What is an opthalmologist's take on an internist doing a funduscopic examination?
Hello. I am a part time nephrologist / Internist who sees a large number of hypertensive and diabetic patients. Although I do send these patients to opthalmology for a formal dilated retinal exam (which is part of the diabetes management anyway), I do have a PanOptic ophthalmoscope which I fiddle around with now and then for someone who is having uncontrolled BP for an undilated exam. It's not where near as good as those retinal photographs in the textbooks, of course. But I have gleaned... Read more (Source: Student Doctor Network)

Ondansetron tablet crushed vs ondansetron ODT
If both are given SL is there a difference in how they are metabolized? I understand ODT is absorbed in the floor of the mouth so it bypasses the liver metabolism but wouldn't this also happen with a crushed ondansetron tablet put under tongue? (Source: Student Doctor Network)

Worst Doctor Experiences as a Patient/Family of Patient
Teaching hospital. Parent admitted for major cardiac surgery. Surgery went over allotted time by 4-5 hours. Attending surgeon explained vaguely in waiting room something about a mistake; they had to scrub into the operation and take over for residents. Compounded by 2 days later at heavily sedated/unconscious parent's bedside during visiting hours. Medschool's attending nephrologist along with one of their fellows or resident come by the foot of bed. Doc sees patient's family member and... Read more (Source: Student Doctor Network)

HOPD with In-office lab?
Lets say a private practice Urology group with an in-house AP POD lab merges with a large hospital and becomes a Hospital Out-Patient Department. Would this affect the legality and/or billing allowances for their existing Path lab? Could they still operate their Path lab? I'm just not sure I know the answer to this. Is a dog feasible in residency?
Lately I've been thinking of adopting a dog. Currently an MS3 headed into fourth year where I've been led to believe I will have relatively reasonable hours where I could still have time to care for a dog. Planning on applying to gen surgery or urology right now, so wondering if anybody here has experience and can say whether or not owning and taking care of a dog is feasible in residency. I'm in a relationship but not sure if I'll be living with a SO during residency so I cant guarantee... Read more (Source: Student Doctor Network)

Questioning validity of oral sedation for pain procedures
Went for initial consultation for the ol' snip snip this past week. Nurse says, "if you want valium beforehand, only 2 of our docs offer it. It's 5mg 1hr prior and it has to be specifically requested beforehand." Makes me wonder -- if urologists aren't routinely giving benzos for vasectomies and those who do only give 5mg...why are pain docs giving any at all for our procedures? Anesthesiologists less stressed than CRNAs
Found this interesting article ranking stress of jobs-t Urologists are the most stressed of all workers. We rank pretty low at 87 even less than nurse anesthetists (96) - I guess we are either well trained and don’t feel stressed or this is just a crappy survey. I would agree I wouldn’t enjoy my job as much if I had to look at people’s junk all the time. (Source: Student Doctor Network)

tizanidine
Seems like PCPs are always freaking out about tizanidine. I used it regular in PMR residency without concern and later as a pain physician. I can understand hypotension risk in the elderly but what other common issues are there? I saw a 26 yr old today with an early spondy, failed interventions/PT, has leg spasms at tizanidine 4mg QID PRN really helps here, but her PCP is worried about liver damage.....despite never checking LFTs and her having no GI symptoms of any kind. What real... Read more (Source: Student Doctor Network)

What should I do if I have a 3.1 gpa?
I'm about to graduate with a BBH/psych double major and started looking into Occupational Therapy a bit too late. I have all the requirements (though not sure if I need to take Physics 2) and i have good grades in all of them except the two A and P labs, which I got Cs in due to health and COVID. (A and P 1 wascovid. In A and P 2 lab's case, I couldn't do a health exemption because of IBD and I ended up missing an exam due to hospitalization for Kidney prob) The only requirement I... No 3rd year surgery letter
I got placed with 6 different preceptors for my 3rd year surgery rotation. Different attending each week. It was a mix of general surg, ortho, ophto, ENT, urology, and derm. It wasn't enough time to make a rapport with any attending, and I barely did anything except shadow/ occasional suturing, and only interviewed patients/presented to one attending. I was hoping to get a LOR from my surgery rotation but that seems dumb now because what would the letter even say? Do I need to do a 4th year... Read more (Source: Student Doctor Network)

RVU production early on
I'm sure the answer to this will be "It depends on you," but I'm curious what a reasonable productivity would look like in years 1 and 2 of practice? I've interviewed with a couple places: 1) PP, large wait list for new patients, PAs run ball pain and rUTI clinics, see some minor post-ops and patients on surveillance. Current partners see either new patients in clinic or bigger post-ops (only see returns if they want to), sounds like there is plenty to go around. Set your own schedule but... Overpaid traffic cop
Citywide protests today against overpaid traffic cops who love to direct traffic and insist on moving cars safely across busy intersections. Local cardiologists protest the fact that traffic exists and motor vehicles travel on roads. Head of american urological association puts out statement - "if traffic cops love traffic so much, then they should get in the car and drive." City hall continues to ignore the fact that traffic lights have been broken since 1986. (Source: Student Doctor Network)
