Distribution of Hepatitis B Virus Genotypes, Subgenotypes, and Prevalence of Hepatitis B Virus Mutations in Untreated Patients With Chronic Hepatitis B From Mekong Delta
Background Drug resistance has become a major concern for antiviral therapy in patients with chronic hepatitis B (CHB), leading to an increased risk of treatment failure. We aimed to identify the prevalence of hepatitis B virus (HBV) DNA polymerase mutations and to determine the frequency of HBV genotypes and subgenotypes in treatment-naive patients with CHB in Mekong Delta, Vietnam. Methods Of the 550 drug-naive CHB patients from Mekong Delta, their serum samples were analyzed for HBV DNA mutations, genotypes, and subgenotypes using polymerase chain reaction sequencing. Results Of the 550 patients who we...

Rates of Prophylaxis Among Pneumocystis jirovecii Pneumonia Cases Attributable to High-dose Steroid Therapy: A Single-center Experience
Background The incidence of Pneumocystis jirovecii pneumonia (PJP) among patients on high-dose steroid therapy is not well established, and few guidelines exist to help guide decisions regarding prophylaxis. We assessed provider prescribing practices regarding pneumocystis prophylaxis and conducted a retrospective chart review of 306 PJP cases at a tertiary care center to identify preventable cases and target interventions to reduce harm. Methods We surveyed providers regarding their threshold to initiate PJP prophylaxis among patients receiving steroid therapy. Subsequently, all encounters with a positive PJP P...

Discontinuation of Transmission Precautions for COVID-19 Patients: Polymerase Chain Reaction Diagnostics, Patient Delays, and Cycle Threshold Values
Conclusions Concerns surround immunocompromised patients and those treated with steroids who might be delayed or incapable of stopping viral replication and thus remain contagious. Our results suggest that clinicians use all available data including Ct values to evaluate the safety of discontinuation of transmission precautions. Use of Industrial Filters by Health Care Workers During Shortages of N95 Respirators in Pandemic Times
The coronavirus disease 2019 (COVID-19) pandemic has led to a significant shortage of personal protective equipment in multiple health care facilities around the world, with the highest impact on N95 respirator masks. The N95 respirator is a mask that blocks at least 95% of very small (0.3 μm) particles and is considered a standard for enhanced respiratory precautions. The N95 mask shortage has created a need for other options for nasal and oral respiratory protection with similar filtration efficiency and “medical-grade” clearance, which can be used in health care settings. However, the literature around various filt...

An Actinomyces Long-Bone Osteomyelitis Case Report and Review
We report a case of monomicrobial subacute late-onset tibia bone osteomyelitis involving Actinomyces presenting 10 years after an open reduction and internal fixation surgical procedure performed to stabilize a traumatic fracture and review actinomycotic osteomyelitis including the rare association of Actinomyces odontolyticus with long bone infections. A Case of Streptococcus intermedius Abscess Within a Malignant Neck Mass and Review of the Literature
Streptococcus intermedius is an organism found commensally in various mucosal sites. Over the past couple decades, it has been recognized as a cause of severe pyogenic infections in sites including the brain, liver, lung, head and neck. Here, we describe an uncommon case of S. intermedius infection in a patient with tonsillar squamous cell carcinoma and a metastatic neck mass with overlying postradiation skin changes. We also review the literature on this pathogen in cancer patients because it remains understudied despite its pathogenicity. (Source: Infectious Diseases in Clinical Practice)

Pulmonary Tuberculosis With Central Nervous System Streptococcus anginosus Coinfection in an Immunocompetent Man
We present a case of pulmonary TB and Streptococcus anginosus (SA) brain abscess in an immunocompetent middle-aged patient. This case presented with left upper and lower limb weakness and was found to have right-sided subdural empyema due to SA along with pulmonary TB. In cases of suspected CNS-TB, which develop neurological deterioration despite adequate treatment, an alternative microbiological diagnosis should be kept in the differentials. The CNS bacterial infections from the SA group occurring in concurrence with pulmonary TB can lead to severe complications. Timely neurosurgical intervention may be needed in such ins...

An Atypical Neurocysticercosis Case Requiring Surgical Intervention
Cysticercosis, caused by tissue invasion of Taenia solium, is estimated to impact nearly 50 million people worldwide, particularly in areas with substandard sanitation and food handling measures. Neurologic dissemination of the larvae represents one of the most undertreated diseases worldwide despite being the leading cause of acquired epilepsy, prompting its addition to the list of major Neglected Tropical Diseases in 2010. Presenting symptoms are pleomorphic, ranging from asymptomatic to headaches and seizure activity. Therefore, high index for clinical suspicion is of utmost importance when obtaining history, particular...
