergillosis is a fungal infection of the lung causing a type 1 hypersensitivity reaction to Aspergillus fumigatus colonization. Patients with preexisting lung pathology are extremely susceptible to this organism and show clinical signs of bronchospasm, pulmonary infiltrates, and eosinophilia. In the following case report, we present an asthmatic patient diagnosed with allergic bronchopulmonary aspergillosis, who, despite treatment, progressed to chronic pulmonary aspergillosis with progressive lung damage requiring surgery. Positive, Mildly Positive, and Uncertain Nasopharyngeal Swab and Outcome in COVID-19 Patients
Conclusions Our study demonstrated that patients with a complete SARS-CoV-2 gene detection nasopharyngeal swab show a higher inflammatory profile, and this can be an indirect measurement of viral load in COVID-19 patients. (Source: Infectious Diseases in Clinical Practice)

Appropriate Antibiotic Prescribing in the Emergency Department: A Cross-sectional Study
Conclusions Antibiotics were prescribed in 15% of ED presentations, and 24% of antibiotic prescriptions were assessed as inappropriate, indicating an urgent need to improve awareness and adherence to antibiotic guidelines in the ED. (Source: Infectious Diseases in Clinical Practice)

Factors Affecting Clinical Outcomes of Carbapenem-Resistant Enterobacteriaceae and Carbapenem-Susceptible Enterobacteriaceae Infections
This study aimed to identify risk factors associated with the mortality and readmission of CRE- and carbapenem-susceptible Enterobacteriaceae (CSE)–infected patients in Thailand. Length of stay (LOS) and the cost of treatment for the infections were also investigated. Method A retrospective cohort study was conducted in patients with CRE- and CSE-positive cultures from January 2016 to June 2019 in an advanced-level government hospital in Thailand. Risk factors for mortality and readmission were analyzed by logistic regression analysis. The differences in LOS and cost of treatment between CRE- and CSE-infected pati...

Tick-Borne Disease–Associated Thrombocytopenia: Epidemiological Description Among United States Veterans and Tick Surveillance Data From Suffolk County, Long Island, New York
Conclusions Based on current epidemiologic trends, and local tick surveillance data, coinfections with tick-borne diseases need to be considered. (Source: Infectious Diseases in Clinical Practice)

Interferon Gamma Release Assay Mitogen Responses in COVID-19
Conclusions Indeterminate IGRA results were associated with higher levels of inflammatory markers. Furthermore, when comparing mitogen responses between COVID-19–positive and COVID-19–negative patients, there was an observed difference in T cell reactivity between the groups. The mitogen response in IGRAs may serve as a surrogate for T cell function and a marker of disease severity in COVID-19. (Source: Infectious Diseases in Clinical Practice)

Study of the Factors that Influence Low-Level Viremias in Human Immunodeficiency Virus Patients in a Tertiary Hospital
Conclusions A relationship was observed between low adherence, PI as the third drug, and LLVs. In patients with possible poor adherence to ART, a third drug with a high genetic barrier was used, but this did not prevent the presence of LLVs, making it necessary to use other strategies and closely monitor these patients. A Retrospective Study of Febrile Neutropenia From a Cancer Hospital: Pathogenic Organisms, Time to Antibiotic Administration, and Prognostic Factors
Conclusions Gram negative was the predominant pathogen of neutropenic patients in this study. Shorter time to antibiotic administration did not contribute to a better outcome. Sepsis was an unfavorable predictive factor of death. (Source: Infectious Diseases in Clinical Practice)

Outcomes of Implementing an Infectious Diseases Approval of Selected Antimicrobials as an Antimicrobial Stewardship Initiative in a Referral Hospital in Iran
This study is a quasiexperimental study that focused on the consumption of the 5 costly antimicrobial drugs, including liposomal amphotericin B, caspofungin, colistimethate sodium, linezolid, and voriconazole between preintervention and postintervention plans (2016–2018). If the antimicrobial was not approved by the infectious disease physician, possible alternative medications were used. Indexes, including total consumption rate, per capita consumption, total cost, per capita cost, mortality rate, and length of hospital stay, were measured annually. Results There were 29,268 and 27,704 hospitalized patients that ...

Diabetes Mellitus Influencing the Bacterial Species in Surgical Site Infections of Orthopedic Trauma Patients
This study compared the prevalence of bacterial species and their antimicrobial resistance in patients with SSI and with or without diabetes mellitus. Methods Patients with a postoperative SSI within 6 weeks after orthopedic surgery due to a fracture or joint injury (in a level 1 trauma center in Germany) were analyzed retrospectively for the time period of 2007 to 2012. Microbiological results were assessed by comparing the prevalence of bacterial species and minimal inhibitory concentration of 34 antimicrobial agents for both patient groups. Results Three hundred three patients (female = 140, male = 163) we...

Effects of Carbapenem Restriction on the Prevalence of Carbapenem-Resistant Acinetobacter baumannii Infection in Adult General Wards
Conclusions Implementing carbapenem restriction can be an effective strategy to decrease the prevalence of CRAB infection, subsequently decreasing mortality. However, the stewardship of other groups of antibiotics, which can be used as alternatives to carbapenems, is important to avoid the emergence of other bacterial strains. Subcutaneous Dirofilariasis in the United States: Literature Review and Case Report
Dirofilariasis is a filarial infection of domestic and sylvatic vertebrates such as canines, felines, raccoons, and bears (the definitive hosts) and is transmitted mostly by mosquitoes (the vector) with the exception being Dirofilaria ursi that is transmitted by the black fly. At least 60 cases of subcutaneous dirofilariasis in humans have been reported; however, the overall incidence of dirofilarial infection has increased, and dirofilariasis is considered an emerging zoonosis. Here, we present a case of subcutaneous Dirofilaria in an otherwise healthy woman in her sixth decade who presented with a painless, mobile, noner...
