random neurocognitive question - deja vu
Hey all, Was wondering if you all had seen this phenomenon before, where a patient (likely suspected dementia-related syndrome) reports beliefs of deja vu (i.e., "Those people were here the last time we came to this store," or "These two teams on TV played already and the plays they're running are exactly the same as last time" or something along those lines). Workup for Old People
Over a year out of residency, working a lot of evening/night shifts, still uncomfortable with old patients that come in at night... 2 scenarios: 1. 85 yo F with dementia, a million other medical problems, unwitnessed GLF fall at SNF, A&O x1 maybe x2 at baseline, doesn't remember what happened. dexmedetomidine (POPrecedex) for agitation, and maybe delirium, and maybe dementia with behavioral disturbance?

Reading and billing MRIs
Do you guys know neurologists who read their own MRIs/CTH etc and bill for them? In the past you could get boarded without any kind of fellowship or training and I know few neurologists who are doing that. What about anyone without a board certification? Hi, Pgy2 here, doing one of my 24 hours call. I am just so sad. I am sad, that I don't feel like I love this feel. I feel like my personality just does not fit this field.
