ESO Launches 2018 ESO EMS Index to Track Nationwide EMS Performance Across Five Metrics
AUSTIN, Texas – ESO Solutions, Inc., the leading data and software company serving emergency medical services (EMS), hospitals and fire departments, today announced the findings of its inaugural report, the 2018 ESO EMS Index. The Index tracks performance of EMS agencies nationwide across five metrics: Stroke assessment and documentation, overdose events, end-tidal carbon dioxide (ETCO2) monitoring, 12-lead electrocardiogram (EKG) use and aspirin administration for chest pain. Data used for the Index are from January 1, 2017 through December 31, 2017. “There are changes on the horizon for EMS agencies across the countr...

Comparison of the Ecarin Chromogenic Assay and Diluted Thrombin Time for Quantification of Dabigatran Concentrations
ConclusionsFor detection of levels below 50 ng/mL both tests have specificities of at least 96%, suggesting that they accurately detect even low levels of drug. Therefore, regardless of whether a chromogenic or clot‐based platform is preferred, the STA‐ECA‐II and dTT are useful tests for measuring dabigatran concentrations. Unfortunately, neither test is licensed by the United States Food and Drug Administration. Although approved in other jurisdictions, the dTT and STA‐ECA‐II are not widely or rapidly available in most hospitals. Therefore, cooperation between regulators and hospitals is urgently needed to rende...

The Impact of Discrepancy between Measured versus Stated Weight on Hemorrhagic Transformation and Clinical Outcomes after Intravenous Alteplase Thrombolysis
Conclusion: More alteplase usage than actual weight led to higher hemorrhagic transformation. As one of the predictors for clinical deterioration, it is important to administrate alteplase based on an accurately measured weight.Cerebrovasc Dis 2017;44:241-247

Stroke warning: Overdosing on this supplement could cause brain bleeding
SUPPLEMENTS are widely used to boost a healthy diet, and vitamin E is often taken for its antioxidant benefits. However, in excess it can lead to a haemorrhagic stroke.

Supplement warning: Overdosing on this to lower high blood pressure could be deadly
SUPPLEMENTS popularly used include vitamin C, vitamin D3 and omega-3 - and potassium may also be taken to ward off high blood pressure, heart disease, stroke, arthritis, cancer, digestive disorders, and infertility. However, too much of the mineral could be deadly.

Stroke warning: Overdosing on THIS popular supplement could cause this side-effect
STROKE medication - known as anticoagulants - reduces your risk of developing a potentially-fatal blood clot. However, consuming too much omega-3, usually through supplements, can stop the drugs from working properly.

Non-Vitamin K antagonist Oral Anticoagulants: Impact of Non-adherence and Discontinuation.
Authors: Rivera-Caravaca JM, Esteve-Pastor MA, Roldán V, Marín F, Lip GYH Abstract INTRODUCTION: Non-Vitamin K Oral Anticoagulants (NOACs) are at least as effective as vitamin K antagonists (VKAs) reducing thromboembolism and mortality in atrial fibrillation (AF). These "fixed-dose regimen" drugs are characterized by not requiring routine monitoring or dosage adjustment. Stroke prevention with OAC is indicated in AF patients with CHA2DS2-VASc ≥2 (≥3 in females) and NOACs are recommended in preference to VKAs. However, underuse, premature discontinuation of treatment and non-adherence to guidelines is common, ...

The Medical Emergency Of Otto Warmbier
All that the doctors who treated Cincinnati, Ohio resident Otto Warmbier knew is what they had seen or maybe read in the news. They knew he had just been released on June 13 from imprisonment in North Korea where he had been held by for more than 17 months. He had been sentenced in March 2016 to 15 years of hard labor for allegedly removing a propaganda poster from a wall at a Pyongyang hotel where he had been staying. The University of Virginia honors student had been visiting the authoritarian state during a five-day trip with a group called Young Pioneer Tours, which is a group out of China – an important note. Ot...

Neighborhood Poverty and 9-1-1 Ambulance Contacts.
CONCLUSION: Our study suggests poverty is a positive, strong, and enduring predictor of ambulance contacts at the neighborhood level. The relationship between neighborhood poverty and ambulance utilization should be considered at multiple levels of EMS decision making. PMID: 28622073 [PubMed - as supplied by publisher]

Survival in Out-of-hospital Rapid Sequence Intubation of Non-Traumatic Brain Pathologies.
CONCLUSIONS: Non-traumatic brain pathologies are seven times more prevalent than traumatic brain injuries in patients that underwent out-of-hospital RSI in Victoria, Australia. Since the mechanisms through which RSI impacts mortality might differ between traumatic brain injuries and NTBP, and given that NTBP is very prevalent, it follows that the use of RSI in NTBP could be unsupported. Predicting the Need for Fluid Therapy —Does Fluid Responsiveness Work?
AbstractFluid overdose can be harmful in critically ill patients. Since central venous pressure (CVP) is currently considered to be an inappropriate indicator of preload, much attention is being given to predicting fluid responsiveness, i.e., the response of stroke volume (SV) or cardiac output (CO) to fluid challenge. However, when fluid responsiveness was evaluated in critically ill patients, including sepsis, only 40 –50% of the patients responded. Moreover, most fluid responders do not show significant hemodynamic improvement after fluid administration. In this review, we discuss why fluid responsiveness based on the...
