Standardised training for endoscopic mucosal resection of large non-pedunculated colorectal polyps to reduce recurrence (*STAR-LNPCP study): a multicentre cluster randomised trial
Conclusion A compact standardised EMR training for LNPCPs significantly reduced recurrences in community hospitals. This strongly argues for a national dedicated training programme for endoscopists performing EMR of ≥20 mm LNPCPs. Interestingly, in sensitivity analysis, this benefit was limited for LNPCPs ≥40 mm. Cirrhosis and Portal Hypertension Worsen Bowel Preparation for Screening Colonoscopy
CONCLUSIONS: Patients with cirrhosis have worse BBPS scores and longer cecal intubation times. Nonobese cirrhotic patients additionally have a lower polyp detection rate. Portal hypertension complications were associated with worsened preparation quality and longer cecal intubation times. Each incremental increase in MELD score lengthened cecal intubation time. In Reply We appreciate the Letters from Drs Imperiale and Baird and colleagues. Regarding sessile serrated lesions (SSLs) of at least 1cm in size, we are following the most recent US Multi-Society Task Force guidelines, which were cited by Imperiale and colleagues. These guidelines state “Because evidence of the risk of metachronous neoplasia associated with serrated lesions is evolving, whenever possible we have chosen not to include SSPs [sessile serrated polyps] and HPs [hyperplastic polyps] in our definitions of low-risk adenoma, high risk adenoma, and advanced neoplasia, and will refer to these lesions separately.â€...

Multi-scale nested UNet with transformer for colorectal polyp segmentation
CONCLUSIONS: The proposed model produced more accurate polyp segmentation than current methods on four different public and one in-house datasets.
