Pharmacokinetics of plasma infusion in congenital thrombotic thrombocytopenic purpura
ConclusionsFurther work is needed to compare treatment of congenital TTP with PI versus recombinant ADAMTS ‐13. PI may provide longer duration of ADAMTS‐13 effect, but is limited by plasma volume required, whereas recombinant therapy can provide a higher ADAMTS‐13 peak. We propose that investigation of interindividual clearance of ADAMTS‐13 is necessary to optimise treatment, to enable rationale for dose and frequency of prophylaxis. This article is protected by copyright. mTOR kinase inhibition reduces tissue factor expression and growth of pancreatic neuroendocrine tumors
ConclusionsTF isoforms are expressed in pNET. Sapanisertib suppresses TF mRNA and protein expression as well as TF co ‐factor activityin vitro andin vivo. Thus, further studies are warranted to evaluate the clinical utility of TF ‐suppressing mTORC1/2 inhibitor sapanisertib in pNET management. This article is protected by copyright. All rights reserved. High levels of coagulation factors and venous thrombosis risk: strongest association for factor VIII and von Willebrand factor
AbstractBackgroundCoagulation factors are essential for robust clot formation. However, elevated levels of procoagulant factors are associated with in increased risk for venous thrombosis. The precise contribution of these factors to the development of venous thrombosis (VT) is not yet understood.ObjectivesWe determined thrombosis risk for the highest levels of 8 selected coagulation factors. Furthermore, we analysed which of these coagulation factors had the strongest impact on the supposed association.MethodsWe used data of 2377 patients with a first VT and 2940 control subjects in whom fibrinogen, von Willebrand factor ...

The protease ‐activated receptor 4 Ala120Thr variant alters platelet responsiveness to low‐dose thrombin and protease‐activated receptor 4 desensitization, and is blocked by non‐competitive P2Y12 inhibition
ConclusionTheF2RL3 rs773902 SNP alters platelet reactivity to thrombin; the allelic effect requires P2Y12, and is not affected by gender. Ticagrelor blocks the enhanced reactivity of rs773902 A platelets. PAR4 encoded by the rs773902 A allele is relatively resistant to desensitization and may contribute to stroke risk. Natural killer cells induce neutrophil extracellular trap formation in venous thrombosis
Conclusions: Thrombus formation was reduced in Tbet‐/‐ and IFN γ‐/‐ mice, suggesting that Tbet/IFN γ expressing cells are required for venous thrombosis. The number of NETs formed during thrombosis was significantly lower in Tbet‐/‐ and IFN γ‐/‐ mice. NET formation was also decreased in WT mice treated with an IFN γ blocking antibody. Injection of recombinant IFNγ in IFNγ‐/‐ mice rescued the phenotype. Natural killer (NK) cells were specifically depleted prior to venous thrombosis induction. NK cell depletion results in decreased NET formation and smaller thrombi sugg...

Dimeric Fc γR ectodomains detect pathogenic anti‐platelet factor 4–heparin antibodies in heparin‐induced thromobocytopenia
ConclusionsThis study found the cell ‐free, function‐based dimeric rsFcγRIIa assay to be convenient, simple, and potentially predictive of HIT. The assay had improved specificity over the IgG ELISA, and correlated strongly with the AcuStar HIT IgG‐specific assay, warranting further evaluation of its potential to identify HIT in larger patient cohorts. Safety of D ‐dimer testing as a stand‐alone test for the exclusion of deep vein thrombosis as compared with other strategies
Essentials The aim of deep vein thrombosis (DVT) diagnostic work ‐up is to maximize both safety and efficiency. We explored whether D‐dimer is safe and efficient as a stand‐alone test to exclude DVT. Our findings suggest it is a safe, efficient and simplified diagnostic strategy. The safety of age‐adjusted D‐dimer as a stand‐alone test requires fur ther investigation.SummaryBackgroundSeveral strategies for safely excluding deep vein thrombosis (DVT) while limiting the number of imaging tests have been explored.ObjectivesTo determine whether D ‐dimer testing could safely and efficiently exclude DVT as a s...

Increased urokinase and consumption of α2‐antiplasmin as an explanation for the loss of benefit of tranexamic acid after treatment delay
ConclusionsTXA protects fibrin but stimulates uPA activity and slows inhibition of plasmin by α2‐antiplasmin. Plasmin proteolytic activity digests fibrinogen and disrupts coagulation, exacerbated when α2‐antiplasmin is consumed by ongoing fibrinolysis. Additional direct inhibition of plasmin by aprotinin may prevent development of coagulopathy and extend the useful time window of TXA treatment. This article is protected by copyright. ConclusionsA total of 172 patients with brain tumors were evaluated (42 DOAC and 131 LMWH). In the primary brain tumor cohort (N=67), the cumulative incidence ofanyICH was 0% in patients receiving DOACs versus 36.8% (95% CI, 22.3 ‐51.3%) in those treated with LMWH with amajorICH incidence of 18.2% (95% CI, 8.4 ‐31.0). In the brain metastases cohort (N=105), DOACs did not increase the risk ofanyICH relative to enoxaparin with an incidence of 27.8% (95% CI, 5.5 ‐56.7%) compared with 52.9% (95% CI, 37.4‐66.2%). ConclusionsThese results indicate that neither traditional fibrin clots, formed by the thrombin:FXIIIa pathway, nor atypical TGM2 ‐cross‐linked fibrin(ogen) contribute to experimental CCl4‐induced liver fibrosis.
