Factors influencing gastrostomy tube feeding duration and nutrition outcomes in pediatric patients with Down syndrome: A descriptive cohort study
CONCLUSION: In our cohort of patients with Down syndrome, age at placement of G-tube did not impact the duration of G-tube feeds. Most patients who had a G-tube placed were likely to require enteral feeds for at least 1 year. Those who had a tracheostomy needed their G-tube for a longer time.PMID:38715451 | DOI:10.1002/jpen.2637 (Source: JPEN Journal Of Parenteral And Enteral Nutrition)

Pre-Operative Characteristics Helping to Avoid Gastrostomy Tube After Mandibular Distraction in Neonates With Pierre-Robin Sequence: A Institutional Case-Series and Review of the Literature
CONCLUSION: MDO may be considered as a surgical option to prevent G-tube placement for neonates with non-syndromic RS who have difficulty with PO feeding but whose airway obstruction is not severe enough to require respiratory support. Based on our institutional experience, a minimum weight of 3.00 kg correlated with higher success rates of PO intake and avoiding a G-tube.PMID:38712740 | DOI:10.1177/00034894241249547 (Source: The Annals of Otology, Rhinology, and Laryngology)

The Safety of Pediatric Bedside Tracheostomy
CONCLUSIONS: Our results suggest that pediatric open bedside tracheostomy in an ICU setting is a safe procedure, with similar complications and outcomes compared to tracheostomy performed in the operating room.PMID:38712736 | DOI:10.1177/00034894241252231 (Source: The Annals of Otology, Rhinology, and Laryngology)

Surgical Stabilization of Rib Fractures in the Geriatric Trauma Population Is Associated with Equivalent Outcomes to a Younger Cohort: A Propensity Matched Analysis
Traumatic rib fractures are a significant cause of morbidity and mortality[1]. Surgical rib fixation has been found to reduce duration of ventilation, hospital length of stay, need for tracheostomy, and mortality in patients with flail chest[2,3]. Significant benefits have also been found in patients with severely displaced rib fractures without flail chest, including reduced pain, improved disability-related quality of life, and reduced narcotic use[4]. As such, the Chest Wall Injury Society (CWIS) has established guidelines recommending surgical stabilization of rib fractures (SSRF) be offered to patients with chest wall...

Effects of mechanical insufflation-exsufflation on ventilator-free days in intensive care unit subjects with sputum retention; a randomized clinical trial
ConclusionIn ventilated subjects in the ICU with high sputum retention, the use of MI-E did not significantly increase the number of ventilator-free days over that with conventional care. (Source: PLoS One)

Tracheostomy – A Comparative Study of Decannulation with Gradual Blocking of the Tube vs. Reduction of the Size of Tube - A Prospective Study
ConclusionThe two decannulation methods, viz., gradual blocking of tube and reduction of tube size, showed comparable outcomes in terms of tube reinsertion rate, mechanical ventilation rate after decannulation, successful decannulation, and complications. Health care disparities in morbidity and mortality in adults with acute and remote status epilepticus: A national study
Epilepsia. 2024 Apr 30. doi: 10.1111/epi.17965. Online ahead of print.ABSTRACTOBJECTIVE: Although disparities have been described in epilepsy care, their contribution to status epilepticus (SE) and associated outcomes remains understudied.METHODS: We used the 2010-2019 National Inpatient Sample to identify SE hospitalizations using International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM)/ICD-10-CM codes. SE prevalence was stratified by demographics. Logistic regression was used to assess factors associated with electroencephalographic (EEG) monitoring, intubation, tracheostomy, gastrostomy...

The effect of dexmedetomidine on the postoperative recovery of patients with severe traumatic brain injury undergoing craniotomy treatment: a retrospective study
ConclusionThe use of DEX in the ICU can effectively reduce the incidence of delirium in patients who return to the general ward after craniotomy. DEX had no adverse effect on the prognosis of patients other than causing bradycardia. (Source: European Journal of Medical Research)

Airway Management for Acute and Reconstructive Burns: Our 30-year Experience
Semin Plast Surg DOI: 10.1055/s-0044-1786008Airway management in both acute and reconstructive burn patients can be a major challenge for evaluation, intubation, and securing the airway in the setting of altered airway structure. Airway evaluation in both acute and reconstructive patients includes examination for evidence of laryngeal and supraglottic edema and structural changes due to trauma and/or scarring that will impact the successful approach to acquiring an airway for surgical procedures and medical recovery. The approach to acquiring a successful airway is rarely standard laryngoscopy and often requires fiberoptic...

Individual patient data meta-analysis on awake pediatric extracorporeal life support: Feasibility and safety of analgesia, sedation and respiratory support weaning, and physiotherapy
CONCLUSION: Awake ECLS strategy with active physiotherapy can be applied in children from neonatal age. Ambulation is also possible in selected cases. Complications related to such management were limited. Further studies on aECLS are needed to evaluate safety and efficacy of early physiotherapy and define patient selection.PMID:38652693 | DOI:10.1177/02676591241240377 (Source: Perfusion)
