Nursing home and residential tx jobs?
I need some help. There are two facilities near me that are offering me some options as i build up my practice. 1) Nursing home with medicare/medicaid mix less than 5 minutes from my house : have about 30 dementia/alzheimers locked unit and another 20-30 patients on other units that they want me to round on at least 1x a month. There is already a medical director there. I would bill on my own which is fine so i'm guessing 50 pts x 60 dollar a visit. 2) Residential tx facility medicaid with... Why is Social Work not considered Public Health or Vice Versa?
Sorry if this seems like a dumb or strange question but for the life of me I can't understand why Social Work which looks at servicing potentially disenfranchised populations, is not considered underneath the umbrella of Public Health/Preventive Medicine? Is a social worker making sure a patient with dementia takes her pills not apart of preventing a potentially catastrophic situation that could adversely affect the health of the public? And are public health providers who study how to... Why is Social Work not considered Public Health or Vice Versa? (Source: Student Doctor Network)

DNR without DNI?
Do you guys typically offer to make patients DNR without also making them DNI? I'm not talking about patients who have conditions that are likely reversible such as pneumonia. Mainly asking about patients with many comorbidities who would be unlikely to be extubated if they were ever intubated or have advanced dementia/COPD/CHF. I've always felt that CPR and intubation cross a certain line and if they are unlikely to bring the patient back to a reasonable baseline, they should not be offered... DNR without DNI? medical power of attorney and dementia
Hi guys, question on medical power of attorney. Can medical power of attorney be revoked if the patient is mentally incompetent? The following usmlerx question says YES, but FA 2016 says "can be revoked by patient IF DECISION MAKING CAPACITY IS INTACT" To me, decision making capacity more or less equals mental competence??? Thanks! Question: The daughter of a pt with Alzheimer dementia is the designated surrogate. When does a designated surrogate lose his or her power?... medical power of attorney and dementia (Source: Student Doctor Network)

Feeling More Positive About Psychiatry
wrapped up my last week of residency and feeling more positive about psychiatry than ever. In the past month alone I have seen a wide range of patients including people with functional neurological disorder, somatization disorder, rapidly progressive dementia (most likely CJD), posterior cortical atrophy, NP-SLE, neurosarcoid, bvFTD phenocopy syndrome, TBIs (you can guess my subspecialty) as well as your garden variety depressed, deluded, deranged and dipsomaniacal patients. In terminating... Feeling More Positive About Psychiatry (Source: Student Doctor Network)

APA Guidelines on Antipsychotics and Agitation & Dementia
What do you all think about the new APA guidelines on treating patients with dementia and agitation? It looks ok to me, but one question I have is which quantifiable measurement tool of behavior should be used? And why do they go out of their way to recommend avoiding Haldol over some other antipsychotics? (Source: Student Doctor Network)
