Changes in Radiation Therapy: New Ultrahypofractionated Radiation, Repeat Breast Conservation Therapy in the Management of Breast Recurrence with Re-irradiation, Proton Therapy
AbstractPurpose of ReviewManagement of breast cancer is multidisciplinary, balancing local and systemic control between treatment modalities. Surgical, radiation, and medical therapies are combined to give the best prognosis. The goal of this review is to examine the advancements in radiotherapy in the treatment of breast cancer.Recent FindingsPublished data supports the shortening of radiation schedules through ultrahypofractionation and utilizing new energy sources through proton beam radiation. Emerging techniques have minimized radiation dosage to organs at risk and skin when compared to traditional radiation sources a...

Comparative study of weekly and three-weekly neoadjuvant carboplatin in triple-negative breast cancer: retrospective analysis in real-world settings
ConclusionsWeekly carboplatin exhibited comparable toxicity, a trend toward fewer interruptions, and similar pCR rates. Prospective studies are essential for validating these findings. (Source: Clinical and Translational Oncology)

Omission of adjuvant radiotherapy in low-risk elderly males with breast cancer
CONCLUSION: Among men ≥ 65 years old with T1-2N0 HR + breast cancer, RT alone did not confer an OS benefit over HT alone. Combination of RT + HT demonstrated significant improvements in OS. De-escalation of treatment through omission of either RT or HT at this point should be done with caution.PMID:38507145 | DOI:10.1007/s12282-024-01560-y (Source: Breast Cancer)

Nipple Reconstruction Using the "Arrow Flap" Technique: Outcomes and Patients Satisfaction
CONCLUSION: The authors believe that the NAC reconstruction has useful functional and aesthetic results particularly appreciated by patients who feel demoralized after breast demolition surgery.PMID:38503614 | DOI:10.1016/j.clbc.2024.01.011 (Source: Clinical Breast Cancer)

Phyllodes Tumors of the Breast —Experience in a Tertiary Care Center
This study was planned to report the clinicopathological features and outcomes of patients with PT treated at our center. Details of all patients who underwent surgery for PT in the last 6 years (December 2017 –December 2023) were obtained from our prospectively maintained database. The demographic, clinical, radiological, pathological, and follow-up details were recorded and analyzed. Statistical analyses were carried out withJamovi version 2.3.18. Out of 61 suspected PTs by triple assessment, 9 were excluded due to a non-phyllodes diagnosis on histopathology. We included 52 women with a mean age of 38.2  ± 11.0 ye...

Cancers, Vol. 16, Pages 1198: Does Breast Surgery Type Alter Incidental Axillary Irradiation? A Dosimetric Analysis of the & ldquo;Sentinel Envahi et Randomisation du Curage & rdquo; SERC Trial
Conclusions. On the Berg level 1, PMRT gives a dose at least equivalent to the one given by post-breast-conserving surgery radiotherapy, making it possible to limit completion axillary lymph node dissections in select pN1a patients treated with a mastectomy. Modern radiotherapy techniques like VMAT tend to increase this incidental dose. (Source: Cancers)

Breast tissue imaging atlas using ultra-fast confocal microscopy to identify cancer lesions
The objective of the study was to create an atlas of ex vivo UFCM images of breast tissues and breast carcinomas based on the first extensive collection of large field-of-view UFCM breast images. One hundred sixty patients who underwent conserving surgery for breast cancer were included. Their fresh surgical specimens were sliced, stained with acridine orange, and imaged at high resolution with large-field-of-view UFCM. The resulting images were digitally false colored to resemble frozen sections. Each UFCM image was correlated with the corresponding definitive histology. Representative images of normal tissue, inflammatio...

Paget ’s disease of the breast: Insights from imaging to guide surgical management
Paget ’s disease (PD) of the breast is a rare condition accounting for only 1–3% of breast cancers, as evident by the paucity of published data and small cohort analyses.1 Due to its multifocal variation and the unreliability of mammography and ultrasound to detect underlying breast cancer in PD, mast ectomy had long been the standard of care for adequate disease control.1,2 However, further studies comparing mastectomy to breast-conserving surgery (BCS) followed by adjuvant radiation have shown no statistical difference in overall survival. Cancers, Vol. 16, Pages 1169: De-Escalation Surgery in cT3-4 Breast Cancer Patients after Neoadjuvant Therapy: Predictors of Breast Conservation and Comparison of Long-Term Oncological Outcomes with Mastectomy
Conclusions: BCS after NAT is a feasible and safe option for patients with cT3-4 BC, without adversely affecting long-term oncological outcomes. Identifying predictors of breast conservation can guide surgical decision-making, ensuring that patients receive optimal treatment. (Source: Cancers)

Local treatment of ipsilateral breast cancer recurrences: comparison of alternative therapeutic options
Magy Onkol. 2024 Mar 14;68(1):86-88. Epub 2024 Jan 30.ABSTRACTWe compared the clinical outcomes of second breast conserving therapy (2ndBCT) versus salvage mastectomy (sMT) for the treatment of ipsilateral breast tumour recurrences (IBTR). 195 patients who presented with an IBTR after previous breast conserving treatment were salvaged either with re-excision and perioperative interstitial brachytherapy (n=39) or sMT (n=156). A total dose of 5×4.4Gy was delivered to the tumour bed, on 3 consecutive days. The median follow-up time was 59 and 56 months. During follow-up 4 (10.2%) and 28 (17.9%) second local recurrences occur...
