Treponema and Bartonella Coinfection in a Human Immunodeficiency Virus–Positive Individual
Infection with human immunodeficiency virus (HIV) leads to an acquired immunocompromised state, allowing for the rise of opportunistic infections. Here we present the first documented case, to our knowledge, of coinfection with Treponema pallidum and Bartonella henselae in an HIV (+) individual. Wound Infection With Enterococcus hirae Associated With Cat Litter
We report a case of wound infection caused by Enterococcus hirae in a 60-year-old woman with poorly controlled diabetes mellitus type II. She initially reported to the hospital because of chest pain and fatigue. During her hospital admission, an erythematous wound, measuring 1.5 cm in diameter, was discovered along the medial aspect of her right foot. Debridement of the wound revealed small pebbles, which the patient identified as cat litter. Wound cultures identified E. hirae, a known feline commensal organism and potential pathogen. Her wound was successfully treated with an inpatient course of intravenous ceftriaxone an...

An Erratic Exanthem: Postvaricella Guillain-Barre Syndrome With Facial Diplegia
Varicella zoster (Chickenpox) usually occurs in childhood due to infection with varicella zoster virus. It manifests as an acute febrile exanthem which is usually self-limiting. Varicella zoster virus infection, in most cases, provides lifelong immunity to an individual after primary infection. In adults, primary varicella is uncommon, and most cases occur as a result of reactivation of the dormant virus. Subsequently, in adults, varicella infection can behave erratically to present with various unusual systemic and neurological complications, which are again uncommonly seen after primary varicella. The most common neurolo...

Vaginal Malakoplakia: Cancer Mimicking Infection—A Case Review and Review of Literature
We present a case of a female patient who sought medical care for acute pelvic discomfort attributed to urinary retention after urinary obstruction. Although the discovery of multiple masses within the vagina and pelvic cavity indicated a tumor, it was originally negative for malignancy. Histology of biopsy samples demonstrated Michaelis-Gutmann bodies mixed with lymphoid cells through immunohistochemical and periodic acid–Schiff stains, consistent with a diagnosis of malakoplakia. Despite negative cultures, multiple courses of antibiotics were given, but the mass progressed and caused urinary obstruction. After pelvic e...

Protecting Vulnerable Patients from Influenza During the COVID-19 Pandemic: An Urgent Call to Action for Health Care Professionals
The 2020–2021 US influenza season, although mild, initially raised concerns about an unprecedented dual threat of SARS-CoV-2, the virus that causes COVID-19, circulating alongside seasonal influenza viruses. Although everyone is susceptible to influenza infection, adults with chronic health conditions (including heart disease, lung disease, and diabetes) are particularly vulnerable to influenza-related complications including hospitalization, disability, and death—as are older adults (65+ years) and adults in underserved communities, in which rates of chronic health conditions are higher. Many of the chronic health con...
