Molecular pathological classification of colorectal cancer —an update
AbstractColorectal cancer (CRC) has a broad range of molecular alterations with two major mechanisms of genomic instability (chromosomal instability and microsatellite instability) and has been subclassified into 4 consensus molecular subtypes (CMS) based on bulk RNA sequence data. Here, we update the molecular pathological classification of CRC with an overview of more recent bulk and single-cell RNA data analysis for development of transcriptional classifiers and risk stratification methods, taking into account the marked inter-tumoural and intra-tumoural heterogeneity of CRC. The importance of the stromal and immune com...

Histopathological risk stratification of pT1 colorectal cancer: novel insights into established risk factors
The proportion of submucosal invasive (pT1) colorectal cancer (CRC) diagnoses has increased substantially since the introduction of CRC screening programmes. A distinct attribute of pT1 CRCs is that they can be potentially cured through local excision. With advancements in endoscopic or minimally invasive surgical resection techniques, local excision has become the primary treatment modality for the majority of these malignant polyps. After local excision, the decision regarding the need for additional surgical intervention is primarily based on the presence of histopathological risk factors for lymph node metastasis (LNM)...

Post-colonoscopy colorectal cancers in a national fecal immunochemical test-based colorectal cancer screening program
Conclusions The 3-year PCCRC rate was low in this FIT-based CRC screening program. Approximately half of PCCRCs were interval PCCRCs. These were mostly caused by missed lesions and were diagnosed at a more advanced stage. This emphasizes the importance of high quality colonoscopy with optimal polyp detection. Autonomous Artificial Intelligence vs Artificial Intelligence –Assisted Human Optical Diagnosis of Colorectal Polyps: A Randomized Controlled Trial
In this study, autonomous artificial intelligence performed with high accuracy in diagnosing small (1 –5-mm) colorectal polyps during colonoscopies and allowed follow-ups to be scheduled more correctly than when humans decided the final diagnosis. (Source: Gastroenterology)

Low Colorectal Cancer Risk after Resection of High-Risk Pedunculated Polyps
Post-Faecal Immunochemical Test (FIT) colonoscopy represents a setting with an enriched prevalence of advanced adenomas. Due to an expected higher risk of Colorectal Cancer (CRC), post-polypectomy surveillance is recommended, generating a substantially increased load on endoscopy services. The aim of our study was to investigate post-polypectomy CRC risk in a screening population of FIT+ subjects after resection of low- (LRA) or high-risk adenomas (HRA). Fibre-rich Foods to Treat Obesity and Prevent Colon Cancer trial study protocol: a randomised clinical trial of fibre-rich legumes targeting the gut microbiome, metabolome and gut transit time of overweight and obese patients with a history of noncancerous adenomatous polyps
This study is designed to characterise changes in (1) body weight; (2) biomarkers of insulin resistance and systemic inflammation; (3) compositional and functional profiles of the faecal microbiome and metabolome; (4) mucosal biomarkers of CRC risk and (5) gut transit. Approximately 60 overweight or obese adults with a history of noncancerous adenomatous polyps within the previous 3 years will be recruited and randomised to one of two weight-loss diets. Following a 1-week run-in, participants in the intervention arm will receive preportioned high-fibre legume-rich entrées for two meals/day in months 1–3 and on...

Recurrent Ileo-ileal Intussusception Caused by Inflammatory Fibroid Polyp: A Rare Case Report
We present a case of a 46-year-old female admitted to our clinic with recurrent ileus attacks. We performed segmental resection of the small bowel due to a 3-cm pedunculated polypoid lesion located in the terminal ileum that caused ileo-ileal intussusception and whose pathology was reported as an inflammatory fibroid polyp. In adults presenting with ileus, the possibility of intussusception should be kept in mind.PMID:38310437 | PMC:PMC10685829 | DOI:10.34172/aim.2023.53 (Source: Archives of Iranian Medicine)
