Objective measures of IADl in clinical practice
What are your guys thoughts on measures like the Texas Functional Living Scale in the assessment of dementia? Is there a certain level of impairment on the objective test that meet the criterion for adl as in the dsm-v, or again is that based on the data as a whole? (Source: Student Doctor Network)

How is decline assessed in clinical practice?
Often in research decline may be assessed by asking a simple question like "do you have memory problems", which of course doesn't actually take into account if the patient has actually declined or not (what if the patient always had memory problems?!?). In clinical practice how is this criterion met? This is in the context of mci/dementia. (Source: Student Doctor Network)

"palliative" non-cancer chronic pain and opioids
This topic has been brought up in a few other threads, but not sure that it has been the focus of any thread. I have seen some of you use the term "palliative" to describe some non-cancer situations where you may consider low dose opioids <50 MME. Hygiene question.
What do you say? If a friend/patient comes to you and states: "My 89 year old mother with mild dementia will not take a shower or will only take a shower every 2-3 weeks. She just wants to "sponge off". [This has been going on for perhaps 3-4 months] Talking and explaining the benefits of hygiene to her do not seem to have much effect. Any thoughts on this? Hello to the respected forum of anesthesia.. Had a case recently and I was thinking about it. Pt was 80 y.o.
