Contextualising opioid-related risk factors before an initial opioid prescription
When asked about the similarities between the region of Valencia, Spain and the State of Oregon in the USA, an artificial-intelligence (AI)-driven chatbot poetically listed several common features, such as natural beauty, strong focus on agricultural industries including wine production, extensive outdoor activities and a shared commitment to sustainable practices. But are there similarities in healthcare delivery, considering vastly different systems and significant practice variability, particularly regarding the management of pain and the role of opioid analgesics? Here, the differences emerge. Oxycodone and hydrocodone...

Development, Physicochemical Characteristics and Pharmacokinetics of a New Sustained-Release Bilayer Tablet Formulation of Tramadol with an Immediate-Release Component for Twice-Daily Administration
ConclusionsTo address the potential concerns regarding existing formulations of tramadol, a twice-daily, extended-release bilayer formulation of tramadol consisting of an IR and SR layer was developed. Pharmacokinetic studies confirmed that the plasma tramadol concentration increased quickly after administration and was maintained over a long period of time. A retrospective study of traffic offenses associated with driving under the influence of drugs in China
CONCLUSIONS: In the sample, the case fatality rate was 70.7%, which was a strong indicator of underreporting, and most minor DUID-related offenses could not be captured by Chinese court files. The frequency of drug screening tests for people should be increased. The traffic police in Qinghai, Tibet, and Xinjiang should always be equipped with fast screening test kits for drugs, which could increase the possibility of identifying DUID offenders.PMID:38054845 | DOI:10.1080/15389588.2023.2285714 (Source: Traffic Injury Prevention)

The effects of dexamethasone added to ilioinguinal/iliohypogastric nerve (IIN/IHN) block on rebound pain in inguinal hernia surgery: a randomized controlled trial
ConclusionsIn conclusion, adding IV dexamethasone as a part of multimodal analgesia  to IIN/IHN block for inguinal hernia surgery resulted in lower MRPS and lower postoperative opioid (tramadol) consumption. For this reason, IV dexamethasone can be added to the IIN/IHN block after inguinal hernia surgery to reduce the incidence of rebound pain, rebound pain scores, and NRS scores for pain, decrease postoperative opioid consumption, and improve the quality of recovery.Clinical trial registrationClinicalTrials.gov: Ref; NCT05172908, Date: December 29, 2021. Long-lasting decreased pain threshold negatively affects functional recovery after arthroscopic rotator cuff repair
ConclusionPatients experiencing poor postoperative clinical outcomes exhibited prolonged lowered PPT. Lowered PPT due to sensitization may adversely affect functional recovery and pain perception. Elevating PPT using weak opioids improved clinical outcomes during the acute perioperative period after ARCR.Level of evidenceIII. (Source: Knee Surgery, Sports Traumatology, Arthroscopy)

The comparison of the analgesic efficacy of continuous paravertebral block alone compared with continuous paravertebral and intercostal nerve block for thoracotomy in adults: a randomized controlled trial
ConclusionContinuous paravertebral nerve block with intercostal nerve block provides effective post-operative pain relief after thoracotomy with reduced narcotic consumption compared to continuous paravertebral nerve block alone.Clinical trial registration numberClinicaltrails.gov NCT04715880. Potentially inappropriate medicine use and predicting risk factors in hospitalized older adult patients: findings of a prospective observational study from Ethiopia
ConclusionsNearly three-fourths of the participants had potentially inappropriate medicine in their medicine list. Therefore, generating local evidence on the clinical, economic, and humanistic consequences may help in determining whether the Beers criteria should be taken into account when prescribing medicine to older adults. Interventions targeting older adult patients with heart failure might reduce inappropriate medicine use. (Source: Journal of Pharmaceutical Policy and Practice)

Clinical efficacy and metabolomic analysis of ozone major autohemotherapy for the treatment of herpes zoster
CONCLUSIONS: Ozone autohemotherapy is an effective treatment for acute herpes zoster patients and can rapidly and effectively alleviate pain symptoms in the short term. The changes in serum metabolomics may provide further insights into the treatment mechanism.PMID:38028414 | PMC:PMC10646704 | DOI:10.5114/ada.2023.130522 (Source: Advances in Dermatology and Allergology)

Ceftriaxone Use Evaluation in Western Zone Tigray Hospitals, Ethiopia: A Retrospective Cross-Sectional Study
CONCLUSION: The appropriateness of ceftriaxone use in Kahsay Abera and Mearg hospitals was 247 (61.8%) and 252 (63%), respectively, in which about one-third of patients' charts were not compliant with the standard treatment guidelines of Ethiopia for general hospitals. In Kahsay Abera and Mearg hospitals, the empiric use of ceftriaxone was 262 (65.5%) and 375 (93.8%), respectively.PMID:38027045 | PMC:PMC10661873 | DOI:10.1155/2023/7688896 (Source: Biomed Res)
