Question from EM - how do you differentiate liver disease vs hemolysis labs?
Hello IM friends, I am in EM and have a question that I think better fits your expertise. I've had several chronic alcoholic patients come in with a lab pattern of anemia, low platelets, and elevated bilirubin. I know this can be expected for some cirrhotics, but it is also the pattern that might prompt consideration of hemolysis/MAHA. What do you do in these situations to differentiate between the two? I'm sure that sending a full hemolysis panel and smear would help guide us, but we... Read more (Source: Student Doctor Network)

Lymphadenectomy in high-risk prostate cancer
Hey! I am asking for a friend here. He is planning a trial and would like to have some feedback on worldwide practice. In your experience, do urologists perform lymphadenectomy in high-risk cN0 prostate cancer (provided the high-risk cancer was known before surgery - based on GS, PSA, cT3) when performing radical prostatectomy as primary treatment. Thank you! (Source: Student Doctor Network)

ABIM Board Eligibility with foreign residency + US fellowship
I wouldn’t be surprised if this becomes a reality in the future. I think nephrology would feel the biggest impact as it has the most number of IMG fellows that haven’t completed US residency training. Source: Sign the Petition More and more International medical graduates are joining fellowships in the US after completing their home country residency and getting their credentials verified. About half of the current fellows in nephrology and geriatrics underwent residency abroad... Click to expand... Read more (Source: Student Doctor Network)

Dialysis catheters
How many of the nephrologists here are placing their own dialysis catheters? Anesthesiologist here in a private practice that currently places all of the dialysis catheters for our hospital. Reimbursement for this is mostly terrible now and it’s becoming a bit of a pain with how busy our OR’s and OB service has become. This is the only hospital I’ve worked at where we are asked to do this. Any insight? Our group is likely only going to continue this service if we get a stipend either from... Nephrology Fellowship 2022 - 2023
Hey guys, Didn't see a fellowship thread for this year. I understand there may not be many of us applying to nephrology, figured I'd start a thread regardless for the few that are passionate about the field and have applied to nephrology despite allll the very many threads about how "nephrology is dead," "nephrology will take anyone with a heartbeat," "you're better off becoming a hospitalist" threads Has anyone heard from UCSF, UCLA, Stanford, or UC Davis? Salvage RT Prostate with Intestine In the Way
Had a young patient (56 yo) who had prostatectomy last year for Gleason 3+4 with low PSA in early ‘21. He had positive margin at the right base on path and had SVI bilaterally. PSA was negative after surgery for 5 measurements then rose to detectable but still low at .05 then .06 a couple months later. He had a PSMA which was negative and was referred to talk about early salvage. Fairly anxious patient who very much wanted RT. Urology sent him to a med onc too, bc of such low PSA they opted...
