Some thoughts on where we are with Covid-19
I hesitated to show you this graphic because it might lead to unwarranted complacency, but I ' ll show it to get it over with and then discuss. (Source:Worldometer)  Basically, the death rate has held pretty steady since last April, at around 400 a day. That ' s obviously well below the previous peaks, but I don ' t think people would tolerate it from any other readily preventable cause. However, deaths are a lagging indicator and the CDC is reporting a widespread increase in cases around the country.  Again, not a major spike like last March but we don ' t know where it ' s going yet. New strains are h...

State of health visiting, UK survey report: a vital safety net under pressure
This report details the findings from a UK survey of frontline health visitors working with families across the United Kingdom. It finds health visitors reporting epidemic levels of poverty, with more parents struggling with the cost-of-living crisis that is forcing them to turn to food banks to feed their children. Alongside this, more parents are living with mental health problems, domestic abuse and adversity, that pose risks to the health and wellbeing of babies and young children. The report makes various policy recommendations including a shift towards prevention and early intervention, equity of access to support an...

Answer to Case 707
Answer to theParasite Case of the Week 707:Sarcocystisspecies. Molecular testing is required for identification of the causative species. This case shows a single intramuscular sarcocyst containing multiple bradyzoites. The clinical history and epidemiology also supports a diagnosis of sarcocystosis. Many of you rightly suggested that the differential diagnosis includesToxoplasma gondii,a related apicomplexan parasite which can form similar-appearing cysts in tissues throughout the body. Toxoplasmosis involving skeletal muscle is seen primarily in profoundly immunocompromised individuals with widely disseminated ...

Various microbial species present in the gut produce beneficial metabolites, such as butyrate, or harmful metabolites, such as isoamylamine, or can provoke chronic inflammation in a variety of ways. An individual can have a better or worse microbiome, assessing these and other...

Expanding Real World Datasets
How are you working to advance research and improve patient outcomes? Are you precisely matching records across disparate datasets? Find out at a Webinar on Feb 1st 1pm ET Sponsored by LexisNexis Risk Solutions Health Care Healthcare’s fragmented data silos and strict but necessary privacy restrictions make it difficult to link real-world datasets. Legacy tokenization technology has helped link records across disparate data sources, but it lacks the accuracy required to uncover actionable insights that can truly improve patient outcomes. Next-generation tokenization technology leveraging a Referential Data Layer is ne...

Chronic Inflammation and Endothelial Dysfunction in Vascular Aging
The age-related decline and dysfunction of the vasculature kills a sizable fraction of humanity, directly through stroke and heart attack, and more indirectly through a number of other mechanisms. The rise of chronic inflammation with age can be blamed for much of this. Unresolved inflammatory signaling is highly disruptive to tissue function throughout the body. In the vasculature, it accelerates the altered macrophage behavior that lies at the root of atherosclerosis. It provokes some of the altered smooth muscle behavior that leads to hypertension as contraction and dilation of blood vessels become poorly controlled. It...

Milwaukee County Addresses Health Equity with Data and GIS Technology
Milwaukee County is at the forefront of addressing racial inequity. During the COVID-19 pandemic they cross referenced vaccination rates with the CDC’s social vulnerability index and plotted the results using Geographic Information System (GIS) technology. Armed with this powerful insight, the County was able to increase vaccinations in populations that needed them the most and by doing so increased the safety of the entire County. They are now incorporating an equity lens into other areas of public health. Healthcare IT Today had the opportunity recently to sit down with Zachary Swingen, Lead GIS Coordinator at Milwauke...

Dehydration as an Accelerator of Aging
In this study, we report on serum sodium in the upper part of the normal range being a risk factor for accelerated aging. In the Atherosclerosis Risk in Communities (ARIC) study, odds to be biologically older than chronological age was increased in the study participants whose serum sodium exceeded 142 mmol/l, reaching 50% increased odds at sodium levels exceeding 144 mmol/l. Such elevated biological age at middle age (47-68 years) translates into an approximate 20% increased risk of premature mortality at sodium levels greater than 144 mmol/l and increased risk to develop chronic diseases, that was already evident at s...

' In response to a rather complicated question from a reader . . . I ' m not a real doctor, I ' m a doctor of philosophy, and I ' m not an expert on the technical ins and outs of Covid-19 mutations and immunity thereto. But the message you need to hear is very simple: the boosters out there right now, today, are very effective at preventing severe disease, even with the newest mutants. So if you don ' t want to go to the hospital and you don ' t want to die, get the shot. They aren ' t as effective at preventing infection against new variants, and hence less effective at preventing transmission, but the big problem the...

This article on senolytic therapies to selectively remove senescent cells in old tissues is in part a matter of Unity Biotechnology talking up their position. The company suffered from first mover disadvantage in bringing senolytic drugs into clinical development. The field has made progress very rapidly over the last decade, and startups founded even just a couple of years after Unity's launch benefited from greater knowledge and a selection of better technologies to work with. Still, one can be talking up one's position and also be right. The accumulation of senescent cells is profoundly harmful, a significant contributi...

A Change Of Heart: " For The Children "
Last night, I archived everything on the blog except " Why Pediatrics " .  More and more, it ' s become a question rather than a statement of purpose.  In the wake of the COVID 19 pandemic, where the entire US system shifted on its axis to shut down community Pediatrics services and " feed " children ' s hospitals (to free up resources for sick adults), the situation for those of us practicing inpatient Pediatrics (as " Pediatric Hospitalists " ) really went to hell.  You could not find/beg a job in the community setting during the pandemic.  Actually, jumping through the ABP ' s hoops to do a...

Evidence for Viral Infection and Inflammation in Familial Alzheimer's Brains
Familial Alzheimer's disease has an earlier onset in comparison to the sporadic Alzheimer's present in much of the population, and is connected to specific variants in genes associated with amyloid-β metabolism. In some cases that means one has to be careful of drawing conclusions based on studies of familial Alzheimer's. Today's research materials note the discovery of viral infection and signs of inflammatory dysfunction in the brains of familial Alzheimer's patients. These patients are likely more vulnerable to the consequences of viral infection, such as increased generation of amyloid-β in its antimicrobial peptide ...

Xish
Earlier I discussed the insane excesses of Xi Jinping ' s " zero Covid " policy. The economic and social costs of the draconian, and doomed, effort to end transmission of the virus in China were unsustainable. The policy provoked the first mass protests of his rule, at great peril to the protesters, created supply chain shortages around the world, and severely damaged the Chinese economy. So yeah, not a good idea. But . . .He suddenly turned 180 on a dime and eliminated all mitigations measures. The result has been equally catastrophic. Eric Fiegl-Ding is an epidemiologist who became well known as an early Cassandra about ...
