Myeloperoxidase‐ANCA‐Positive and ANCA‐Negative Patients With Granulomatosis With Polyangiitis: Distinct Patient Subsets
Conclusion We were unable to demonstrate important clinical differences between MPO‐ANCA+ and PR3‐ANCA+ patients with GPA. The risk of relapse was associated more closely with disease type than with ANCA type in this patient cohort. These findings deserve consideration in the assessment of relapse risk in patients with AAV. This article is protected by copyright. All rights reserved. (Source: Arthritis and Rheumatism)

Nationwide Trends in Hospitalizations and In‐Hospital Mortality of Granulomatosis with Polyangiitis
Conclusion: The findings from these nationally representative, contemporary, inpatient data indicate that in‐hospital mortality of GPA has declined substantially over the past two decades while the overall hospitalization rate for GPA increased slightly. Infection remains a common principal hospitalization diagnosis among GPA patients, including hospitalizations resulting in mortality. This article is protected by copyright. All rights reserved. Clinical spectrum and outcome of pulmonary nocardiosis: 5-year experience
Conclusion: The diagnosis of pulmonary nocardiosis is often missed and delayed resulting in delay in appropriate treatment and thus high mortality. A lower threshold for diagnosing pulmonary nocardiosis needs to be exercised, in chest symptomatic patients with underlying chronic lung diseases or systemic immunosuppression, for the early diagnosis, and treatment of this uncommon but potentially lethal disease. Despite treatment mortality remains high, especially in those with brain abscess, HIV positivity, need for mechanical ventilation, solid-organ transplantation, and elderly. (Source: Lung India)

Inhibitors and Antibody Fragments as Potential Anti-Inflammatory Therapeutics Targeting Neutrophil Proteinase 3 in Human Disease.
Abstract Proteinase 3 (PR3) has received great scientific attention after its identification as the essential antigenic target of antineutrophil cytoplasm antibodies in Wegener's granulomatosis (now called granulomatosis with polyangiitis). Despite many structural and functional similarities between neutrophil elastase (NE) and PR3 during biosynthesis, storage, and extracellular release, unique properties and pathobiological functions have emerged from detailed studies in recent years. The development of highly sensitive substrates and inhibitors of human PR3 and the creation of PR3-selective single knocko...

An eosinophilic variant granulomatosis with polyangiitis involving the dura, bilateral orbits, and mastoids.
We report a case of a 57-year-old lady presenting with left eye swelling, left ear pain and discharge, but with no pulmonary or renal symptoms. Investigations revealed positive cytoplasmic antineutrophil cytoplasmic antibodies  and proteinase 3 antibodies. The CT and MRI showed meningeal thickening and bilateral structural changes of the orbits and mastoids. Lacrimal gland biopsy showed non necrotizing granulation with an eosinophilic infiltration. She was diagnosed with eosinophilic variant of GPA of the eyes and mastoid bones bilaterally extending to dura and sparing the lungs and kidneys. She responded to corticosteroi...

Diagnoses and Management of Drug Hypersensitivity and Anaphylaxis in Cancer and Chronic Inflammatory Diseases: Reactions to Taxanes and Monoclonal Antibodies
Abstract Due to the increase in utilization of chemotherapies and antibodies, drug hypersensitivity reactions have increased dramatically worldwide, preventing the use of first-line therapies and impacting patients’ survival and quality of life. Some of the more frequently used medications in cancer include taxanes for ovarian, lung, breast, and prostate cancers. Monoclonal antibodies are used in the treatment of neoplastic, autoimmune, and inflammatory diseases, and their clinical applications are becoming broader. Monoclonal antibody targets include CD20, HER-2, EGFR, IL-6 receptor, TNF-α, CD30, VEGF...

[Case Report Comment] Clinical overlap in proximal artery stenosis: two disorders or one?
ANCA-associated vasculitis comprises three conditions that share overlapping clinical and serological features and are characterised by necrotising inflammation of small vessel walls: granulomatosis with polyangiitis (Wegener's granulomatosis), microscopic polyangiitis, and eosinophilic granulomatosis with polyangiitis (Churg Strauss syndrome). Although the cause is unknown, ANCA antibodies, cytokine-primed neutrophils, and B lymphocytes are recognised to play an important part in disease pathogenesis. (Source: LANCET)
