IL-6 is a predictive biomarker for stroke associated infection and future mortality in the elderly after an ischemic stroke.
CONCLUSIONS: These data suggest that IL-6 may be a key biomarker for predicting stroke associated infection and mortality in the first two years post stroke. PMID: 23872300 [PubMed - as supplied by publisher]

Application of A(2)DS(2) score for predicting post-stroke pneumonia in elderly patients.
CONCLUSION: The A(2)DS(2) score can be useful for predicting post-stroke pneumonia and for routine monitoring of high-risk elderly stroke patients in the clinical setting. PMID: 24273263 [PubMed - as supplied by publisher]

Risk of developing pleural empyema in patients with stroke: a propensity-matched cohort study
In conclusion, patients with stroke are at an elevated risk of developing pleural empyema. The risk is greater for those with hemor rhagic stroke than for those with ischemic stroke. The risk increases further for patients who undergo VP shunt implantation.

Complex interplay of multiple biological systems that contribute to post-stroke infections.
Abstract Stroke is a leading contributor of death and disability around the world. Despite its recognised debilitating neurological deficits, a devastating clinical complication of surviving stroke patients that needs more attention is infection. Up to half of the patients develop infections after stroke, and a high proportion of them will die as a direct consequence. Major clinical trials that examined preventive antibiotic therapy in stroke patients have demonstrated this method of prevention is not effective as it does not reduce incidence of post-stroke pneumonia or improve patient outcome. Additionally, retro...

Effects of Stroke Unit Care in Acute Ischemic Stroke Patient Ineligible for Thrombolytic Treatment.
Conclusion: The stroke unit increases the survival rate among stroke patient compared with the general medical ward and have less complication. PMID: 29911837 [PubMed - in process]

Technology for maintaining oral care after stroke: considerations for patient-centered practice.
CONCLUSION: The available device and technologies may help substantially with the accommodations needed for post-stroke oral care, improving the oral health of stroke survivors. Good oral health confers benefit to overall health and well-being and could enhance recovery and rehabilitation outcomes. Nonetheless, more research is necessary to demonstrate the feasibility and effectiveness of technology in stroke contexts. IMPLICATIONS FOR REHABILITATION Oral care may be challenging after stroke due to patient fatigue, hemiparesis, cognitive impairments, and other impaired body functions. Poor oral hygiene may impact stroke re...

A Prospective Stroke Register in Sierra Leone: Demographics, Stroke Type, Stroke Care and Hospital Outcomes
This study reports the demographics, stroke types, stroke care and hospital outcomes for stroke in Freetown, Sierra Leone. Methods: A prospective observational register recorded all patients 18 years and over with stroke between May 2019 and April 2020. Stroke was defined according to the WHO criteria. Pearson's chi-squared test was used to examine associations between categorical variables and unpaired t-tests for continuous variables. Multivariable logistic regression, to explain in-hospital death, was reported as odds ratios (ORs) and 95% confidence intervals. Results: Three hundred eighty-five strokes were registered, an...

The Timing of Stroke Care Processes and Development of Stroke Associated Pneumonia: A National Registry Cohort Study
ConclusionTimes from stroke onset and admission to certain key stroke care processes were associated with SAP. Understanding how timing of these care processes relate to SAP may enable development of preventive interventions to reduce antibiotic use and improve clinical outcomes.

Are beta blockers effective in preventing stroke associated infections? - a systematic review and meta-analysis
CONCLUSION: Due to the lack of robust evidence, this meta-analysis may not support the preventive effect of beta blockers on stroke associated infection. But beta blockers treatment may be associated with development of post-stroke urinary tract infection and stroke associated pneumonia in some stroke patients.PMID:35585021 | DOI:10.18632/aging.204086

Dysphagia in Patients with Acute Ischemic Stroke: Early Dysphagia Screening May Reduce Stroke-Related Pneumonia and Improve Stroke Outcomes
Background: Dysphagia is associated with poor outcome in stroke patients. Studies investigating the association of dysphagia and early dysphagia screening (EDS) with outcomes in patients with acute ischemic stroke (AIS) are rare. The aims of our study are to investigate the association of dysphagia and EDS within 24 h with stroke-related pneumonia and outcomes. Methods: Over a 4.5-year period (starting November 2007), all consecutive AIS patients from 15 hospitals in Schleswig-Holstein, Germany, were prospectively evaluated. The primary outcomes were stroke-related pneumonia during hospitalization, mortality, and disabilit...

Different clinical predictors of aspiration pneumonia in dysphagic stroke patients related to stroke lesion: A STROBE-complaint retrospective study
In conclusion, MMSE can be a clinical predictor of the occurrence of aspiration pneumonia in patients with supratentorial stroke. In contrast, total MBI score and combined results of a salivagram and VFSS can be clinical predictors of the occurrence of aspiration pneumonia in patients with infratentorial stroke.

Ischemic Stroke and Infection: A brief update on mechanisms and potential therapies
Biochem Pharmacol. 2021 Sep 17:114768. doi: 10.1016/j.bcp.2021.114768. Online ahead of print.ABSTRACTIschemic stroke triggers a multifaceted inflammatory response in the brain that contributes to secondary brain injury and infarct expansion. In parallel with brain inflammation, ischemic stroke also leads to post-stroke immunosuppression. Stroke-induced leukopenia then predisposes patients to opportunistic infections potentially leading to pneumonia or unrinary tract infections and a worsened stroke outcome. There is evidence that the hypothalamic-pituitary-adrenal axis plays an important role in the etiology of post-stroke...
